It usually begins at puberty – although young children can be affected – and tends to stop in middle age: in women, for example, attacks often cease after MENOPAUSE. It frequently disappears during pregnancy. The disorder tends to run in families. In susceptible individuals, attacks may be provoked by a wide variety of causes including: anxiety, emotion, depression, shock, and excitement; physical and mental fatigue; prolonged focusing on computer, television or cinema screens; noise, especially loud and high-pitched sounds; certain foods – such as chocolate, cheese, citrus fruits, pastry; alcohol; prolonged lack of food; irregular meals; menstruation and the pre-menstrual period.
Anything that can provoke a headache in the ordinary individual can probably precipitate an attack in a migrainous subject. It seems as if there is an inherited predispostion that triggers a mechanism whereby in the migrainous subject, the headache and the associated sickness persist for hours, a whole day or even longer.
The precise cause is not known, but the generally accepted view is that in susceptible individuals, one or other of these causes produces spasm or constriction of the blood vessels of the brain. This in turn is followed by dilatation of these blood vessels which also become more permeable and so allow ?uid to pass out into the surrounding tissues. This combination of dilatation and outpouring of ?uid is held to be responsible for the headache.
Two types of migraine have been recognised: classical and common. The former is relatively rare and the headache is preceded by a slowly extending area of blindness in one or both eyes, usually accompanied by intermittent ‘lights’. The phenomenon lasts for up to 30 minutes and is followed by a bad, often unilateral headache with nausea, sometimes vomiting and sensitivity to light. Occasionally, passing neurological symptoms such as weakness in a limb may accompany the attack. The common variety has similar but less severe symptoms. It consists of an intense headache, usually situated over one or other eye. The headache is usually preceded by a feeling of sickness and disturbance of sight. In 15–20 per cent of cases this disturbance of sight takes the form of bright lights: the so-called AURA of migraine. The majority of attacks are accompanied by vomiting. The duration of the headache varies, but in the more severe cases the victim is usually con?ned to bed for 24 hours.
Treatment consists, in the ?rst place, of trying to avoid any precipitating factor. Patients must ?nd out which drug, or drugs, give them most relief, and they must always carry these about with them wherever they go. This is because it is a not uncommon experience to be aware of an attack coming on and to ?nd that there is a critical quarter of an hour or so during which the tablets are e?ective. If not taken within this period, they may be ine?ective and the unfortunate victim ?nds him or herself prostrate with headache and vomiting. In addition, sufferers should immediately lie down; at this stage a few hours’ rest may prevent the development of a full attack.
When an attack is fully developed, rest in bed in a quiet, darkened room is essential; any loud noise or bright light intensi?es the headache or sickness. The less food that is taken during an attack the better, provided that the individual drinks as much ?uid as he or she wants. Group therapy, in which groups of around ten migrainous subjects learn how to relax, is often of help in more severe cases, whilst in others the injection of a local anaesthetic into tender spots in the scalp reduces the number of attacks. Drug treatment can be e?ective and those a?icted by migraine may ?nd a particular drug or combination of drugs more suitable than others. ANALGESICS such as PARACETAMOL, aspirin and CODEINE phosphate sometimes help. A combination of buclizine hydrochloride and analgesics, taken when the visual aura occurs, prevents or diminishes the severity of an attack in some people. A commonly used remedy for the condition is ergotamine tartrate, which causes the dilated blood vessels to contract, but this must only be taken under medical supervision. In many cases METOCLOPRAMIDE (an antiemetic), followed ten minutes later by three tablets of either aspirin or paracetamol, is e?ective if taken early in an attack. In milder attacks, aspirin, with or without codeine and paracetamol, may be of value. SUMATRIPTAN (5-hydroxytryptamine [5HT1] AGONIST – also known as a SEROTONIN agonist) is of value for acute attacks. It is used orally or by subcutaneous injection, but should not be used for patients with ischaemic heart disease. Naratriptan is another 5HT1 agonist that is an e?ective treatment for acute attacks; others are almotriptan, rizariptan and zolmitriptan. Some patients ?nd beta blockers such as propranolol a valuable prophylactic.

