Apoplexy

Aphasia

Inability to speak caused by disease in or injury to the cerebral cortex in the left half of the BRAIN (in a right-handed person), affecting the generation and content of speech as well as the understanding of language; often accompanied by problems with reading and writing (see DYSPHASIA). Comprehension and expression of language occur in two zones of the cerebral cortex (the outer layer of the main part of the brain). They are known as Wernicke’s area (comprehension) and Broca’s area (speech formulation).... aphasia

Appendicitis

This is an in?ammatory condition of the APPENDIX, and is a common surgical emergency, affecting mainly adolescents and young adults. It is usually due to a combination of obstruction and infection of the appendix, and has a variable clinical course ranging from episodes of mild self-limiting abdominal pain to life-threatening illness. Abdominal pain beginning in the centre of the abdomen but which later shifts position to the right iliac fossa is the classic symptom. The patient usually has accompanying fever and sometimes nausea, vomiting, loss of appetite, diarrhoea, or even constipation. The precise symptoms vary with the exact location of the appendix within the abdomen. In some individuals the appendix may ‘grumble’ with repeated mild attacks which resolve spontaneously. In an acute attack, the in?ammatory process begins ?rst in the wall of the appendix but, if the disease progresses, the appendix can become secondarily infected and pus may form within it. The blood supply may become compromised and the wall become gangrenous. Eventually the appendix may rupture, giving rise to a localised abscess in the abdomen or, more rarely, free pus within the abdomen which causes generalised PERITONITIS. Rupture of the appendix is a serious complication and the patient may be severely unwell. Surgeons recognise that in order to make sure patients with appendicitis do not progress to peritonitis, a certain percentage of normal appendixes are removed when clinical signs are suspicious but not diagnostic of disease.

Treatment The best treatment is prompt surgical removal of the diseased appendix, usually with antibiotic cover. If performed early, before rupture occurs, APPENDICECTOMY is normally straightforward and recovery swift. If the appendix has already ruptured and there is abscess formation or free intra-abdominal pus, surgery is still the best treatment but postoperative complications are more likely, and full recovery may be slower.... appendicitis

Apex

The pointed portion of any organ which has a conical shape. The apex of each lung reaches about 3·5–5 cm (1••• or 2 inches) above the collar-bone into the neck. In health, the apex of the heart can be felt below the ?fth rib immediately inside the nipple.... apex

Apgar Score

A method of assessing at birth whether or not a baby requires resuscitation. The newborn is routinely assessed at 1 minute of age and again at 5 minutes, and a value of 0, 1 or 2 given to each of ?ve signs: colour, heart rate, muscle tone, respiratory (or breathing) e?ort, and the response to stimulation. A total score of 7 or more indicates that the newborn child is in excellent condition. An Apgar score of 5 or less at 15 or 20 minutes predicates an increased risk of subsequent CEREBRAL PALSY.... apgar score

Appendicectomy

Appendix

A term applied to the appendages of several hollow organs: for example, the larynx has two pouches called appendices, and the epiploic appendices are the tags of fat that hang from the exterior part of the large intestine. The commonest application, however, is to the vermiform appendix of the large intestine. This is a short, slim, blind-ended tube up to 10 cm long attached to the caecum (a pouch at the start of the large intestine). Its function is unknown, though it may once have had one in ancestral humans. It is, however, prone to in?ammation and infection (see APPENDICITIS).... appendix

Apnoea

A general term meaning the cessation of breathing. Apnoea is a medical emergency: death soon follows if breathing is not quickly restored (see APPENDIX 1: BASIC FIRST AID). Apnoea may be caused by an obstruction to the airway, for example by the tongue during general ANAESTHESIA, or by a disturbance of the mechanisms that control breathing. Rapid heavy breathing reduces the blood levels of carbon dioxide and can lead to a brief period of apnoea.

Neonatal apnoeic attacks may represent a serious emergency, being caused by prematurity, milk aspiration, heart failure, infection, HYPOXIA, HYPOGLYCAEMIA or HYPOCALCAEMIA. If stimulation of the baby does not immediately restore breathing, then bag-and-mask ventilation should be used.... apnoea

Apoptosis

This is a genetically controlled type of cell death. There is an orchestrated collapse of a cell (see CELLS), typi?ed by destruction of the cell’s membrane; shrinkage of the cell with condensation of CHROMATIN; and fragmentation of DNA. The dead cell is then engulfed by adjacent cells. This process occurs without evidence of the in?ammation normally associated with a cell’s destruction by infection or disease.

