Hoarseness: From 1 Different Sources


A rough, husky, or croaking voice. Short-lived hoarseness is often due to overuse of the voice, which strains the muscles in the larynx. It is also commonly caused by inflammation of the vocal cords in acute laryngitis. Persistent hoarseness may be due to chronic irritation of the larynx, which can be caused by smoking, excessive consumption of alcohol, chronic bronchitis, or constant dripping of mucus from the nasal passages. Polyps on the vocal cords may also cause hoarseness. In people with hypothyroidism, hoarseness can result from formation of tissue on the vocal cords. In young children, hoarseness may be a symptom of croup. Occasionally persistent hoarseness in adults has a more serious cause, such as cancer of the larynx (see larynx, cancer of), thyroid cancer, or lung cancer.

Resting the voice helps in strain- or laryngitis- related cases. If hoarseness persists for more than 2 weeks, a doctor should be consulted. A laryngoscopy may be performed to exclude a serious underlying cause.

Larynx, Disorders Of

Obstruction of the larynx is potentially dangerous in adults but can sometimes be life-threatening in infants and children. Stridor – noisy, di?cult breathing – is a symptom of obstruction. There are several causes, including congenital abnormalities of the larynx. Others are in?ammatory conditions such as acute laryngitis (see below), acute EPIGLOTTITIS and laryngo-tracheo-bronchitis (croup – see below); neurological abnormalities; trauma; and inhalation of foreign bodies.

Laryngitis In?ammation of the mucous membrane of the larynx and vocal chords may be acute or chronic. The cause is usually an infection, most commonly viral, although it may be the result of secondary bacterial infection, voice abuse or irritation by gases or chemicals. ACUTE LARYNGITIS may accompany any form of upper-respiratory-tract infection. The main symptom is hoarseness and often pain in the throat. The voice becomes husky or it may be lost. Cough, breathing diffculties and sometimes stridor may occur. Acute airway obstruction is unusual following laryngitis but may occasionally occur in infants (see laryngotracheo-bronchitis, below).

Treatment Vapour inhalations may be soothing and reduce swelling. Usually all that is needed is rest and analgesics such as paracetamol. Rarely, airway intervention – either ENDOTRACHEAL INTUBATION or TRACHEOSTOMY – may be necessary if severe airway obstruction develops (see APPENDIX 1: BASIC FIRST AID). A?ected patients should rest their voice and avoid smoking.

Chronic laryngitis can result from repeated attacks of acute laryngitis; excessive use of the voice – loud and prolonged, singing or shouting; tumours, which may be benign or malignant; or secondary to diseases such as TUBERCULOSIS and SYPHILIS.

Benign tumours or small nodules, such as singer’s nodules, may be surgically removed by direct laryngoscopy under general anaesthetic; while cancer of the larynx may be treated either by RADIOTHERAPY or by SURGERY, depending on the extent of the disease. Hoarseness may be the only symptom of vocal-chord disturbance or of laryngeal cancer: any case which has lasted for six weeks should be referred for a specialist opinion.

Thyroid Cancer

A rare disease that accounts for around 1 per cent of all cancers, cancer of the THYROID GLAND usually presents as an isolated hard nodule in the neck. The rate at which the nodule grows depends upon the patient’s age and type of cancer cell. Pain is not usually a feature, but the increasing size may result in the tumour pressing on vital structures in the neck – for example, the nerves controlling the LARYNX (resulting in hoarseness) and the PHARYNX (causing di?culty in swallowing). If more than one nodule is present, they are likely to be benign, not malignant. Treatment is by surgical removal after which the patient will need to take THYROXINE for the rest of his or her life. Radioactive iodine is usually given after surgery to destroy any residual cancerous cells. If treated early, the outlook is good.... thyroid cancer

Beclometasone

A corticosteroid drug that is used in the treatment of asthma and hay fever (see rhinitis, allergic). Beclometasone, which is prescribed as an inhaler or nasal spray, controls the symptoms by reducing inflammation and mucus production in the lining of the nose or, in asthma, inflammation of the airways. The drug is often given with bronchodilator drugs in the management of asthma. A severe asthma attack may require the dose to be increased. The action of beclometasone is slow, however, and its full effect takes several days to occur. Adverse effects of the drug may include hoarseness, throat irritation, and, on rare occasions, fungal infections in the mouth. Beclometasone is also prescribed in the form of a cream or ointment to treat inflammation of the skin caused by eczema.... beclometasone