Strangulation of a person’s neck, either with a ligature or with the hands, obstructs the jugular veins in the neck, preventing the normal out?ow of blood from the brain and head. The TRACHEA is also compressed, cutting o? the supply of air to the lungs. The combination of these effects leads to HYPOXIA and damage to the brain. If not quickly relieved, unconsciousness and death follow. Strangulation may be deliberate or accidental – the latter being a particular hazard for children, for example, when playing with a rope. closer than 6 metres away, the rays of light from this near object will be diverging by the time they reach the eye. In order to focus these diverging beams of light, the refracting power of the lens must increase. In other words the lens must accommodate.
The lens loses its elasticity with age, and thus becomes less spherical when tension in the zonule relaxes. This results in an increased longsightedness (presbyopia) requiring reading glasses for correction. (See AGEING.)... accommodation
For long it had been realised that in certain cases of ASTHMA, adrenaline had not the usual bene?cial e?ect of dilating the bronchi during an attack; rather it made the asthma worse. This was due to its acting on both the alpha and beta adrenergic receptors. A derivative, isoprenaline, was therefore produced which acted only on the beta receptors. This had an excellent e?ect in dilating the bronchi, but unfortunately also affected the heart, speeding it up and increasing its output – an undesirable e?ect which meant that isoprenaline had to be used with great care. In due course drugs were produced, such as salbutamol, which act predominantly on the beta2 adrenergic receptors in the bronchi and have relatively little e?ect on the heart.
The converse of this story was the search for what became known as BETA-ADRENOCEPTORBLOCKING DRUGS, or beta-adrenergic-blocking drugs. The theoretical argument was that if such drugs could be synthesised, they could be of value in taking the strain o? the heart – for example: stress ? stimulation of the output of adrenaline ? stimulation of the heart ? increased work for the heart. A drug that could prevent this train of events would be of value, for example in the treatment of ANGINA PECTORIS. Now there is a series of beta-adrenoceptor-blocking drugs of use not only in angina pectoris, but also in various other heart conditions such as disorders of rhythm, as well as high blood pressure. They are also proving valuable in the treatment of anxiety states by preventing disturbing features such as palpitations. Some are useful in the treatment of migraine.... adrenergic receptors