Apoptosis, ?rst identi?ed in 1972, is involved in biological activities including embryonic development, ageing and many diseases. Its importance to the body’s many physiological and pathological processes has only fairly recently been understood, and research into apoptosis is proceeding apace.

In adults, around 10 billion cells die each day

– a ?gure which balances the number of cells arising from the body’s stem-cell populations (see STEM CELL). Thus, the body’s normal HOMEOSTASIS is regulated by apoptosis. As a person ages, apoptopic responses to cell DNA damage may be less e?ectively controlled and so result in more widespread cell destruction, which could be a factor in the onset of degenerative diseases. If, however, apoptopic responses become less sensitive, this might contribute to the uncontrolled multiplication of cells that is typical of cancers. Many diseases are now associated with changed cell survival: AIDS (see AIDS/HIV); ALZHEIMER’S DISEASE and PARKINSONISM; ischaemic damage after coronary thrombosis (see HEART, DISEASES OF) and STROKE; thyroid diseases (see THYROID GLAND, DISEASES OF); and AUTOIMMUNE DISORDERS. Some cancers, autoimmune disorders and viral infections are associated with reduced or inhibited apoptosis. Anticancer drugs, GAMMA RAYS and ULTRAVIOLET RAYS (UVR) initiate apoptosis. Other drugs – for example, NONSTEROIDAL ANTI-INFLAMMATORY DRUGS (NSAIDS) – alter the process of apoptosis. Research is in train to harness new knowledge about apoptosis for the development of new treatments and modi?cations of existing ones for serious disorders such as cancer and degenerative nervous diseases.... apoptosis

Apraxia

Apraxia, or dyspraxia, is the loss of ability to make accurate skilled movements. The cause is a disorder of the cerebral cortex of the BRAIN: the patient is unable to coordinate his or her movements. Apraxia di?ers from clumsiness resulting from muscular weakness, loss of sensation or disease in the cerebellum (see BRAIN).

The condition is usually a consequence of disease in the brain’s parietal lobes, though frontal-lobe disease may cause it. A person with gait apraxia has normal power in the legs and no abnormal signs suggesting cerebellar disease, but cannot perform the normal act of walking because of malfunction in the cerebrum.... apraxia

Appetite

Appetite is the craving for the food necessary to maintain the body and to supply it with suf?cient energy to carry on its functions. The ultimate cause of appetite is a question of supply and demand in the muscles and various organs, but the proximate cause is doubtful. Unlike hunger, it is probably an acquired, rather than an inborn, sensation. Whatever other factors may be concerned, the tone of the STOMACH is of importance. Signi?cant factors in stimulating appetite are anticipation and the sight and smell of well-cooked food. Individuals who eat unsuitable substances such as faeces are described as suffering from pica, which occurs sometimes during pregnancy, in children, and often in mental disorders. The two chief disorders, however, are excessive increase of appetite, and diminution or loss of appetite (see also EATING DISORDERS).

Excessive appetite may simply be a bad habit, due to habitual over-indulgence in good food and resulting in GOUT, OBESITY, etc. – according to the other habits and constitution of the person. It may also be a sign of DIABETES MELLITUS or thyrotoxicosis (see under THYROID GLAND, DISEASES OF).

Diminished appetite is a sign common to almost all diseases causing general weakness, because the activity of the stomach and the secretion of gastric juice fail early when vital power is low. It is the most common sign of DYSPEPSIA due to gastritis, and of cancer of the stomach. In some cases it is a manifestation of stress or strain such as domestic worry or dif?culties at work. Indeed, appetite seems to be particularly susceptible to emotional disturbances, as is evidenced by the linked conditions of BULIMIA (pathological overeating) and anorexia nervosa (pathological dieting) – see also EATING DISORDERS.... appetite

Apocrine

Secretory glands, especially found in the armpit and groin, that secrete oily sweat derived from shed cell cytoplasm, and which contain aromatic compounds that possess emotional information for those nearby. Lacrimal Apparatus