Goitre

SIMPLE GOITRE A benign enlargement of the THYROID GLAND with normal production of hormone. It is ENDEMIC in certain geographical areas where there is IODINE de?ciency. Thus, if iodine intake is de?cient, the production of thyroid hormone is threatened and the anterior PITUITARY GLAND secretes increased amounts of thyrotrophic hormone with consequent overgrowth of the thyroid gland. Simple goitres in non-endemic areas may occur at puberty, during pregnancy and at the menopause, which are times of increased demand for thyroid hormone. The only e?ective treament is thyroid replacement therapy to suppress the enhanced production of thyrotrophic hormone. The prevalence of endemic goitre can be, and has been, reduced by the iodinisation of domestic salt in many countries. NODULAR GOITRES do not respond as well as the di?use goitres to THYROXINE treatment. They are usually the result of alternating episodes of hyperplasia and involution which lead to permanent thyroid enlargement. The only e?ective way of curing a nodular goitre is to excise it, and THYROIDECTOMY should be recommended if the goitre is causing pressure symptoms or if there is a suspicion of malignancy. LYMPHADENOID GOITRES are due to the production of ANTIBODIES against antigens (see ANTIGEN) in the thyroid gland. They are an example of an autoimmune disease. They tend to occur in the third and fourth decade and the gland is much ?rmer than the softer gland of a simple goitre. Lymphadenoid goitres respond to treatment with thyroxine. TOXIC GOITRES may occur in thyrotoxicosis (see below), although much less frequently autonomous nodules of a nodular goitre may be responsible for the increased production of thyroxine and thus cause thyrotoxicosis. Thyrotoxicosis is also an autoimmune disease in which an antibody is produced that stimulates the thyroid to produce excessive amounts of hormone, making the patient thyrotoxic.

Rarely, an enlarged gland may be the result of cancer in the thyroid.

Treatment A symptomless goitre may gradually disappear or be so small as not to merit treatment. If the goitre is large or is causing the patient di?culty in swallowing or breathing, it may need surgical removal by partial or total thyroidectomy. If the patient is de?cient in iodine, ?sh and iodised salt should be included in the diet.

Hyperthyroidism is a common disorder affecting 2–5 per cent of all females at some time in their lives. The most common cause – around 75 per cent of cases – is thyrotoxicosis (see below). An ADENOMA (or multiple adenomas) or nodules in the thyroid also cause hyperthyroidism. There are several other rare causes, including in?ammation caused by a virus, autoimune reactions and cancer. The symptoms of hyperthyroidism affect many of the body’s systems as a consequence of the much-increased metabolic rate.

Thyrotoxicosis is a syndrome consisting of di?use goitre (enlarged thyroid gland), over-activity of the gland and EXOPHTHALMOS (protruding eyes). Patients lose weight and develop an increased appetite, heat intolerance and sweating. They are anxious, irritable, hyperactive, suffer from TACHYCARDIA, breathlessness and muscle weakness and are sometimes depressed. The hyperthyroidism is due to the production of ANTIBODIES to the TSH receptor (see THYROTROPHIN-STIMULATING HORMONE (TSH)) which stimulate the receptor with resultant production of excess thyroid hormones. The goitre is due to antibodies that stimulate the growth of the thyroid gland. The exoph-

thalmos is due to another immunoglobulin called the ophthalmopathic immunoglobulin, which is an antibody to a retro-orbital antigen on the surface of the retro-orbital EYE muscles. This provokes in?ammation in the retro-orbital tissues which is associated with the accumulation of water and mucopolysaccharide which ?lls the orbit and causes the eye to protrude forwards.

Although thyrotoxicosis may affect any age-group, the peak incidence is in the third decade. Females are affected ten times as often as males; the prevalence in females is one in 500. As with many other autoimmune diseases, there is an increased prevalence of autoimmune thyroid disease in the relatives of patients with thyrotoxicosis. Some of these patients may have hypothyroidism (see below) and others, thyrotoxicosis. Patients with thyrotoxicosis may present with a goitre or with the eye signs or, most commonly, with the symptoms of excess thyroid hormone production. Thyroid hormone controls the metabolic rate of the body so that the symptoms of hyperthyroidism are those of excess metabolism.

The diagnosis of thyrotoxicosis is con?rmed by the measurement of the circulating levels of the two thyroid hormones, thyroxine and TRIIODOTHYRONINE.