In experimental amoebiasis of rats, the root powder and ethanolic extract showed amoebicidal activity against Entamoeba histolytica.An alcoholic extract of the roots and essential oil from it, were found to inhibit passive cutaneous anaphylaxis in the mouse and rat.The plant produces the labdane diterpenoid, forskolin in its tuberous roots.Forskolin was discovered during a screening of medicinal plants by Central Drug Research Institute, Luc- know, India, in 1974. (Planta Medica, 1985, 51, 473-477.) The screening revealed the presence of a hypoten- sive and spasmolytic principle, named coleonol (later the name was changed to forskolin). The basic mechanism of forskolin is the activation of an enzyme, adenylate cyclase, which increases the amount of cyclic adeno- sine monophosphate (cAMP) in cells. Raised intracellular cAMP level exhibits following physiological effects : inhibition of platelet activation and degranulation; inhibition of mast cell degranualation and histamine release; relaxation of the arteries and other smooth muscles; increased insulin secretion; increased thyroid function; increased lipolysis.Forskolin, in clinical studies, reduced intraocular pressure when it was applied to the eyes for treating glaucoma. It has been shown to be a direct cerebral vasodilator. It has also been studied as a possible bron- chodilator (in the treatment of asthma) and has been shown to effectively reverse methacholine-induced broncho constriction in extrinsic asthmatics.Standardized Coleus extracts containing forskolin (18% in 50 mg) find application in weight-loss programmes. Aescin. Compound isolated from Horse-chestnuts to decrease capillary permeability and swelling. Topical. Decoction of Horse-chestnuts as a lotion. Or: infusion of Bladderwrack.
Aromatherapy and Herb essences. Combination for external use. Ingredients: Almond oil 47ml; Fennel oil 1ml; Juniper oil 1ml; Cypress essence 0.5ml; Lemon essence 0.5ml. Apply to affected areas morning and evening; small area 5 drops, large area 10 drops (Gerard). Gentle massage with a string glove, loofah or massage glove.
Diet. Reduce calorie intake. Raw fresh fruits and vegetable salads to account for 50 per cent of the diet. No sweet or dried fruits. Conservatively-cooked vegetables. Seafood. Iodine-rich foods. Wholegrain cereals. Protein: beans, chicken, poached eggs, fish, little lean meat: no pork, bacon or ham. Low-fat yoghurt. Cold-pressed unsaturated oils for salad dressings with lemon juice. ASTHMA. Spasmodic contraction of the bronchi following exercise, emotional tension, infection, allergens, pollens, house dust, colds.
Symptoms. Obstruction of airways with wheezing, rales or whistling sounds with a sense of constriction. Often related to eczema – ‘eczema of the epithelium’. Infantile eczema treated with suppressive ointments may drive the condition ‘inwards’ and worsen asthma. “My son’s eczema has got better, but he now has asthma” is a common observation.
Causes: hypersensitivity to domestic animals, horses and pet birds. Common salt. Red or white wine allergy. An older generation of practitioners recognised a renal-bronchial asthma encouraged by faulty kidney function. With addition of a relaxing diuretic (Dandelion, Buchu or Parsley root) to a prescription, respiratory symptoms often abate.
Broncho-dilators such as Ephedra and Wild Thyme are widely used by the practitioner. To relieve spasm: Lobelia, Pleurisy root. White Horehound, Ammoniacum, Cramp bark, Garlic, Grindelia, Hyssop.
Anti-cough agents serve to remove sticky sputum: Coltsfoot, Garden Thyme, Slippery Elm bark, Maidenhair Fern, Linseed, Bayberry bark.
For the chronic asthmatic, bacterial invasion spells distress, when Echinacea or Balm of Gilead should be added. Where an irregular pulse reveals heart involvement, add: Hawthorn or Lily of the Valley.
Lobelia is of special value for the anxious patient with spasm of the bronchi. Should be tried before resorting to powerful spray mists which frequently produce gastro-intestinal disturbance.
Alternatives. Teas. Coltsfoot, Comfrey, Horehound (White), Mullein, Skullcap, Marshmallow, Thyme, Valerian, Wild Cherry bark, Elecampane, Plantain. Formula: equal parts herbs Coltsfoot, Mullein, Valerian. 1-2 teaspoons to each cup boiling water; infuse 10-15 minutes; dose, 1 cup twice daily and when necessary.

Practitioner. Ephedra, Lobelia, Gelsemium, Grindelia, Euphorbia (pill-bearing spurge), Skunk Cabbage, Senega, Pulsatilla, Lily of the Valley (cardiac asthma), Thyme. Formula. Equal parts, Tincture Lobelia simp; Tincture Belladonna; Tincture Ephedra. 5-10 drops thrice daily (maintenance), 10-20 drops for spasm.
Cockayne, Ernest, FNIMH. Hyssop tea for children throughout childhood to avoid respiratory disorders. Dr Finlay Ellingwood. Gelsemium 3.5ml; Lobelia 3.5ml. Distilled water to 120ml. One 5ml teaspoon in water every 3 hours.
Dr Alfred Vogel. Ephedra 20 per cent; Ipecac 15 per cent; Hawthorn berry 10 per cent; Blessed Thistle 5 per cent; Burnet Saxifrage 5 per cent; Garden Thyme 5 per cent; Grindelia 1 per cent. 10-15 drops in water thrice daily.
Dr Wm Thomson. 1 teaspoon Ephedra herb to cup boiling water; infuse 10-15 minutes. Half-1 cup 2-3 times daily.
Traditional. 2 teaspoons shredded Elecampane root in cup cold water; stand overnight. Next day, heat to boiling point when required. Strain. Sips, hot, with honey: 1 cup 2-3 times daily.
Potter’s Asthma & Bronchitis Compound 32. 40g medicinal teabags. Ingredients: Clove BPC 4.84 per cent; Elecampane root 17.24 per cent; Horehound 26.20 per cent; Hyssop 17.24 per cent; Irish Moss 17.24 per cent; Liquorice 17.24 per cent. Dose: 1-2 teaspoons when necessary.
Chinese Medicine. Decoction or extract from the Gingko tree widely used, as also is Ephedra, Garlic, Liquorice and Bailcalensis.
Tablets/capsules. Lobelia. Iceland Moss, (Gerard). Euphorbia (Blackmore).
Powders. Formula. Lobelia 2; Hyssop 1; Elderflowers 1; Grindelia quarter; Liquorice quarter: pinch Cayenne. Dose: 750mg (three 00 capsules or half a teaspoon) 2-3 times daily.
Aromatherapy. 6 drops Rosemary oil in 2 teaspoons Almond oil for massage upper chest to relieve congestion.

