Extension: From 2 Different Sources


Extension is the process of straightening or stretching a limb. When used in the natural sense, it involves the contraction of the muscles opposing those used in FLEXION. In cases of fractured limbs (see BONE, DISORDERS OF – Bone fractures), extension is employed during the application of splints, in order to reduce the displacement caused by the fracture, and prevent movement of the broken ends of bone. It is e?ected by gently and steadily pulling upon the part of the limb beyond the fracture. Extension of a more permanent type is used in the after-treatment of some fractures, as well as in diseases of the spine, by placing the patient upon an inclined bed and a?xing weights to his or her lower limbs, or to his or her head by means of adhesive plaster or of straps.

Cruciate Ligaments

Two strong ligaments in the interior of the knee-joint, which cross one another like the limbs of the letter X. They are so attached as to become taut when the lower limb is straightened, and they prevent over-extension or bending forwards at the knee. The cruciate ligaments are sometimes strained or torn as a result of sporting injuries or vehicular accidents; surgery may be needed to repair the damage, but the knee will be permanently weakened.... cruciate ligaments

Freudian Theory

A theory that emotional and allied diseases are due to a psychic injury or trauma, generally of a sexual nature, which did not produce an adequate reaction when it was received and therefore remains as a subconscious or ‘affect’ memory to trouble the patient’s mind. As an extension of this theory, Freudian treatment consists of encouraging the patient to tell everything that happens to be associated with trains of thought which lead up to this memory, thus securing a ‘purging’ of the mind from the original ‘affect memory’ which is the cause of the symptoms. This form of treatment is also called psychocatharsis or abreaction.

The general term, psychoanalysis, is applied, in the ?rst place, to the method of helping the patient to recover buried memories by free association of thoughts. In the second place, the term is applied to the body of psychological knowledge and theory accumulated and devised by Sigmund Freud (1856–1939) and his followers. The term ‘psychoanalyst’ has traditionally been applied to those who have undergone Freudian training, but Freud’s ideas are being increasingly questioned by some modern psychiatrists.... freudian theory

Hirsutism

The growth of hair of the male type and distribution in women. It is due either to the excess production of androgens (see ANDROGEN), or to undue sensitivity of the hair follicle to normal female levels of circulating androgens. The latter is called idiopathic hirsutism, because the cause is unknown. The increased production of androgens in the female may come from the ovary (see OVARIES) and be due to POLYCYSTIC OVARY SYNDROME or an ovarian tumour, or the excess androgen may come from the adrenal cortex (see ADRENAL GLANDS) and be the result of congenital adrenal HYPERPLASIA, an adrenal tumour or CUSHING’S SYNDROME. However, there is a wide range of normality in the distribution of female body hair. It varies with di?erent racial groups: the Mediterranean races have more body hair than Nordic women, and the Chinese and Japanese have little body hair. It is not abnormal for many women, especially those with dark hair, to have hair apparent on the upper lip, and a few coarse hairs on the chin and around the nipples are not uncommon. Extension of the pubic hair towards the umbilicus is also frequently found. Dark hair is much more apparent than fair hair, and this is why bleaching is of considerable bene?t in the management of hirsutism.

The treatment of hirsutism is that of the primary cause. Idiopathic hirsutism must be managed by simple measures such as bleaching the hair and the use of depilatory waxes and creams. Coarse facial hairs can be removed by electrolysis, although this is time-consuming. Shaving is often the most e?ective remedy and neither increases the rate of hair growth nor causes the hairs to become coarser.... hirsutism

Importance: Holostemma is a twining shrub with large flowers. The roots of Holostemma are useful in ophthalmopathy, orchitis, cough, burning sensation, stomachalgia, constipation, fever and tridoshas. The leaves, flowers and fruits are eaten as vegetable. The root is also used in spermatorrhoea. It is used in preparations of Vidaryadiganam, Dhanwandharam thaila, Manasamithravatakam, Balarishta and Anuthaila. It is also useful in eye diseases and it imparts resistance to diseases.

Distribution: The plant occurs in tropical countries. In India, it is found in Himalayas, Dehradun, Konkan, Bombay, Deccan, Karnataka, Kerala and Tamilnadu. It grows over hedges and in open forests especially on the lower slopes of the hills. It is also distributed in Sri Lanka, Burma and W. China.