Treatment There are several e?ective treatments for thyrotoxicosis. ANTITHYROID DRUGS These drugs inhibit the iodination of tyrosine and hence the formation of the thyroid hormones. The most commonly used drugs are carbimazole and propylthiouricil: these will control the excess production of thyroid hormones in virtually all cases. Once the patient’s thyroid is functioning normally, the dose can be reduced to a maintenance level and is usually continued for two years. The disadvantage of antithyroid drugs is that after two years’ treatment nearly half the patients will relapse and will then require more de?nitive therapy. PARTIAL THYROIDECTOMY Removal of three-quarters of the thyroid gland is e?ective treatment of thyrotoxicosis. It is the treatment of choice in those patients with large goitres. The patient must however be treated with medication so that they are euthyroid (have a normally functioning thyroid) before surgery is undertaken, or thyroid crisis and cardiac arrhythmias may complicate the operation. RADIOACTIVE IODINE THERAPY This has been in use for many years, and is an e?ective means of controlling hyperthyroidism. One of the disadvantages of radioactive iodine is that the incidence of hypothyroidism is much greater than with other forms of treatment. However, the management of hypothyroidism is simple and requires thyroxine tablets and regular monitoring for hypothyroidism. There is no evidence of any increased incidence of cancer of the thyroid or LEUKAEMIA following radio-iodine therapy. It has been the pattern in Britain to reserve radio-iodine treatment to those over the age of 35, or those whose prognosis is unlikely to be more than 30 years as a result of cardiac or respiratory disease. Radioactive iodine treatment should not be given to a seriously thyrotoxic patient. BETA-ADRENOCEPTOR-BLOCKING DRUGS Usually PROPRANOLOL HYDROCHLORIDE: useful for symptomatic treatment during the ?rst 4–8 weeks until the longer-term drugs have reduced thyroid activity.

Hypothyroidism A condition resulting from underactivity of the thyroid gland. One form, in which the skin and subcutaneous tissues thicken and result in a coarse appearance, is called myxoedema. The thyroid gland secretes two hormones – thyroxine and triiodothyronine – and these hormones are responsible for the metabolic activity of the body. Hypothyroidism may result from developmental abnormalities of the gland, or from a de?ciency of the enzymes necessary for the synthesis of the hormones. It may be a feature of endemic goitre and retarded development, but the most common cause of hypothyroidism is the autoimmune destruction of the thyroid known as chronic thyroiditis. It may also occur as a result of radio-iodine treatment of thyroid overactivity (see above) and is occasionally secondary to pituitary disease in which inadequate TSH production occurs. It is a common disorder, occurring in 14 per 1,000 females and one per 1,000 males. Most patients present between the age of 30 and 60 years.

Symptoms As thyroid hormones are responsible for the metabolic rate of the body, hypothyroidism usually presents with a general sluggishness: this affects both physical and mental activities. The intellectual functions become slow, the speech deliberate and the formation of ideas and the answers to questions take longer than in healthy people. Physical energy is reduced and patients frequently complain of lethargy and generalised muscle aches and pains. Patients become intolerant of the cold and the skin becomes dry and swollen. The LARYNX also becomes swollen and gives rise to a hoarseness of the voice. Most patients gain weight and develop constipation. The skin becomes dry and yellow due to the presence of increased carotene. Hair becomes thinned and brittle and even baldness may develop. Swelling of the soft tissues may give rise to a CARPAL TUNNEL SYNDROME and middle-ear deafness. The diagnosis is con?rmed by measuring the levels of thyroid hormones in the blood, which are low, and of the pituitary TSH which is raised in primary hypothyroidism.

Treatment consists of the administration of thyroxine. Although tri-iodothyronine is the metabolically active hormone, thyroxine is converted to tri-iodothyronine by the tissues of the body. Treatment should be started cautiously and slowly increased to 0·2 mg daily – the equivalent of the maximum output of the thyroid gland. If too large a dose is given initially, palpitations and tachycardia are likely to result; in the elderly, heart failure may be precipitated.

Congenital hypothyroidism Babies may be born hypothyroid as a result of having little or no functioning thyroid-gland tissue. In the developed world the condition is diagnosed by screening, all newborn babies having a blood test to analyse TSH levels. Those found positive have a repeat test and, if the diagnosis is con?rmed, start on thyroid replacement therapy within a few weeks of birth. As a result most of the ill-effects of cretinism can be avoided and the children lead normal lives.