Nebulizer. A germicidal solution is made from 5 drops oil Eucalyptus in one cup boiling water. Use in nebulizer for droplet therapy.

Supportives. Yoga. Singing. Cures have been reported of patients on taking up singing. “During singing, up to 90 per cent of the vital capacity may be used without a conscious effort to increase tidal volume.” (Dr M. Judson, New England Journal of Medicine)
Diet. Low salt, low fat, high fibre, cod liver oil, carrots, watercress, Soya beans or flour, lecithin, sunflower seed oil, green vegetables, raw fruit, fresh fish. These foods are valuable sources of antioxidant vitamins and minerals essential for the body’s defence mechanism. A diet deficient in these reduces ability of the airways to withstand the ravages of cigarette smoke and other air pollutants.
Foods that are craved are ones often causing sensitivity. Among problem foods are: milk, corn, wheat, eggs, nuts, chocolate, all dairy products, fat of meats. Check labels for tartrazine artificial colouring.
Salt intake. Linked with chest diseases. “Those who eat a lot of salt had more sensitive airways than those with low salt intake . . . excess salt tended to cause most pronounced symptoms.” (Institute of Respiratory Diseases, Oavia, Italy)
Asthma mortality could be significantly reduced by sufferers lowering their salt consumption, an epidemiologist predicted.