Botany: Holostemma ada-kodien Schult. syn. Holostemma annulare (Roxb.) K. Schum.

Holostemma rheedii Wall. belongs to the family Asclepiadaceae. It is a laticiferous twining shrub with large conspicuous flowers. Leaves are simple, opposite and cordate. Flowers are purple, arranged in axillary umbellate cymes. Fruits are thick follicles, 9 cm long, cylindrical and bluntly pointed. The roots are long upto 1 m or more, irregularly twisted, thick and cylindrical. When dry it is yellowish brown to brown black in colour with nearly smooth surface bearing white scars and small depressions. A mature root is about 1-2 cm thick when extracted for use (Warrier et al, 1995).

Agrotechnology: Holostemma prefers a tropical climate. The plant is propagated vegetatively by stem cuttings, but mainly by seeds. The seeds are collected from the plant in November-December before being dispersed. Seeds are cleaned, dried and stored for sowing. The stored seeds after soaking in water for 4-5 hours are sown in the seedbeds. About one month old seedlings are then planted in polybags of size 14x10cm which are filled with soil, sand and dried cowdung in 1:1:1 ratio, respectively. Polybags should be kept in shade and irrigated. About 1-1.5 month old seedlings are ready for transplanting. Pits of 30cm cube size are taken at 1-1.2m distance and filled with 10kg dried cowdung and sand. This is covered with surface soil and formed into a mound. Seedlings are transplanted on to the mounds from the polybags carefully. Regular irrigation is to be given till flowering. To aid in trailing, staking is given one month after planting. Flowering and fruiting occurs in November-December. Harvesting can be done at the end of second year when the vines start drying up. Harvesting is done by digging up the tubers. The tubers are cut into pieces of 10cm length and dried in sun before sale (Prasad et al, 1997).

Properties and activity: Holostemma tubers give -amyrin, lupeol and -sitosterol. Alanine, aspartic acid, glycine, serine, threonine and valine were detected chromatographically (Hussain et al, 1992). The root is antidiabetic, antigonorrhoeic, bechic, alterative, tonic, lactative, ophthalmic, emollient, stimulant, aphrodisiac, expectorant and galactagogue.

Importance: Musali is a small, geophilous herb, the tuberous rootstock of which is used as a rejuvenating and aphrodisiac drug. It cures morbid vata and pitta, improves complexion and is useful in general debility, deafness, cough, asthma, piles, skin diseases, impotence, jaundice, urinary disorders, leucorrhoea and menorrhagia (Nadkarni, 1954; Aiyer and Kolammal, 1963; Mooss, 1978). Rootstock is the officinal part and it enters into the Ayurvedic formulations like Vidaryadighrta, Vidaryadi lehya, Marmagulika, Musalyadi churna etc. The Pharmacognosy of C. orchioides has been discussed by Aiyer, Kolamma l (1963), Raghunathan, and Mitra (1982). A bibliographical study on C. orchioides has been done by Pandey et al (1983).

Distribution: The plant is found in all districts of India from near sea level to 2300m altitude, especially in rock crevices and laterite soil. It has been recorded to occur in the sub tropical Himalayas from Kumaon eastwards ascending to 1800m, the Khasia hills, Bengal, Asssam, Konkan, Kanara, the western peninsula and Madras extending south as far as a Cape Comerin. It is also distributed in Sri Lanka, Japan, Malaysia and Australia.

Botany: Curculigo orchioides Gaertn. syn. C. malabarica Wight, C. brevifolia Dryand, Hypoxis dulcis Stand belongs to the family Amaryllidaceae. Musali is a small herbaceous plant with cylindrical rootstock. Leaves are simple, sessile, crowded on the short stem with sheathing leaf bases. Flowers are bright yellow. Seeds are black, deeply grooved in wavy lines.