Thyroiditis In?ammation of the thyroid gland. The acute form is usually caused by a bacterial infection elsewhere in the body: treatment with antibiotics is needed. Occasionally a virus may be the infectious agent. Hashimoto’s thyroiditis is an autoimmune disorder causing hypothyroidism (reduced activity of the gland). Subacute thyroiditis is in?ammation of unknown cause in which the gland becomes painful and the patient suffers fever, weight loss and malaise. It sometimes lasts for several months but is usually self-limiting.

Thyrotoxic adenoma A variety of thyrotoxicosis (see hyperthyroidism above) in which one of the nodules of a multinodular goitre becomes autonomous and secretes excess thyroid hormone. The symptoms that result are similar to those of thyrotoxicosis, but there are minor di?erences.

Treatment The ?rst line of treatment is to render the patient euthyroid by treatment with antithyroid drugs. Then the nodule should be removed surgically or destroyed using radioactive iodine.

Thyrotoxicosis A disorder of the thyroid gland in which excessive amounts of thyroid hormones are secreted into the bloodstream. Resultant symptoms are tachycardia, tremor, anxiety, sweating, increased appetite, weight loss and dislike of heat. (See hyperthyroidism above.)... goitre

Habitat: North-Western Himalayas and Persia; cultivated in Kumaon, Uttar Pradesh, Rajasthan, Madhya Pradesh; in gardens throughout India.

English: Spanish Jasmine.

Ayurvedic: Jaati, Jaatikaa, Jaatimalli, Pravaaljaati, Saumanasyaayani, Sumanaa, Chetikaa, Hriddgandhaa, Maalati, Chameli.

Unani: Yaasmin.

Siddha/Tamil: Manmadabanam, Mullai, Padar-malligai, Pichi, Malli

Folk: Chameli.

Action: Flowers—calming and sedative, CNS depressant, astringent and mild anaesthetic. A syrup prepared from the flowers is used for coughs, hoarsenesses and other disorders of the chest. Plant—diuretic, anthelmintic, emmenagogue; used for healing chronic ulcers and skin diseases. Oil—externally relaxing.

Indian oil sample gave benzyl acetate 26.3, benzyl benzoate 19.2, phytol 10.6, jasmone 8.5, methyl jasmonate 6.3, linalool 5.4, geranyl linalool 3.5, eugenol 2.9, isophytyl acetate 2.7, and isophytol 2.4%.

The leaves gave ascorbic acid, an- thranilic acid and its glucoside, indole oxygenase, alkaloid jasminine and salicylic acid.

The flowers contain pyridine and nicotinate derivatives; tested positive for indole.

The flowers and leaf juice is used for treating tumours.

Singer’s Nodule

A small excrescence on the vocal cords (see LARYNX) which causes hoarseness. This tends to develop in people who abuse their voices – for example, singers, or people who shout excessively.... singer’s nodule

Terminalia spp.

Combretaceae

The genus Terminalia includes a large group of medicinally valuable trees. They belong to the family Combretaceae.

The most important medicinal species of the genus Terminalia are the following.

1) T. arjuna (Roxb.ex DC) Wight & Arn.

San: Arjunah, Kakubhah;

Hin: Arjun, Kahu, Kahua;

Mal: Marutu, Nirmarutu, Venmarutu, Attumarutu, Pulamatti;

Tam: Attumarutu, Nirmarutu, Vellaimarutu, Marutu;

Kan: Maddi.

It is a large evergreen tree commonly found in Madhya Pradesh, Bihar and Peninsular and India. It has buttressed trunk and spreading crown with drooping branches. Bark is smooth, grey outside and flesh coloured inside, flaking off in large flat pieces. Leaves are simple, sub-opposite, oblong or elliptic, coriaceous, crenulate, pale dull green above, pale brown beneath, often unequal sided, nerves 10-15 pairs and reticulate. Flowers are white, arranged in panicles of spikes with linear bracteoles. Fruits are ovoid or oblong with 5-7 short, hard angles or wings, the lines on the wings oblique and curving upward (Warrier et al 1996).