Acute. Eye is brick red and brick hard. Agonisingly painful, vision much reduced, pupil dilated and oval, the cornea steamy and the iris greenish, sees rainbow rings around lights, misty vision, pain in head and eyes, colours appear dull, can read for only short periods, unable to walk confidently downstairs, damage to retina and optic nerve from build-up of fluid.
Etiology: Damage from past inflammations, high blood pressure, steroids, stress, diet deficiencies, injury. Develops more in far-sighted people.
Ocular emergency requiring immediate hospital specialist treatment. If admission to hospital is delayed Pilocarpine may save the day: 1 drop of 1 per cent solution to each eye to constrict the pupil and open the drainage angle. This lasts 4-5 hours. Apply 1 drop 4 times every 24 hours. In the absence of Pilocarpine, a practitioner may prescribe Tincture Gelsemium BPC 1963, 5 drops in water not more than thrice daily.
A history of eyelids that are stuck down in the mornings reveals blockage from inflammatory exudate, tension rises and may precipitate glaucoma.
Chronic (gradual and long-continued). Usually in the elderly. Sometimes genetic. Chronic rise in painless intra-ocular pressure arrests blood supply to the optic discs thus disrupting bundles of retinal nerve fibres. ‘Deeply cupped discs’. Condition usually unsuspected. A sight destroyer.
Symptoms: bumping into objects and people. As above.
Treatment. Surgical drainage incision through the iris relieves tension. The object is to contract the pupil and focussing (ciliary) muscle which promotes the escape of watery fluid from the eye. Agents which contract the pupil are Pilocarpine, Adrenalin. Promotion of the body’s own supply of Adrenalin is mildly assisted by Ginseng. All cases should receive Echinacea to enhance resistance. Herbal medicine often stabilises the condition, with remedies such as Pulsatilla.
Alternatives:– Maintenance anti-inflammatory. Tea: fresh or dried herbs. Equal parts: Nettles, Marigold petals, Horsetail. Mix. 2 teaspoons to each cup boiling water; infuse 15 minutes. Dose: half-1 cup thrice daily.
Traditional. It was common practice in the South of France to douche the eye with dilute lemon juice, doubtless because Vitamin C has an osmotic effect, drawing away fluid.
Rutin (Buckwheat). 20mg thrice daily. Tablets, powder, etc.
Canasol. A non-hallucinogenic alkaloid of the marijuana plant (cannabis) has been used with success. Blood Tonics. See entry. Healthy blood contributes to healthy eyes and common blood tonics have been responsible for some cures in the early stages.
Bilberries. Mr Eric Wright suffered from glaucoma for many years. At 74 he was nearly blind, walked with a white stick, and couldn’t read or write. Improvement was impressive after taking Bilberry extract. His specialist agreed that his sight was at its best in three years since surgery to reduce intra-ocular pressure.
Diet. Begin 3-day fast, followed by 3 days on fruit and vegetable juices. Wholefoods thereafter. Increase protein intake. Repeat fast every 3 months. Fresh Bilberries as desired. Dr Rolf Ulrich links coffee with glaucoma. (Clinical Physiology)
Supplements. Daily. Vitamin A 7500iu, Vitamin B1 15mg, Vitamin B2 10mg, Vitamin B6 10mg, Vitamin C 3g, Vitamin E 500mg, Zinc.
Notes. Stress automatically raises intra-ocular pressure for which relaxation techniques are indicated. Tobacco worsens by causing constriction of blood vessels supplying the optic nerve. Abstain alcohol. Glaucoma becomes more prevalent in an ageing population. Patients with a strong history and with high blood pressure and diabetes should be screened.

Causes: eating too fast, carbonated drinks, stomach irritation (hot peppers, vinegars, alcohol). Where persistent, there may be constriction of the lower gullet by early neoplasm caused by drinking piping-hot tea when X-ray and specialist advice should be sought.

Main causes of a raised pressure include increase in blood thickness, kidney disorder or loss of elasticity in the arteries by hardening or calcification.
Well defined physical problems account for 10 per cent of high blood pressure cases. By the age of 60, a third of the peoples of the West are hypertensive. Other causes: genetic pre-disposition, endocrine disorders such as hyperactive thyroid and adrenal glands, lead and other chemical poisoning, brain tumour, heart disorder, anxiety, stress and emotional instability.
Other causes may be food allergies. By taking one’s pulse after eating a certain food one can see if the food raises the pulse. If so, that food should be avoided. Most cases of high blood pressure are related to lifestyle – how people think, act and care for themselves. When a person is under constant stress blood pressure goes up. It temporarily increases on drinking the stimulants: alcohol, strong tea, coffee, cola and caffeine drinks generally.
Symptoms. Morning headache (back of the head), possible palpitation, visual disturbances, dizziness, angina-like pains, inability to concentrate, nose-bleeds, ringing in the ears, fatigue, breathlessness (left ventricular failure).
Dr Wm Castelli, Director of the Framlingham Heart Study in Massachusetts, U.S.A., records: “The greatest risk is for coronary heart disease (CHD). Hypertensives have more than double the risk of people with normal blood pressure and seven times the risk of strokes.”
In countries where salt intake is restricted, a rise in blood pressure with age is not seen.
Simple hypotensive herbs may achieve effective control without the side-effects of sleep disturbance, adverse metabolic effects, lethargy and impaired peripheral circulation.
Essential hypertension is where high blood pressure is not associated with any disease elsewhere; it accounts for 90 per cent cases. Most of the remainder have kidney disease except for a few other abnormalities.
Alternatives. Balm, Black Haw, Black Cohosh (blood pressure of the menopause), Cactus, Cramp bark, Chamomile (German). Garlic, Buckwheat, Lily of the Valley, Balm, Mistletoe, Motherwort. Passion flower, Nettles, Lime flowers, Wood Betony, Yarrow, Rosemary, Hawthorn flowers, Olive leaves, Dandelion. Where there is nerve excitability: Valerian.
Tea No 1. Equal parts: Hawthorn leaves and flowers, Mistletoe, Lime flowers. Mix. 2 teaspoons to each cup boiling water; infuse 5-10 minutes. 1 cup 2-3 times daily. Alternative:–
Tea No 2. Equal parts: Nettles, Lime flowers, Yarrow, Passion flower. Mix. 2 teaspoons to each cup boiling water; infuse 5-10 minutes. 1 cup 2-3 times daily.
Nettles. Nettle tea is capable of removing cholesterol deposits (“fur”) from artery walls, increasing their elasticity. Like so many herbs they are rich in chlorophyll. The tea may be made as strong as desired. Mistletoe. 2-3 teaspoons cut herb (fresh or dried) to cup cold water. Allow to infuse overnight (at least 8 hours). 1 cup morning and evening.
Garlic. Juice from one Garlic corm expressed through a juicer taken morning and evening. Garlic dilates blood vessels. Alternative: 2-3 Garlic capsules at night.