A detailed description of the plant is as follows (Victoria, 1998). Rootstock is straight, cylindrical, tuberous, 5-22cm long, 0.5-0.8 cm thick, brownish surfaces marked with closely spaced prominent transverse wrinkles in the upper or basal half. It bears a few stout lateral roots of 5 or more cm long. Lateral roots are dull white in colour and spongy externally. The fresh cut surfaces of the rootstock has a starch white colour and mucilaginous. A few fibrous roots also occur. Leaves are sessile or short petiolate with sheathing bases, 15-45x1.2-2.5 cm size, linear or linear lanceolate, membranous, glabrouus or sparsely sofly hairy and plicate in bud. The leaf tips when contacts the soil, develops roots and produce adventitious buds. Inflorescence is axillary, scapose racemose, the scape very short and hidden among the bases of leaves underground, clavte, flattened with the pedicels, bracts and the ovary concealed in the leaf sheaths. The lower big flowers on the scape are mostly bisexual and the upper small ones staminate. Flowers are epigynous bright yellow, bisexual or unisxual with lanceolate, memb ranous bract.. Perianth gamophyllous, rotate & six lobed, locate at the top of a slender sterile long extension of the ovary by means of which the perianth is exposed above the ground. Perianth lobes similar, elliptic oblong 1.2-1.6 cm long, 0.2-0.3 cm broad, outer lobes hairy on the back, inner ones sparsely hairy along nerves. Stamens 6 in number, filamentous filiform, short 2mm long, adnate to the base of the perianth lobes, Anthers linear or linear lanceolate, basifixed and sagittate,.Ovary inferior, hidden among the leaves usually below the ground, tricarpellary syncarpous, lanceolate and trilocular with a fairly long slender beak or extension -the stipe. Ovules many in each cell attached by a distinct long funicle. Style short columnar, 2mm with a 3 lobed stigma. Lobes elongate, erect and appressed. Fruit is a capsule about 1.5-2cm long, 8mm broad, oblong, glabrescent with a slender beak and spongy septa. Seeds 1-many, oblong, black, shiny with crustaceous testa grooved deeply in wavy lines.

Properties and activity: Rao and Beri (1951) have identified glucose, mannose, xylose and glucuronic acid from the rootstock of C. orchioides. The rootstock is also reported to contain glycoside, polysaccharides (hemicellulose and other polysaccharides), starch, resin, tannin, mucilage, fat and calcium oxalate. The hexane extract contains an alkaloid-lycorine, sterols including -sitosterols and sapogenin identified as yuccagenin (Rao et al, 1978). The flavone glycoside from the rootstock has been identified as 5,7- dimethoxy glucopyranoside (Yadav et al, 1974; Sharma et al 1975). Mehta et al (1980) have isolated a number of fatty acids from C. orchioides root oil by GLC techniques. They are palmitic, oleic, linolenic linoleic, arachidic and behenic acid. Kubo et al (1983) isolated a new phenolic glycoside namely, curculigoside from the rhizomes and its structure has been elucidated as 5- hydroxy-2-0- -d-glucopyranosyl benzl 1,2,6-dimethoxy benzoate. Yamasaki et al (1994) developed HPLC method for estimating the curculigoside content in curculigo rhizome.

Two new aliphatic hydroxy ketone 27-hydroxy tricontan-6-one (M. P. 84-85o C) and 23- hydroxy tricontan-2-one (M. P. 109-110 o C) were isolated from the rhizome by Misra et al (1984). They also isolated 21-hydroxy tetracontan-20-one and 4-methyl heptade canoic acid from the root stock. Porwal et al (1988) have isolated and identified three new compounds from the rhizome as N- acetyl-N-hydroxy-2-carbamic acid methyl ester, 3-acetyl-5-carbomethoxy-2H-3,4,5,6-tetrahydro-1,2,3,5,6-oxatetrazine and N, N, N’, N’-tetra methyl succinamide. The rhizomes of C. orchioides yielded a new phenolic glycoside corchioside a, characterised as orcinol-3- -D-xylopyranosyl- (1 6)- -D-glucopyranoside and hentriacontanol (Garg et al, 1989).

A new aliphatic compound has been isolated from the rhizomes and characterised as 25- dihydroxy-33-methyl pentatricontan-one (Mehta et al, 1990). Misra et al (1990) isolated a new natural triterpene alcohol-Curculigol charactrised as 24-methy cycloart-7-en-3-beta-20-diol. A novel pentacyclic triterpene has been isolated from the rhizomes of C.orchioides and characterised as 31- methyl-3-oxo-20-ursen-28-oic acid (Metha and Gawarikar,1991). Xu and Xu (1992) and Xu et al (1992 a, b) have isolated 13 cycloartane type. Triterpene glycosides from C. orchioides rhizome and characterised them as curculigo saponin A-M.