The bark is useful in fractures, ulcers, urethrorrhoea, leucorrhoea, diabetes, vitiated conditions of pitta, anaemia, cardiopathy, fatigue, asthma, bronchitis, tumours, internal and external haemorrhages, cirrhosis of the lever and hypertension. It is used in fractures and the powdered bark is taken with milk. The bark powder is diuretic and has a general tonic effect in cases of cirrhosis of liver. The bark has been considered by the ayurvedic physicians as well as by modern practitioners as a cardiac tonic. It is given as a decoction with milk (NRF, 1998). In Ayurveda, “Arjunaghrita” and “Arjunarishta” are two important cardiotonic preparations of this drug.

Fruits contain flavanones - arjunone and 5,7,2’, 4’ - tetramethoxy flavone and a chalcone - cerasidin. Other constituents are -sitosterol, friedelin, methyloleanolate, gallic acid, ellagic acid and arjunic acid. Bark gave a triterpene arjungenin, triterpene glucosides I, II and III. Stem bark gave flavones - baicalein and arjunolone characterised as 6,4’ - dihydraxy - 7-methoxy flavone. Stem bark yields oxalic acid and tannins besides complex glycosides (Bhatra et al, 1980). Bark is alexertic, styptic, antidysenteric, astringent, antiasthmatic, febrifuge, expectorant, cardiotonic aphrodisiac and diuretic. Fruit is deobstruent. Stem-bark is CVS and CNS active, diuretic and abortifacient. Aerial part is CNS depressant and semen coagulant.

2) T. alata Heyne ex Roth. Syn. T. tomentosa (Roxb. Ex. Dc.) W & A.

This tree is distributed in Himalaya from Kangra eastwords to Goalpara in Assam and southwards throughout the Peninsular India, upto 1200 m. The bark of the tree is widely used in ulcers, fractures, bronchitis and diarrhoea. Hydrolysis of the gum gives oligosaccharides, disaccharides and monosaccharides. Leaves and fruits give -sitosterol. Bark is diuretic, antihaemorrhagic, styptic, cardiotonic and semen coagulant.

3) T. bellirica (Craertn.) Roxb.

San: Aksha, Anilaghanaka, Baheduka, Harya, Kalinda;

Hin: Bulla, Sagona;

Ben: Bahera, Baheri;

Tam: Akkam, Kalanduri, Tani;

Tel: Bhutavasamu Tadi, Tandra, Vibhutakamu.

Belliric Myrobalan is distributed throughout India, upto 900 m. Its bark is used in anaemia and leucoderma. The fruit is used in bronchitis, strangury, sore throat, diseases of eye, nose, heart and bladder, hoarseness and piles. It forms an important constituent of the ayurvedic drug ‘triphala’. Furits contain -sitosterol, gallic and ellagic acids, ethyl gallate, galloyl glucose, chebulagic acid and a cardiac glucoside bellaricanin. Alcoholic extract of the fruit possesses bile-stimulating activity. Alcoholic extract, 30 mg/kg does not affect blood pressure and respiration, but a higher dose of 60 mg/kg produces a fall in blood pressure. Furit has anticancerous and flower has spermicidal activity. Bark is mild diuretic. Fruit is astringent, antidropsical, antileprotic, antiinflammatory, antidiarrhoeal, antibilious, stomachic, antiasthmatic, tonic, anticephalgic, bechic, anthelmintic and attenuant. Kernel is narcotic. Semi -ripe fruit is purgative. Gum is demulcent (Husain et al, 1992)

4) T. bialata steud.

White Chugalam or silver grey wood is a common tree of Andaman Islands. Its bark is used as a cardiac stimulant.

5) T. Catappa Linn.

Indian Almond or Tropical Almond is a popular tree cultivated throughout the warmer parts of India including Andaman Islands and other adjacent island. Oil from the kernel is a substitute for almond oil. The leaf is used in scabies and colic. Husk and endocarp contain tannins and pentosans. Oil from kernel contains oleic, linoleic, palmitic and stearic acids. Heart wood and stem bark contains -sitosterol and its palmitate. Heartwood in addition contains terminolic acid and triterpenic methyl esters. The aerial part of the plant is diuretic. The bark is astringent, mild diuretic, cardiotonic and antidysenteric. Leaf is sudorific, antirheumatic, antileprotic and anticephalalgic.

6) T. Coriacea (Roxb.) syn. T. tomentosa (Roxb. ex. DC.) W. & A. var. coriacea (Roxb.) C. B. Clarke

Leathery Murdah is a tree commonly used as a cardiac stimulant. It is widely distributed in the drier and warmer parts of Andhra Pradesh and Tamil Nadu upto 1350 m and in Central India. Its bark is mainly used as a cardiac stimulant and in atonic diarrhoea and callous ulcer. It is also CVS active.