Practitioner Formula. Tinctures: equal parts: Lily of the Valley, Mistletoe, Valerian. Dose: 30-60 drops thrice daily.
Where high blood pressure is due to faulty kidney function diuretics such as Dandelion or Bearberry will be added according to individual requirements. Dandelion root is one of the most widely-used potassium-conserving agents for increasing flow of urine, as well as being a mild beta-blocker to reduce myocardial infarction. Broom (Sarothamnus scoparius) (diuretic) is not used in cases of high blood pressure. It is good practice to assess kidney function in all new cases of hypertension for renal artery stenosis.
Evidence from two major studies confirms that diuretics rather than beta-blockers should be the treatment of choice for most elderly hypertensives. The addition of a diuretic (Yarrow, etc) to prescriptions for the elderly is commended.

Liver oil. Results from studies at the University of Munich, Germany, show that when an ounce of Cod Liver oil was added to the typical Western diet, better pressure readings and lower cholesterol levels followed. When the flavour renders it objectionable to the palate, taste may be masked by stirring briskly into fruit juice.
General. Stop smoking. Watch weight. Moderate exercise. Avoidance of stress by relaxation, yoga, music, etc. These relieve constriction of peripheral blood vessels. Curb temper
Diet. Avoid processed and fast foods high in fat and salt, and empty calories. Cheese and meat sparingly. Eat plenty of natural foods. Positively reject coffee, strong tea and alcohol. “There is a significant drop in plasma Cortisol with a fall in blood pressure after stopping alcohol.” (Dr J.F. Potter, University of Birmingham, England) It is well-documented that a vegetarian diet is associated with a lower blood pressure.
Salt. The association of salt with blood pressure is larger than generally appreciated and increases with age and initial blood pressure. Even a small reduction in salt (3g) may reduce a systolic and diastolic pressure by 5mmHg and 2.5mmHg respectively. All processed foods containing salt should be avoided. Supplementation. Inositol, zinc, Vitamin C, Vitamin B6. (Dr C. Pfeiffer) Vitamin E to improve circulation. Check with practitioner pressure level before starting 200iu increasing to 400iu daily. Magnesium: 300mg daily. Choline.