The system that produces and drains tears. The lacrimal apparatus of the eye includes the main and accessory lacrimal glands and the nasolacrimal drainage duct. The main gland lies just within the upper and outer

margin of the eye orbit and drains on to the conjunctiva. It secretes tears during crying and when the eye is irritated. The accessory gland lies within the conjunctiva, and maintains the normal tear film, secreting it directly onto the conjunctiva. Tears drain through the lacrimal puncta, tiny openings towards the inner ends of the upper and lower eyelids. The puncta are connected by narrow tubes to the lacrimal sac, which lies within the lacrimal bone on the side of the nose. Leading from the sac is the nasolacrimal duct, which opens inside the nose.... lacrimal apparatus

Sleep Apnoea

A disorder in which there are episodes of temporary cessation of breathing (lasting 10 seconds or longer) during sleep.

People with sleep apnoea may not be aware of any problem during the night, but they may be sleepy during the day, with poor memory and concentration. Severe sleep apnoea is potentially serious and may lead to hypertension, heart failure, myocardial infarction, or stroke.

Obstructive sleep apnoea is the most common type and may affect anyone, but more often middle-aged men, especially those who are overweight. The most common cause is over-relaxation of the muscles of the soft palate in the pharynx, which obstructs the passage of air. Obstruction may also be caused by enlarged tonsils or adenoids. The obstruction causes snoring. If complete blockage occurs, breathing stops. This triggers the brain to restart breathing, and the person may gasp and wake briefly.

In central sleep apnoea, breathing stops because the chest and diaphragm muscles temporarily cease to work, probably due to a disturbance in the brain’s control of breathing. Causes include paralysis of the diaphragm and disorders of the brainstem. Snoring is not a main feature.People who are overweight may find losing weight helps.

Alcohol and sleeping drugs aggravate sleep apnoea.

In one treatment, air from a compressor is forced into the airway via a mask worn over the nose.

Night-time artificial ventilation may be needed.

Tonsillectomy, adenoidectomy, or surgery to shorten or stiffen the soft palate may be performed.... sleep apnoea

Aphanamixis Polystachya

Family: Meliaceae.

Habitat: The sub-Himalayas tracts, Sikkim, Assam, Bengal, western Ghats and the Andamans.

Ayurvedic: Rohitaka, Daadima- chhada, Daadima-pushpaka, Plihaghna. Tecoma undulata G. Don., Bignoniaceae, is also equated with Rohitaka.

Siddha/Tamil: Malampuluvan.

Action: Bark—strongly astringent, used in the diseases of the liver and spleen, and for tumours, enlarged glands. Seed oil—used in muscular pains and rheumatism. All parts of the plant exhibit pesticidal activity. Seed extract—antibacterial, antifungal.

An aqueous extract of the bark, when injected i.p. in normal guinea pigs, showed reduction in absolute lymphocyte count and an increase in spleen weight. The bark appears to be an effective immunosuppressive drug similar to prednisolone.

The stembark contains a limonoid, ammorinin and a saponin, poriferas- terol-3-rhamnoside.... aphanamixis polystachya

Plant Part Used: Stalk, leaves, roots, seeds.

Dominican Medicinal Uses: The stalks and leaves are traditionally eaten raw or taken as a juice for treating obesity, high blood pressure, high cholesterol, diabetes and menopausal hot flashes.

Safety: The stalks, leaf and root are widely consumed and generally considered safe. Cases of allergic reaction to the root have been reported. Plants infected with pink rot fungus can cause phototoxicoses.

Contraindications: Internal use of the seeds and essential oil are contraindicated during pregnancy (emmenagoge, abortifacient, uterine stimulating effects) and patients with renal disorders (potential kidney-irritating effect of oil).

Drug Interactions: Celery seeds and seed extract: anticoagulants, warfarin (risk of bleeding, drug potentiation); thyroxine (lowered T4 levels).

Laboratory & Preclinical Data: In vivo: anti-hyperlipidemic, anti-inflammatory, antinociceptive (plant extract); hepatoprotective (seeds).

In vitro: antimicrobial, antioxidant (plant extract); cercaricidal (essential oil); vasodilation (chemical constituent).

Habitat: Native to Europe; cultivated in northwestern Himalayas and in hills of Uttar Pradesh, Himachal Pradesh and southern India.

English: Celery.

Ayurvedic: Ajmodaa, Ajmoda, Ajmodikaa, Dipyaka.