7) T. myriocarpa Heurck. & Muell. Arg.

Ben: Panisaj; Ass: Hollock, Jhalna.

Hollock is a tree of the Himalayas widely distributed from Nepal to Arunachal Pradesh and in Assam at 1000 m. The bark is cardiac stimulant and mild diuretic. Bark give -sitosterol, fructose and 4,4’,5,5’,6,6’ - hexadydroxy diphenic acid dilactone. Bark also contains tannis - ellagic, gallic, chebulinic and chebulagic acids.

8) T. Pallida Brandis.

The plant is distributed throughout south India, upto 600m. Its bark is a mild diuretic.

9) T. Paniculata Roth.

Tel: Nimiri, Pulamaddi, Putamanu, Pulanallamanu;

Flowering Murdah is a tree which is widely used in opium poisoning. It is distributed in the Western and Eastern Ghats, upto 1200m. The bark is used in parotitis and flowers in opium poisioning. Heart wood give 3, 3’-0-di-methylellagic acid and 3,4,3’0-trimethyl flavellagic acid, -sitosterol, an uncharacterized triterpene carboxylic acid; a glycoside -3,3’ di-0 - methyl ellagic acid - 4 - monoglucoside and 0 - penta methyl flavellogic acid. The stem bark is anticancerous, diuretic, cardiotonic CVS active and shows antagonism of amphetamine hyperactivity. Flower is anticholerin (Husain et al, 1992)

Chebulic myrobalan is a medium deciduous tree, the fruit of which is a common constituent of “Triphala” capable of imparting youthful vitality and receptivity of mind and sense. It is a major constituent in the ayurvedic preparations like Abhayarishta, Abhaya modak, Haritaki khand, Triphaladi churnam and Agastya rasayanam. In allopathy it is used in astringent ointments. In unani system, it is used as a blood purifier. The pulp of the fruit is given in piles, chronic diarrhoea, dysentery, costiveness, flatulence, asthma, urinary disorders, vomiting, hiccup, intestinal worms, ascites and enlarged spleen and liver. Powder of the fruit is used in chronic ulcers and wounds, carious teeth and bleeding ulceration of the gums. The bark is a good cardiac tonic. The fruit is valuable for its tannins and dyes. The wood is used for building purposes, agricultural implements, plywood and match box industries. It is also grown as a shade tree.

The plant is found throughout India chiefly in deciduous forests, on dry slopes upto 900m especially in Bengal, Tamil Nadu, West coast and Western Ghats. The plant is also reported in Sri Lanka, Nepal and Burma.

Terminalia chebula Retz. syn. Myrobalanus chebula (Retz.) Gaertner comes under family Combretaceae. It is a medium sized deciduous tree with a cylindrical bole, rounded crown, spreading branches with dark brown bark and brownish gray heartwood. Leaves are simple, alternate or subopposite, ovate or elliptic ovate with short petioles bearing 2 glands below the blades. Flowers pale yellow or white in 4-10cm long axillary spikes. Calyx tube hairy pale yellow and 5 lobed; no petals. Stamens consist of 10 filaments subulate, anthers small; ovary inferior, 1-celled with 2-3 pendulous ovule. Fruit is a drupe, ovoid glossy, glabrous, faintly angled and yellow to orange brown in colour. Seeds are hard and pale yellow.

Kernel oil of Chebulic myrobalan contains 6 fatty acids viz. Palmitic, stearic, oleic, linoleic, arachidic and behenic acid. The fruits contain chebulinic acid, tannic acid, gallic acid, chebulin and tannin. Leaves contain terpenes and saponins and -sitosterol is present in the bark (Beri, 1970; Khalique and Nizamuddin, 1972; Miglani and Chawla, 1974). Fruits are astringent, purgative, tonic, carminative, alternative and antispasmodic. Flowers and fruits are antiviral and hypoglycaemic. Wood is oxytocic and hypothermic (Husain et al, 1992).