Etiology. Obscure; though cases may be traced to auto-toxaemia, Vitamin B deficiency, menstruation, malaria drugs (chloroquine).
Symptoms: dizziness, nausea, vomiting, tinnitus, sound distortions, heavy sweating, loss of hearing; usually in one ear only. Early diagnosis essential for effective treatment. This may mean reference to a department of otolaryngology or otoneurology.
Treatment. Antispasmodics. Nervines. Sometimes a timely diuretic reduces severity – Uva Ursi, Dandelion root, Wild Carrot.
Alternatives. Current European practice: Betony, German Chamomile, Passion flower, Hawthorn, Hops, Feverfew, White Willow.
Tea. Combine, equal parts: Valerian, Wild Carrot, Agrimony. 2 teaspoons to each cup boiling water; infuse 15 minutes. Half-1 cup every 2 hours during attack; thrice daily thereafter.
Decoction. Mistletoe: 2 teaspoons to each cup cold water steeped overnight. Bring to boil. Allow to cool. Half-1 cup, as above.
Tablets/capsules. Feverfew, Mistletoe, Prickly Ash.
Formula. Ginkgo 2; Dandelion 1; Black Cohosh 1. Dose: Liquid Extracts: 1 teaspoon. Tinctures: 2 teaspoons. Powders: 500mg (two 00 capsules or one-third teaspoon). Thrice daily.

The waste products in bile include the pigments bilirubin and biliverdin, which give bile its greenish-brown colour; bile salts, which aid in the breakdown and absorption of fats; and cholesterol. Bile passes out of the liver through the bile ducts and is then concentrated and stored in the gallbladder. After a meal, bile is expelled and enters the duodenum (the first section of the small intestine) via the common bile duct. Most of the bile salts are later reabsorbed into the bloodstream to be recycled by the liver into bile. Bile pigments are excreted in the faeces. (See also biliary system; colestyramine.)
bile duct Any of the ducts by which bile is carried from the liver, first to the gallbladder and then to the duodenum (the first section of the small intestine). The bile duct system forms a network of tubular canals. Canaliculi (small canals) surround the liver cells and collect the bile. The canaliculi join together to form ducts of increasing size. The ducts emerge from the liver as the 2 hepatic ducts, which join within or just outside the liver to form the common hepatic duct. The cystic duct branches off to the gallbladder; from this point the common hepatic duct becomes the common bile duct and leads into the duodenum.

bile duct cancer See cholangiocarcinoma. bile duct obstruction A blockage or constriction of a bile duct (see biliary system). Bile duct obstruction results in accumulation of bile in the liver (cholestasis) and jaundice due to a buildup of bilirubin in the blood. Prolonged obstruction of the bile duct can lead to secondary biliary cirrhosis. The most common cause of obstruction is gallstones. Other causes include a tumour affecting the pancreas (see pancreas, cancer of), where the bile duct passes through it, or cancer that has spread from elsewhere in the body. Cholangiocarcinoma (cancer of the bile ducts) is a very rare cause of blockage. Bile duc.
obstruction is a rare side effect of certain drugs. It may also be caused by cholangitis (inflammation of the bile ducts), trauma (such as injury during surgery), and rarely by flukes or worms.
Bile duct obstruction causes “obstructive” jaundice, which is characterized by pale-coloured faeces, dark urine, and a yellow skin colour.
There may also be itching.
Other symptoms may include abdominal pain (with gallstones) or weight loss (with cancer).
Treatment depends on the cause, but surgery may be necessary.

Coagulation disorders are usually due a deficiency of or abnormality in the enzymes (coagulation factors) involved in blood clotting. Defects may be congenital or acquired later in life. The
main congenital coagulation defects are von Willebrand’s disease, haemophilia, and Christmas disease.
Acquired defects of coagulation factors may develop at any age due to severe liver disease, digestive system disorders that prevent the absorption of vitamin K (needed to make certain coagulation factors), or the use of anticoagulant drugs. Disseminated intravascular coagulation (DIC) is an acquired disorder that is both complex and serious. It may be the result of underlying infection or cancer. In this condition, platelets accumulate and clots form within small blood vessels; coagulation factors are used up faster than they can be replaced, and severe bleeding may result.
Coagulation disorders are treated by replacement of the missing factor, factors extracted from fresh blood, or fresh frozen plasma. Genetically engineered factors may be used. Anticoagulants are sometimes used to suppress excess clotting activity in.
Thrombocytopenia, which results from insufficient platelets in the blood, produces surface bleeding into the skin and gums and multiple small bruises. Platelet defects may be inherited, associated with the use of certain drugs (including aspirin), or a complication of certain bone marrow disorders such as myeloid leukaemia. Treatment consists of platelet transfusions. Rarely, abnormal bleeding is caused by a bloodvessel defect or scurvy. Elderly people and patients on long-term courses of corticosteroid drugs may suffer mild abnormal bruising due to loss of skin support to the smallest blood vessels.

