Anamu Tea

Cymbopogon Citratus

(DC.) Stapf.

Synonym: Andropogon citratus DC.

Family: Poaceae.

Habitat: Grown in Punjab, Maharashtra, Gujarat and Karnataka.

English: Lemongrass.

Ayurvedic: Bhuutika, Bhuutikaa.

Action: Leaf—stimulant, sudorific, antiperiodic, anticatarrhal. Essential oil—carminative, anticholerin, depressant, analgesic, antipyretic, antibacterial, antifungal.

The lemongrass contains a volatile oil, with citral (about 70%), citronellal, geraniol and myrcene as its main constituents. Cetral and citronellal exhibit marked sedative activity.

The lemongrass is taken as a tea for digestive problems; it relaxes muscles of the stomach and gut, relieves spasm and flatulence. In catarrhal conditions, it is taken as a febrifuge.

An infusion of fresh leaves on oral administration has been found to produce dose-dependent analgesia in rats. This analgesic acitivity is caused by myrcene present in the leaf.

Geraniol and d-limonene from the essential oil induce activity of glu- tathione S-transferase, a detoxifying enzyme, which is believed to be a major factor for chemical carcinogen detoxification.... cymbopogon citratus

Myristica Fragrans

Family: Myristicaceae.

Habitat: Native to the Moluccas Islands; grown in the Nilgiris, Kerala, Karnataka and West Bengal.

English: Nutmeg, Mace.

Ayurvedic: Jaatiphala, Jaatishasya, Maalatiphala (seed kernel).

Jaatipatri, Jaatipatra, Jaatipatraka, Jaatikosha (mace).

Unani: Jauzbuwaa (seed), Bisbaasaa (mace).

Siddha/Tamil: Jaathikkai, Saadikai (nutmeg); Saadippatthiri, Jaadip- patiri (mace).

Action: Nutmeg—carminative, spasmolytic, antiemetic, orexi- genic; topically anti-inflammatory. Mace—stimulant carminative. Narcotic in high doses.

Nutmeg is used in flatulency, diarrhoea, nausea and vomiting. Mace is used in rheumatism, chronic bowel complaints and asthma. When roasted, both nutmeg and mace are used for diarrhoea, colic, flatulence and dyspepsia.

Key application: Dried seed and aril—included among unapproved herbs by German Commission E. Following actions have been considered: antispasmodic, MAO inhibition, inhibition of prostaglandin synthesis.

The Ayurvedic Pharmacopoeia of India recommends the kernel of the fruit in spermatorrhoea.

An aqueous extract of nutmeg is reported to show anti-secretory activity against E. coli heat-labile enterotoxin; the hexane soluble fraction of the alcoholic extract inhibited the heat-labile and heat-stable-enterotoxin-induced secretory response in animal studies.

The hexane extract contains myris- ticin, an anti-inflammatory principle, and licarin-B and dehydro di- isoeugenol which exhibited CNS depressant properties. The extracts of nutmeg decreased kidney prostaglan- din levels in rats. They also inhibited platelet aggregation (due to eugenol and isoeugenol). The anti-inflammatory activity observed in carrage- enan-induced oedema in rats and enhanced vascular permeability in mice, are attributed to myristicin present in mace.

Mace also activates hepatic detoxification process. Monomeric and dimer- ic phenyl propanoids (myristicin, de- hydro diisoeugenol) from mace, on p.o. administration in mice, produced suppression of lipid peroxidation in liver.

Seeds contain about 0.24% myris- ticin, whereas volatile oil about 3.12%.

The resorcinols, malabaricones B and C, isolated from the seed coat (mace) exhibited strong antibacterial and antifungal activities. Neoplasm inhibitors, phenylpropyl derivatives, have been isolated from pulverized mace.

Importance: Gymnema, Australian Cowplant, Small Indian Ipecacuanha or Periploca of the woods is a woody climber. It is reported to cure cough, dyspnoea, ulcers, pitta, kapha and pain in the eyes. The plant is useful in inflammations, hepatosplenomegaly, dyspepsia, constipation, jaundice, haemorrhoids, strangury, renal and vesical calculi, helminthiasis, cardiopathy, cough, asthma, bronchitis, intermittent fever, amenorrhoea, conjuctivitis and leucoderma. The fresh leaves when chewed have the remarkable property of paralysing the sense of taste for sweet and bitter substance for some time (Warrier et al, 1995). The drug is described as a destroyer of madhumeha (glycosuria) and other urinary disorders. Root has long been reputed as a remedy for snakebite. Leaves triturated and mixed with castor oil are applied to swollen glands and enlargement of internal viscera as the liver and spleen (Nadkarni, 1954). The drug is used to strengthen the function of heart, cure jaundice, piles, urinary calculi, difficult micturition and intermittent fevers (Sharma,1983). The drug enters into the composition of preparations like Ayaskrti, Varunadi kasaya, Varunadighrtam, Mahakalyanakaghrtam, etc. They suppress the activity of taste of tongue for sweet taste and for this reason it was considered that it destroys sugar, hence the name Madhunashini or Gurmar and has been prescribed as an anti-diabetic. The crude drug as well as its dried aqueous extract is mainly used in bronchial troubles.