The root stock are mucilaginous, sweet, cooling, bitter, emollient, diuretic, aphrodisiac, depurative, alternative, appetiser, carminative, viriligenic, antipyretic and tonic (Sivarajan and Indira, 1994; Warrier et al, 1994).

The uterine stimulant activity of the flavone glycoside extracted from C. orchioides has been studied by Dhawan and Saxena (1958), Sharma et al (1975) and Dhar et al (1979).

The plant extract of C. orchioides showed hypoglycaemic, spasmolytic and anticancer properties (Dhar et al,1968). Phagocytic activity (Kubo et al, 1983) and immunoadjuvant activity (Oru et al, 1982) of phenolic glycosides, curculigoside isolated from the rhizome of the plant have been reported. Porwal and Mehta (1985) discussed the medicinal importance of the plant and its use in indigenous system of medicine as a tonic. Sharma et al (1991) reported the influence of MAK an ayurvedic food supplement constituting C. orchioides against Dimethyl benz anthracene induced mammary tumours in rats. Samanta (1992) reported the modulation of male infertility by Ayurvedic drug, which constitutes C. orchioides. Immunostimulant activity of C. orchioides has been demonstrated by Saxena (1992). Immunological activites of curculigo saponin G were assayed in mice and the results showed that it promoted proliferation of spleen lymphocyctes very significantly and increased the weight of the thymus in vivo in mice (Xu et al,1992).

Pharmacological studies in China, on the alcoholic extract obtained from the plant showed several active effects such as adaptogenic, anti-inflammatory, anticonvulsant, sedative, androgenic and immunopromoting activities (Xu et al, 1992).

Curculigo orchioides is distributed widely throughout the country. The demand of the raw materials and derivatives of the plant for the indigenous drug industries are satisfied mainly from the wild source, depleting the natural population and thus the species have become extinct or endangered. Ansari (1993) have reported C. orchioides as a threatened plant from Madhulia forest of Garakhpur. Augustin and Souza (1995) also considered the plant as an endangered species. As the information on the cultivation of C.orchioides is scanty, it is very necessary to develop suitable agrotechniques for the domestication and large-scale cultivation of the plant.... holostemma

Leaf-crown

The cuticular extension to fine pointed processes in the nematodes which have no lip; extension from the rim of the mouth called “external leaf-crown”, from the rim of the buccal capsule called “internal leaf-crown” as in Strongyloidea.... leaf-crown

Lip

An extension of cuticle around the mouth of nematodes; there may be three, one dorsal and two subventral as in Ascaroidea or two as in Spiruroidea or absent as in Strongyloides and Filarioidea.... lip

Pulmonary Embolism

The condition in which an embolus (see EMBOLISM), or clot, is lodged in the LUNGS. The source of the clot is usually the veins of the lower abdomen or legs, in which clot formation has occurred as a result of the occurrence of DEEP VEIN THROMBOSIS (DVT) – THROMBOPHLEBITIS (see VEINS, DISEASES OF). Thrombophlebitis, with or without pulmonary embolism, is a not uncommon complication of surgical operations, especially in older patients. This is one reason why nowadays such patients are got up out of bed as quickly as possible, or, alternatively, are encouraged to move and exercise their legs regularly in bed. Long periods of sitting, particularly when travelling, can cause DVT with the risk of pulmonary embolism. The severity of a pulmonary embolism, which is characterised by the sudden onset of pain in the chest, with or without the coughing up of blood, and a varying degree of SHOCK, depends upon the size of the clot. If large enough, it may prove immediately fatal; in other cases, immediate operation may be needed to remove the clot; whilst in less severe cases anticoagulant treatment, in the form of HEPARIN, is given to prevent extension of the clot. For some operations, such as hip-joint replacements, with a high risk of deep-vein thrombosis in the leg, heparin is given for several days postoperatively.... pulmonary embolism

Purkinje Cells

Large specialised nerve cells occurring in great numbers in the cortex (super?cial layer of grey matter) of the cerebellum of the BRAIN. They have a ?ask-shaped body, an AXON and branching tree-like extensions called dendrites, which extend towards the surface of the brain (see NEURON(E)).... purkinje cells

Velarium

A folded-in extension of the edge of the bell in the cubozoa which helps create a jet of water to propel the jellyfish forwards It may contract differentially to enable a change of direction.... velarium

Eyelid

A fold of tissue at the upper or lower edge of an eye socket.