FAMILY: Lamiaceae
SYNONYMS: Mesosphaerum suaveolens, mint weed, bush mint, bush tea, pignut, chan, wild spikenard.
GENERAL DESCRIPTION: An erect, annual herb up to 3 metres high, belonging to the mint family, with a hollow stem, and bearing purple-blue lipped flowers and dark brown, shield-shaped fruits. The oval leaves, which are wrinkled along the edge and pointed, have a strong mint-like aroma when crushed; the taproot is white or brown. It is a very well known pseudo-cereal plant in the Latin America region.
DISTRIBUTION: A native of tropical America, growing in warm and semi-warm regions, commonly found in the rain forests of Ecuador, as well as in the Caribbean and Hawaii; it has also been naturalized in Australia since the mid-nineteenth century. This herb is commonly found alongside roadsides and watercourses and is considered a weed in many tropical areas around the world, especially because the plant can be invasive.
OTHER SPECIES: The mint family is generally known to produce the most variation in essential oil composition and therefore there are many chemotypes of H. suaveolens. Ocotea essential oil from the flowers of the Ocotea quixos tree, which is native to the deep Amazon, is another species found in South America, which yields a little-known essential oil sharing many of phytochemical properties and functions of H. suaveolens.
HERBAL/FOLK TRADITION: Dorado Azul has been used by many indigenous cultures in India, Nigeria and elsewhere for hundreds of years to treat bacterial infections, diarrhoea, tumours, skin diseases, gastro-intestinal disorders and as a natural mosquito repellent and pesticide. The herb has also traditionally been used for the treatment of asthma, hormonal balance (oestrogen), inflammation, muscular aches and pains and for wound healing.
Today the essential oil of Dorado Azul is recognized largely for its role in treating respiratory conditions and in supporting the sinuses and lungs, much like mint and eucalyptus. In addition to its respiratory benefits, Dorado Azul oil is also proving to be a very effective ingredient in essential oil blends that repel insects and pests. Indeed, recent studies have confirmed it is effective as a natural crop and grain insecticide. A study in 1999 revealed that the oil was effective against both gram positive (Bacillus cereus and Staphylococcus aureus) and negative (E. coli, Pseudomonas spp.) bacterial strains; as well as Candida albicans. A further study exhibited broad-spectrum antibacterial and antifungal activity against Aspergillus niger and Micrococcus luteus respectively.
ACTIONS: Analgesic, anti-inflammatory, antifungal, antimicrobial, antioxidant, anticancer, insecticidal, anti-infectious, expectorant, immune stimulant.
EXTRACTION: Essential oil distilled from the stems, leaves and flowers.
CHARACTERISTICS: The essential oil is a reddish liquid with a fresh-herbaceous odour. It blends well with peppermint, eucalyptus, ravintsara, tea tree, myrtle, balsam and spice oils.
PRINCIPAL CONSTITUENTS: Main constituents are sabinene (16.5 per cent), trans-?-bergamotene and ?-caryophyllene (19.8 per cent), terpinen-4-ol (9.6 per cent) and ?-pinene (8.6 per cent). Another study (using a different chemotype) reported sabinene (30 per cent), terpinen-4-ol (11.4 per cent), terpinolene (5.6 per cent), 1,8-cineole (5.2 per cent), ?-pinene (4.4 per cent) and ?-terpinene (4.2 per cent).
SAFETY DATA: No reported contra-indications: however since there are many variable chemotypes of this oil, the safety data will depend on the specific chemical make-up of the individual oil.