Agrotechnology: Terminalia species are, in general, subtropical trees. Young plants prefer shade while the matured plants tolerate light frost and drought. It grows well in hilly areas. This is propagated by seeds. Natural multiplication happens rarely due to the poor seeds germination. Seeds soaked in water for 48 hours before sowing in seedbeds which should be covered with straw after sowing. It is watered immediately. Usually it takes 3-5 months to germinate. It can be transferred to polybags at two-leaf stage. One-year-old seedlings are ready for transplanting. For transplanting, pits are made of 50cm cube at a spacing of 4m. Organic manure, added regularly, promotes growth. Irrigation is required during first year. Weeds should be removed regularly. This plant grows slowly. It fruits within 6-7 years. This is continued for many years. It is coppiced well. Fruits are collected immediately after falling down or covered with soil to protect it from pests. Fruits dried well in sun and used or stored. Croup

A common condition in infants and young children in which narrowing and inflammation of the airways causes hoarseness, stridor (a grunting noise during breathing), and a barking cough.

Croup may be caused by a viral or bacterial infection that affects the larynx, epiglottis (see epiglottitis), or trachea. Other causes include diphtheria, allergy, spasm caused by deficient calcium in the blood, and inhalation of a foreign body. Most cases are due to a viral infection and are mild.

Humidifying the air can help to make breathing easier. Nebulized corticosteroid drugs (see nebulizer) and oxygen may be prescribed. Infections are treated with antibiotic drugs.... croup

Larynx, Cancer Of

A cancerous tumour of the larynx. The exact causes of this cancer are not known, but smoking and high alcohol consumption may be associated factors. Hoarseness is the main symptom, particularly when the tumour originates on the vocal cords. At an advanced stage, symptoms may include difficulty in breathing and swallowing, and coughing up blood.

If laryngoscopy reveals a tumour on the larynx, a biopsy is carried out.

If the tumour is small, radiotherapy or laser treatment may be used.

For unresponsive and large tumours, partial or total laryngectomy may be considered.... larynx, cancer of

Postnasal Drip

A watery or sticky discharge from the back of the nose into the nasopharynx.

The fluid may cause a cough, hoarseness, or the feeling of a foreign body.

Voice, Loss Of

Inability to speak normally. Temporary partial loss of voice commonly results from straining of the muscles of the larynx through overuse of the voice or from inflammation of the vocal cords in laryngitis. Persistent or recurrent voice loss may be due to polyps on the vocal cords, thickening of the cords in hypothyroidism, or interference with the nerve supply to the larynx muscles due to cancer of the larynx, thyroid gland, or oesophagus. Leprosy

Also known as Hansen’s disease, this is a chronic bacterial infection caused by Mycobacterium leprae affecting the skin, mucous membranes, and nerves. Infection is now almost con?ned to tropical and subtropical countries – mostly in Africa and India. There are two distinct (polarised) clinical forms: tuberculoid and lepromatous. The former usually takes a benign course and frequently burns out, whereas the latter is relentlessly progressive; between these two polar forms lies an intermediate/dimorphous group. Susceptibility may be increased by malnutrition. Nasal secretions (especially in lepromatous disease) are teeming with M. leprae and constitute the main source of infection; however, living in close proximity to an infected individual seems necessary for someone to contract the disease. M. leprae can also be transmitted in breast milk from an infected mother.

Only a small minority of those exposed to M. leprae develop the disease. The incubation period is 3–5 years or longer. The major clinical manifestations involve skin and nerves: the former range from depigmented, often anaesthetic areas, to massive nodules; nerve involvement ranges from localised nerve swelling(s) to extensive areas of anaesthesia. Advanced nerve destruction gives rise to severe deformities: foot-drop, wrist-drop, claw-foot, extensive ulceration of the extremities with loss of ?ngers and toes, and bone changes. Eye involvement can produce blindness. Laryngeal lesions produce hoarseness and more serious sequelae. The diagnosis is essentially a clinical one; however, skin-smears, histological features and the lepromin skin-test help to con?rm the diagnosis and enable the form of disease to be graded.

Although the World Health Organisation had originally hoped to eliminate leprosy worldwide by 2000, that has proved an unrealistic target. The reason is an absence of basic information. Doctors are unable to diagnose the disorder before a patient starts to show symptoms; meanwhile he or she may have already passed on the infection. Doctors do not know exactly how transmission occurs or how it infects humans – nor do they know at what point a carrier of the bacterium may infect others. The incidence of new infections is still more than 650,000 cases a year or about 4.5 cases per 10,000 people in those countries worst affected by the disease.