The eyelids are held in place by ligaments attached to the socket’s bony edges.

They consist of thin plates of fibrous tissue (called tarsal plates) covered by muscle and a thin layer of skin.

The inner layer is covered by an extension of the conjunctiva.

Along the edge of each lid are two rows of eyelashes.

Immediately behind the eyelashes are the openings of the ducts leading from the meibomian glands, which secrete the oily part of the tear film.

The lids act as protective shutters, closing as a reflex action if anything approaches the eye.

Screening Test

The screening of apparently healthy people to identify those who may have treatable diseases. Cervical smears are done when screening women to detect if they have cancer or precancer of the neck of the womb (cervix). Newborn babies are screened for hip dislocation. Screening tests are not designed to diagnose individual persons, but rather to divide a population into a large number at low risk and a small number at high risk of a condition. This allows clinicians to concentrate on a sub-section of the population. All screening tests produce false negative and false positive results, a problem often misunderstood by those at the receiving end. Factors to be assessed when planning screening procedures include the severity, frequency and distribution of the disease, and the availability and e?ectiveness of treatment. Convenience, safety, sensitivity and cost should also be assessed. Cancer

An invasive growth which gradually emerges into life and, undisciplined, eats its way into neighbouring tissues. Malignancy is the growth of abnormal cells with the ability to form a primary lesion from which cells may be bloodborne to other parts of the body (metastasis). Growth usually follows the line of the lymph vessels (Violet leaves have an affinity for lymph vessels).

Course of the disease is unpredictable, cases surviving for many years on primary or supportive herbal treatment. Suspected malignancy should be referred to modern hospital treatment immediately. Early detection is vital.

Common signs calling attention are: (1) Unusual bleeding or discharge. (2) Tired feeling all the time. (3) Thickening or lump in breast or elsewhere. (4) Sudden change in hair texture and colour. (5) Irritable cough or hoarseness. (6) Extreme mental depression. (7) Obvious change in a mole or wart. (8) Muscle weakness and cramps. (9) A sore that does not heal. (10) Change in bowel or bladder habit. (11) Sudden weakness of the eyes. (12) Difficulties in swallowing; indigestion. (13) Excess wind in stomach or bowel.

Tumour-killing effect of chemotherapy may be intensified and side-effects minimised (loss of weight, and of white blood cells) when certain neoplastic herbs are prescribed. Cytotoxic drugs inhibit the ability of Vitamin C to stimulate the body’s defences. Herbs enhance the body’s self-healing ability to eliminate. An inoperable cancer would appear to be good grounds for herbal medication which often relieves pain and preserves a man’s dignity in his hour of extremity.

A series of medical trials in Finland revealed that terminal cases had 12 per cent lower mean serum selenium concentration than controls. Other similar trials point to the need for selenium supplements. Those with both low selenium and low Vitamin E levels are especially at risk.

Plants with a special reference to cancer include: Blue Flag, Burdock, Clivers, Condurango, Echinacea, Guaiacum, Houseleek, Poke root. There are many more referred to in medical literature.

Poke Root. John Bartram reported in the late 18th century that from his experience among the Mohawk Indians, Poke root (Phytolacca decandra) was a “cure” for cancer. (American Indian Medicine, Virgil J. Vogel)

Blood Root. For internal or external bleeding of cancer.

Calendula (Marigold). For the same purpose.

Mistletoe. Dr Alfred Vogel advises an extract of the plant (Loranthus europaeus) as grown on the Oak tree: dose: 10-15 drops.

Almonds. Edgar Cayce, Virginia Beach, USA, with some successes to his credit, advised eating three almonds a day to counter any tendency towards the disease.

Laetrile. From Apricot kernels that contain cyanogenic glucosides. Though competent physicians have reported positive results in some terminal cases without prior surgery or radiation, the remedy has been withdrawn from general practice because of possible toxicity.