Distribution: It is a tropical climber. It mainly grows in Western Ghats, Konkan, Tamil Nadu and some parts of Bihar. The plant is cultivated in plains of India but the drug is mainly important from Afghanistan and Iran.

Botany: Gymnema sylvestre (Retz.)R. Br. syn. Asclepias germinata Roxb. belonging to the family Asclepiadaceae is a large, woody much branched climber with pubescent young parts. Leaves are simple, opposite, elliptic or ovate, more or less pubescent on both sides, base rounded or cordate. Flowers are small, yellow and arranged in umbellate cymes. Fruits are slender and follicles are upto 7.5cm long (Warrier et al, 1995).

Two allied species, G. hirsutum found in Bundelkh and Bihar and Western Ghats and G. montanum growing wild in Eastern Ghats and Konkan are also used for the same purpose and are also called “Gurmar” (Thakur et al, 1989).

Agrotechnology: The plant can be propagated both by seeds and stem cuttings. Seedlings are to be raised in polybags. Pits of size 50cm cube are to be taken, filled with 10kg dried cowdung or FYM and covered with topsoil. On these pits about 3-4 months old seedlings are to be transplanted from polybags. Trailing can be facilitated by erecting poles and tying the plants to the poles. The plant will attain good spread within one year. Regular weeding, irrigation and organic manure application are beneficial. The plant is not attacked by any serious pests or diseases. Leaves can be collected from the first year onwards at an internal of one week. This can be continued for 10-12 years. Fresh or dried leaves can be marketed (Prasad et al, 1997).

Properties and activity: Nonacosane and hentriacontane were isolated from the hexane extract of leaves. An attempt to isolate nitrogenous compounds led to the isolation of amino acids such as leucine, iso-leucine, valine, allanine and - amynobutyric acid. Isolation of trimethyl amine oxide was of particular interest. An alkaloid gynamine which is a trace constituent was isolated and identified (Sinsheimer et al, 1967). Antisweet constituent of the leaves has been found to be a mixture of triterpene saponins. These have been designated as gymnemic acids A,B,C and D which have the gymnemagenin and gymnestrogenins as the aglycones of gymnemic acid A and B and gymnemic acid C and D respectively. These are hexahydroxy triterpenes the latter being partially acylated. The sugar residues are glucuronic acid and galacturonic acid while ferulic and angelic acids have been attached as the carboxylic acid.

Chewing of leaves reduces sensitivity to sweet substances. Effects of gymnema extracts had been variable. While verifying the effect of G. sylvestre leaves on detoxification of snake venom, it has been reported that a toxic component of venom ATP and gymnemate bind at the same site inhibiting venom ATP-ase. The active principles which have been identified as glycosides (7 gymnemic acids) suggest that the topical and selective anaesthetic effect of the plant might result from the competition of the receptor sites between glycosides and the sweet substances (Warren et al, 1969). The leaves are antidiabetic and insulinotropic. Gymnemic acid is antiviral. The plant is bitter, astringent, acrid, thermogenic, antiinflammatory, anodyne, digestive, liver tonic, emetic, diuretic, stomachic, stimulant, anthelmintic, alexipharmic, laxative, cardiotonic, expectorant, antipyretic and uterine tonic.... gymnema

Syzygium Aromaticum

Siddha/Tamil: Kiraambu, Lavangam.

Action: Carminative, antiinflammatory, antibacterial. Flower buds—antiemetic, stimulant, carminative. Used in dyspepsia, gastric irritation. Oil—employed as a local analgesic for hypersensitive dent- lines and carious cavaties; internally as a carminative and antispasmodic.

Key application: In inflammatory changes of oral and pharyngeal mucosa; in dentistry; for topical anesthesia. (German Commission E.)

Eugenin, triterpene acids, crategolic acid and steroid glucosides afford anti- inflammatory and antiseptic proper ties to the buds. Eugenol, a major component of the oil, is antibacterial. Acetone extract of clove, eugenol and acetyleugenol possess cholagogue activity. The eugenol and acetyleugenol components of the clove oil inhibit arachidonate-, adrenalin- and collagen- induced platelet aggregation.

Clove terpenes show significant activity as inducers of detoxifying enzyme, glutathione-S-transferase (GST) in mouse liver and intestine and bring about carcinogen detoxification.

Alcohol Dependence

An illness characterized by habitual, compulsive, longterm, heavy consumption of alcohol and the development of withdrawal symptoms when drinking is suddenly stopped.

Three causative factors interact in the development of the illness: personality, environment, and the addictive nature of alcohol. Inadequate, insecure, or immature personalities are more at risk. Environmental factors are important, especially the ready availability, affordability, and social acceptance of alcohol. Genetic factors may play a part in causing dependence in some cases, but it is now widely believed that anyone, irrespective of personality, environment, or genetic background, can become an alcoholic. Stress is often a major factor in precipitating heavy drinking.