Unani: Karafs.

Siddha/Tamil: Celery-keerai.

Folk: Ajmodaa.

Action: Anti-inflammatory (used in rheumatic disorders, inflammation of the urinary tract), diuretic, carminative, nervine, sedative, antiemetic, antispasmodic, antiseptic (used in bronchitis, asthma, as well as liver and spleen diseases), emmenagogue. Essential oil from seeds—tranquilizer, anticonvulsant, antifungal. Seeds are used in the treatment of chronic skin disorders including psoriasis.

Key application: As diuretic. (The British Herbal Pharmacopoeia.)

Celery yields an essential oil (3%), major constituent being d-limonene (50%) and phathalides and beta-seli- nene; coumarins, furanocoumarins (bergapten); flavonoids (apiin and api- genin). Alkaloid fraction of seeds showed tranquilizing activity in animals. The phthalides are sedative in mice and exhibit antiepileptic activity in rats and mice. The aqueous extract of the celery has been shown to reduce adjuvant-induced arthritis in rats, and to be hypotensive in patients as well as animals. The tincture of the plant exhibits drop in blood pressure accompanied by an increase in urine output.... apium graveolens

Approved Names For Medicines

The term used for names devised or selected by the British Pharmacopoeia Commission for new drugs. European Union law (1992) requires the use of a Recommended International Non-proprietary Name (rINN) for medicinal substances. In most cases the British Approved Name (BAN) and rINN were the same when the legislation was introduced; where there were di?erences, the BAN was modi?ed to meet the new requirements.

Pharmaceutical manufacturers usually give proprietary (brand) names to the drugs they develop, though doctors in the NHS are expected to prescribe using approved – nonproprietary or generic – titles. Most nonproprietary titles are those in the European Pharmacopoeia, British Pharmacopoeia Commission or the British Pharmaceutical Codex. Action: Leaves—applied hot in rheumatic pains. Smoked to relieve headache. Seed—paste used in stomatitis. Wood—paste used in mouth and tongue sores. Seeds and roots—employed as stomachic. Bark—used in rheumatism and diseases of genitourinary tract. The Ayurvedic Pharmacopoeia of India indicated the use of the fruit in emesis and giddiness.

The seeds and leaves contain cal- literpenone and its monoacetate; the former also contain fatty acids, beta- sitosterol and its beta-D-glucoside.

About the Nutrients in This Food Apples are a high-fiber fruit with insoluble cellulose and lignin in the peel and soluble pectins in the flesh. Their most important vitamin is vitamin C. What Happens When You Cook This Food When you cook an unpeeled apple, insoluble cellulose and lignin will hold the peel intact through all normal cooking. Cloudy apple juice is made simply by chopping or shredding apples and then pressing out and straining the juice. Medical Uses and/or Benefits As an antidiarrheal. The pectin in apple is a natural antidiarrheal that helps solidif y stool. Shaved raw apple is sometimes used as a folk remedy for diarrhea, and purified pectin is an ingredient in many over-the-counter antidiarrheals. Lower cholesterol levels. Soluble fiber (pectin) may interfere with the absorption of dietary fats, including cholesterol. The exact mechanism by which this occurs is still unknown, but one theory is that the pectins in the apple may form a gel in your stomach that sops up fats and cholesterol, carrying them out of your body as waste. Potential anticarcinogenic effects. A report in the April 2008 issue of the journal Nutrition from a team of researchers at the Universit y of Kaiserslautern, in Germany, suggests that several natural chemicals in apples, including but yrate (produced naturally when the pectin in apples and apple juice is metabolized) reduce the risk of cancer of the colon by nourishing and protecting the mucosa (lining) of the colon.

Adverse Effects Associated with This Food Intestinal gas. Extract of apricot pits, known medically as Laetrile, has been used by some alternative practitioners to treat cancer on the theory that the cyanide in amygdalin is released only when it comes in contact with beta-glucuronidase, an enzyme common to tumor cells. Scientifically designed tests of amygdalin have not shown this to be true. Laetrile is illegal in the United States.

In some studies with laboratory animals, dried apricots have been as effective as liver, kidneys, and eggs in treating iron-deficiency anemia. To keep them from turning brown as they dry, apricots may be treated with sulfur dioxide. This chemical may cause serious allergic reactions, including anaphylactic shock, in people who are sensitive to sulfites.