Much needless suffering may be incurred because of out-moded resistance of doctors and governments against prescribing morphine early in cancer patients. It is estimated that 50-80 per cent of patients do not receive satisfactory pain-relief because doctors fear tolerance of the drug would increase, necessitating a higher dosage. From the beginning of time the Opium Poppy has been the most effective analgesic for the terminal condition. Morphine is a respiratory depressant and some authorities believe it should be given before the final stages in continuous doses for adequate pain control. Risks must be balanced with benefits. Dangerous in asthmatics.

Way of Life. Herbal medication of malignant disease involves the patient with his treatment. Here is something he or she can do to regain some control over their life. It can give them the satisfaction of knowing that in some way they are ‘fighting back’ thus influencing the quality of life and a sense of well- being.

If improvement in cancer is not possible maybe the condition can be stabilised and the patient helped to cope with the very unpleasant side-effects of chemotherapy and radiation. Thus, may be restored the body’s natural balance and a possible extension of lifespan.

For this, patients and practitioners may need information and support. That is why suggestions for malignant disease are included in this book. Moreover, well-meaning friends and relatives may exert pressure on the patient ‘to leave no stone unturned’ in search of a cure. Thus every possible secondary treatment should be considered since any one may prove to contribute towards recovery. It is hoped that this book will invite a therapeutic alliance with members of the medical profession as well as with other practitioners.

Amoeba

Life Force, The.

The spirit of the body concerned with life and survival. An extension of the spirit of the Creator in the human body. The intelligence and power behind the immune system.

A weak immune system, with little ability to withstand infection or injury, may be genetic in origin. May be acquired by faulty diet, chemicalised food and medicine, antibiotics, vaccines, and steroid drugs that exhaust the glandular system.

The Life Force can be sustained by a diet of wholefoods, organically grown vegetables and an absence of chemicals in medicine, food and environment. It is safe-guarded by a relaxed life-style which predisposes to a balanced personality capable of meeting the stresses of modern living with equanimity and self-possession. Mastoiditis

An infection of the mastoid bone behind the ear, with possible destruction of bone. Usually due to extension of infection (streptococcal, etc) from the middle ear (otitis media) when that condition is wrongfully or neglectfully treated.

Symptoms: Mastoid bone behind the ear is tender to touch. Feverishness, red flush over mastoid area, deafness with throbbing earache, malaise, heavy discharge from the ear through perforated eardrum. Diagnostic sign: pinna (external ear) is displaced.

Treatment. Indicated: anti-microbials, anti-bacterials, alteratives with nervines as supportives. Yarrow tea.

Decoction. Combine: Echinacea 3; Wild Indigo 2; Poke root 1. 1 teaspoon to each cup water gently simmered 20 minutes. Half-1 cup every 2 hours with pinch of Cayenne.

Formula. Echinacea 2; Wild Indigo 1; Pulsatilla 1; few grains of Cayenne or Tincture Capsicum drops. Dose: Liquid Extracts: 30-60 drops (2-4ml). Tinctures: 4-8ml. Powders: 500mg (two 00 capsules or one- third teaspoon). Every 2 hours according to age. Children under 5 years – one-quarter dosage; under 12 years – half dosage.

Vitamin C. Copious fluids: fruit juices. Yarrow tea.

Topical. Goldenseal Ear Drops. Oil of Mullein, Sage or Lavender. Gentle massage with Tea Tree oil or Rosemary oil around the mastoid bone and in front of the ear 3/4 times daily.

Claw-hand

n. flexion and contraction of the fingers with extension at the joints between the fingers and the hand, giving a claw-like appearance. Diabetic Amyotrophy

an acute mononeuropathy of the femoral nerve, usually of microvascular origin, associated with chronic poor diabetic control. Symptoms are thigh pain and progressive weakness of knee extension. Examination reveals wasting of the quadriceps muscle group and loss of the knee jerk. It may affect both legs and recovery is usually slow. Treatment is with physiotherapy and improved control of the diabetes; the condition never seems to recur in the same leg. Hammer Toe

Syringomyelia

n. a disease of the spinal cord in which longitudinal cavities form within the cord, usually in the cervical (neck) region. The centrally situated cavity (syrinx) is especially likely to damage the motor nerve cells and the nerve fibres that transmit the sensations of pain and temperature. Characteristically there is weakness and wasting of the muscles in the hands with a loss of awareness of pain and temperature.