Alcohol dependence usually develops in 4 main stages that occur over a number of years. In the 1st phase, tolerance to alcohol develops in the heavy social drinker. In the 2nd phase, the drinker experiences memory lapses relating to events during the drinking episodes. In the 3rd phase, there is loss of control over alcohol consumption. The final phase is characterized by prolonged binges of intoxication and mental or physical complications.

Behavioural symptoms are varied and can include furtive, aggressive, or grandiose behaviour; personality changes (such as irritability, jealousy, or uncontrolled anger); neglect of food intake and personal appearance; and lengthy periods of intoxication.

Physical symptoms may include nausea, vomiting, or shaking in the morning; abdominal pain; cramps; numbness or tingling; weakness in the legs and hands; irregular pulse; enlarged blood vessels in the face; unsteadiness; confusion; memory lapses; and incontinence. After sudden withdrawal from alcohol, delirium tremens may occur.

Alcohol-dependent persons are more susceptible than others to a variety of physical and mental disorders (see alcohol-related disorders).

Many alcoholics require detoxification followed by long-term treatment. Different methods of treatment may be combined. Psychological treatments involve psychotherapy and are commonly carried out as group therapy. Social treatments may offer practical help and tend to include family members in the process. Physical treatment generally includes the use of disulfiram, a drug that sensitizes the drinker to alcohol so that he or she experiences unpleasant side effects when drinking. Hepatitis, Acute

Short-term inflammation of the liver, which usually recovers in 1–2 months. In some cases, acute hepatitis may progress to chronic hepatitis (see hepatitis, chronic), but it rarely leads to acute liver failure.

Acute hepatitis is fairly common.

The most frequent cause is infection with one of the hepatitis viruses (see hepatitis, viral), but it can arise as a result of other infections such as cytomegalovirus infection or Legionnaires’ disease.

It may also occur as a result of overdose of halothane or paracetamol or exposure to toxic chemicals including alcohol (see liver disease, alcoholic).

Symptoms range from few and mild to severe with pain, fever, and jaundice.

Blood tests, including liver function tests, may be used for diagnosis.

In most cases of acute viral hepatitis, natural recovery occurs within a few weeks.

If the disorder is caused by exposure to a chemical or drug, detoxification using an antidote may be possible.

Intensive care may be required if the liver is badly damaged.

Rarely, a liver transplant is the only way of saving life.

Alcoholism

n. the syndrome due to physical *dependence on alcohol, such that sudden deprivation may cause withdrawal symptoms – tremor, anxiety, hallucinations, and delusions (see delirium tremens). The risk of alcoholism for an individual depends on genetic and environmental factors. Several years’ heavy drinking is often needed for addiction to develop, with a wide range from 1 to 40 years. Alcoholism impairs intellectual function, physical skills, memory, and judgment. Social skills can also be affected. Heavy consumption of alcohol causes *cardiomyopathy, *peripheral neuropathy, *cirrhosis of the liver, and enteritis. Treatment is usually provided on an out-patient basis, in specialist units for detoxification or medical wards. Unsupervised sudden withdrawal carries a mortality of about 10%, mostly due to seizures. If there are complicating psychiatric problems detoxification may be part of psychiatric treatment. Psychological aspects of treatment include helping the patient to understand the psychological pressures that led to his or her heavy drinking, treatment of underlying anxiety, and *motivational interviewing. Drugs such as *disulfiram (Antabuse), which cause vomiting if alcohol is taken, can help in treatment. Liver

n. the largest gland of the body, weighing 1200–1600 g. Situated in the top right portion of the abdominal cavity, the liver is divided by fissures (fossae) into four lobes: the right (the largest lobe), left, quadrate, and caudate lobes. It is connected to the diaphragm and abdominal walls by five ligaments: the membranous falciform (which separates the right and left lobes), coronary, and right and left triangular ligaments and the fibrous round ligament, which is derived from the embryonic umbilical vein. Venous blood containing digested food is brought to the liver in the hepatic portal vein (see portal system). Branches of this vein pass in between the lobules and terminate in the sinusoids (see illustration).

Oxygenated blood is supplied in the hepatic artery. The blood leaves the liver via a central vein in each lobule, which drains into the *hepatic vein. The liver is supplied by parasympathetic nerve fibres from the vagus nerve, and by sympathetic fibres from the solar plexus. The liver has a number of important functions. It synthesizes *bile, which drains into the *gall bladder before being released into the duodenum. The liver is an important site of metabolism of carbohydrates, proteins, and fats. It regulates the amount of blood sugar, converting excess glucose to *glycogen; it removes excess amino acids by breaking them down into ammonia and finally *urea; and it stores and metabolizes fats. The liver also synthesizes *fibrinogen and *prothrombin (essential blood-clotting substances) and *heparin, an anticoagulant. It forms red blood cells in the fetus and is the site of production of plasma proteins. It has an important role in the detoxification of poisonous substances and it breaks down worn out red cells and other unwanted substances, such as excess oestrogen in the male (see also Kupffer cells). The liver is also the site of *vitamin A synthesis; this vitamin is stored in the liver, together with vitamins B12, D, and K.