Sleep Apnoeas

A sleep apnoea is conventionally de?ned as the cessation of breathing for ten seconds or more. Apnoeas, which affect around 5 per cent of adults and are markedly more common in men, may occur as frequently as 400 times per night. They can be due to a failure of the physiological drive to breathe (central sleep apnoeas) but much more often are due to a transient obstruction of the airway between the level of the soft PALATE and the LARYNX (obstructive sleep apnoeas) when the airway dilator muscles over-relax. Any factor such as alcohol or sedative drugs that accentuates this, or that makes the airway narrower (such as obesity or large TONSILS), will tend to cause sleep apnoeas.

Vigorous respiratory movements are made to overcome the obstruction during each apnoea. These are associated with snoring and snorting noises. The apnoea ends with a mini-arousal from sleep. As a result, sleep becomes fragmented and sleep deprivation, manifested as sleepiness during the day, is common. This may result in accidents – for instance, at work or while driving – and sleep apnoea is also linked with an increased risk of STROKE, heart attacks and HYPERTENSION.

The diagnosis of sleep apnoea has recently been facilitated by linking specially designed software with ELECTROCARDIOGRAPHY performed during sleep, with minimal disturbance of the subject.

Description: Most wild apples look enough like domestic apples that the survivor can easily recognize them. Wild apple varieties are much smaller than cultivated kinds; the largest kinds usually do not exceed 5 to 7.5 centimeters in diameter, and most often less. They have small, alternate, simple leaves and often have thorns. Their flowers are white or pink and their fruits reddish or yellowish.

Habitat and Distribution: They are found in the savanna regions of the tropics. In temperate areas, wild apple varieties are found mainly in forested areas. Most frequently, they are found on the edge of woods or in fields. They are found throughout the Northern Hemisphere.

Edible Parts: Prepare wild apples for eating in the same manner as cultivated kinds. Eat them fresh, when ripe, or cooked. Should you need to store food, cut the apples into thin slices and dry them. They are a good source of vitamins.

Anaemia: Aplastic

Failure of the bone marrow to produce red cells because of infection, also neutropenia and depletion of platelets in the blood through chronic disease (TB etc) elsewhere in the body, or through chemicals in food and medicine. Other causes include food preservatives, X-ray radiation, fluoride in water supply and environmental pollution. The condition is serious.

Possibility of mercurial poisoning. A 59-year-old man employed filling thermometers with mercury developed aplastic anaemia and died. His urine contained 1.01mg mercury per litre. (D.R. Ryrie. Brit. Medical Journal, i/1970, 499. A similar report D.R. Wilson, ibid., ii/1966, 1534)

Symptoms. Headache, dizziness, pallor of skin, loss of weight and appetite, sore or burning tongue, jaundice, bruising, nose-bleeds. A low state of the immune system exposes the subject to infection. Treatment. Hospital supervision. Necessary to identify the causative toxic agent and eliminate it. Condition fails to respond to usual preparations of iron taken by mouth. No specific exists but supportive adaptogen herbs sustain and raise haemoglobin levels, marginally increasing red cells.

To facilitate elimination of toxic chemicals:– Teas: Alfalfa, Red Clover, German Chamomile, Ground Ivy, Milk Thistle, Gotu Kola, Nettles, Fennel.

Tea. Formula. Equal parts: Dandelion, Nettles, Alfalfa. 1 heaped teaspoon in each cup boiling water, infuse 10-15 minutes. One cup freely.

Decoction: Gentian – 1 teaspoon in cup cold water. Steep overnight; drink on rising.

Tinctures. To stimulate bone marrow. Formula: equal parts, Echinacea, Prickly Ash bark, Horsetail. Dose: 1-2 teaspoons thrice daily, taken in water or cup of one of the above teas.

Tincture Cinnamon BP (1949). Dose: 2-4ml (30-60 drops).

Powders. Combine: Gentian 1; Yellow Dock 1; Echinacea 2; Cinnamon quarter; Cayenne quarter. Dose: 500mg (two 00 capsules or one-third teaspoon), before meals.

Liquid Extracts: Combine Echinacea 2; Black Cohosh half; Damiana 1; Ginger quarter. Dose: 1 teaspoon in cup Red Clover tea, before meals.

Treatment of cancer with radiotherapy or anticancer drugs can temporarily interfere with the cell-producing ability of bone marrow, as can certain viral infections and other drugs. Long-term exposure to insecticides or benzene fumes may cause more persistent aplastic anaemia, and a moderate to high dose of nuclear radiation is another recognized cause. An autoimmune disorder is responsible in about half of all cases. Aplastic anaemia sometimes develops for no known reason.

A low level of red blood cells may cause symptoms common to all types of anaemia, such as fatigue and breathlessness. White-cell deficiency increases susceptibility to infections; platelet deficiency may lead to a tendency to bruise easily, bleeding gums, and nosebleeds.

The disorder is usually suspected from blood-test results, particularly a blood count, and is confirmed by a bone marrow biopsy.

Blood and platelet transfusions can control symptoms.

Immunosuppression is used to treat anaemia due to an autoimmune process.

Ulcer, Aphthous

A small, painful ulcer that occurs, alone or in a group, on the inside of the cheek or lip or underneath the tongue. Aphthous ulcers are most common between the ages of 10 and 40 and affect more women than men. The most severely affected people have continuously recurring ulcers; others have just 1 or 2 ulcers each year.

Each ulcer is usually small and oval, with a grey centre and a surrounding red, inflamed halo. The ulcer, which usually lasts for 1–2 weeks, may be a hypersensitive reaction to haemolytic streptococcus bacteria. Other factors commonly associated with the occurrence of these ulcers are minor injuries(such as at an injection site or from a toothbrush), acute stress, or allergies (such as allergic rhinitis). In women, aphthous ulcers are most common during the premenstrual period. They may also be more likely if other family members suffer from recurrent ulceration.

Analgesic mouth gels or mouthwashes may ease the pain of an aphthous ulcer.

Some ointments form a waterproof covering that protects the ulcer while it is healing.

Ulcers heal by themselves, but a doctor may prescribe a paste containing a corticosteroid drug or a mouthwash containing an antibiotic drug to speed up the healing process.... ulcer, aphthous

Apical Abscess

an infection in the bone around the apex of a tooth. An acute abscess is extremely painful and if left untreated may cause swelling of the jaw and sometimes the face. A chronic abscess may be completely asymptomatic. An abscess invariably results from damage to and infection of the pulp of the tooth. Treatment is drainage and *root canal treatment or extraction of the tooth; antibiotics may give temporary relief. Juxtaglomerular Apparatus

(JGA) a microscopic structure within the kidney that is important in regulating blood pressure, body fluid, and electrolytes. It is situated in each nephron, between the afferent arteriole of the glomerulus and the returning distal convoluted tubule of the same nephron. The JGA consists of specialized cells within the distal tubule (the macula densa), which detect the amount of sodium chloride passing through the tubule and can secrete locally acting vasoconstrictor substances that act on the associated afferent arteriole to induce a reduction in filtration pressure (tubuloglomerular feedback). Modified cells within the afferent arterioles secrete *renin in response to a fall in perfusion pressure or feedback from the macula densa and form a central role in the renin-*angiotensin-aldosterone axis. Obstructive Sleep Apnoea

(OSA, obstructive sleep apnoea syndrome, OSAS) a serious condition in which airflow from the nose and mouth to the lungs is restricted during sleep, also called sleep apnoea syndrome (SAS). It is defined by the presence of more than five episodes of *apnoea per hour of sleep associated with significant daytime sleepiness. Snoring is a feature of the condition but it is not universal. There are significant medical complications of prolonged OSA, including heart failure and high blood pressure. Patients perform poorly on driving simulators, and driving licence authorities may impose limitations on possession of a driving licence. There are associated conditions in adults, the *hypopnoea syndrome and the upper airways resistance syndrome, with less apnoea but with daytime somnolence and prominent snoring. In children the cause is usually enlargement of the tonsils and adenoids and treatment is by removing these structures. In adults the tonsils may be implicated but there are often other abnormalities of the pharynx, and patients are often obese. Treatment may include weight reduction or nasal *continuous positive airways pressure (nCPAP) devices, *mandibular advancement splints, or noninvasive ventilation. Pituitary Apoplexy

acute intrapituitary haemorrhage, usually into an existing tumour, presenting as severe headache and collapse. It is a medical emergency.