Treatment Introduction of the sulphone compound, dapsone, revolutionised management of the disease. More recently, rifampicin and clofazimine have been added as ?rst-line drugs for treatment. Second-line drugs include minocycline, o?oxacin and clarithromycin; a number of regimens incorporating several of these compounds (multi-drug regimens – introduced in 1982) are now widely used. A three-drug regime is recommended for multi-bacillary leprosy and a two-drug one for parcibacillary leprosy. Dapsone resistance is a major problem worldwide, but occurs less commonly when multi-drug regimens are used. Older compounds – ethionamide and prothionamide

– are no longer used because they are severely toxic to the liver. Corticosteroids are sometimes required in patients with ‘reversal reaction’. Supportive therapy includes physiotherapy; both plastic and orthopaedic surgery may be necessary in advanced stages of the disease. Improvement in socio-economic conditions, and widespread use of BCG vaccination are of value as preventive strategies. Early diagnosis and prompt institution of chemotherapy should prevent long-term complications.... leprosy

Moringa Pterygosperma

English: Drumstick, Horse-Radish.

Ayurvedic: Shigru (white var.), Madhu Shigru, Sigra, Shobhaan- jana, Haritashaaka. Raktaka, Murangi, Mochaka, Akshiva, Tikshnagandhaa.

Unani: Sahajan.

Siddha/Tamil: Murungai.

Action: All parts of the tree are reported to be used as cardiac and circulatory stimulant. Pods—antipyretic, anthelmintic; fried pods are used by diabetics. Flowers—cholagogue, stimulant, diuretic. Root juice—cardiac tonic, antiepileptic. Used for nervous debility, asthma, enlarged liver and spleen, deep-seated inflammation and as diuretic in calculus affection. Decoction is used as a gargle in hoarseness and sore throat. Root and fruit—antiparalytic. Leaf—juice is used in hiccough (emetic in high doses); cooked leaves are given in influenza and catarrhal affections. Root-bark—antiviral, anti-inflammatory, analgesic. Bark—antifungal, antibacterial. Stem-bark and flower—hypo- glycaemic. Seeds—an infusion, anti-inflammatory, antispasmodic and diuretic; given in venereal diseases.

Along with other therapeutic applications, The Ayurvedic Pharmacopoeia of India indicated the use of the dried root bark in goitre, glycosuria and lipid disorders (also dried seeds), and leaf, seed, root bark and stem bark in internal abscess, piles and fistula-in-ano.

The plant contains antibacterial principles, spirochin and pterygosper- min which are effective against both Gram-Positive and Gram-Negative bacteria.

The leaves contain nitrile glycosides, niazirin and niazirinin and mustard oil glycosides. The mustard oil glycosides showed hypotensive, bradycardiac effects and spasmolytic activity, justifying the use of leaves for gastrointestinal motility disorders.

The roots possess antibacterial, anti- choleric and antiviral properties due to the presence of pterygospermin, Spiro chin and benzylisothiocyanate. The root extract exhibited significant anti- inflammatory activity in carrageenan- induced paw-oedema in rats.

The leaves exhibited hypoglycaemic activity, although the plasma insulin level did not alter much.

The root and bark showed antifer- tility activity through biphasic action on the duration of the estrous cycle of female rats.

Poliomyelitis

Once known as infantile paralysis, this disease is caused by a viral infection involving the BRAIN and SPINAL CORD. Since the development of e?ective vaccines in the 1950s (see IMMUNISATION), polio has been practically eliminated in most developed countries. People who have not been fully vaccinated, however, may get the disease: it remains a serious risk for unvaccinated travellers to Africa, Asia or southern Europe. Most reported cases are now from sub-Saharan Africa.

Pathology There are three types of virus, infection spreading by the stools-contaminated hands-mouth route. Children are most susceptible.

One attack usually produces permanent immunity, and second attacks are rare. The virus typically affects the anterior horn cells of the spinal cord, especially those in the lumbar region; the grey matter of the brain stem and cortex may also be damaged.

Vaccination is given to infants at two, three and four months: a booster dose is given at around the age of ?ve. The vaccine contains all three types of polio virus. Two types of vaccine are available: inactivated polio virus (IPV) contains dead virus and is administered by injections; oral polio vaccine (OPV) contains live, harmless strains. The latter is used in the United Kingdom.

Symptoms The incubation period is around 7–14 days, the onset being marked by a mild fever and headache which improves after a few days. In around 85 per cent of infected children there is no further progression, but in some – after approximately one week – the symptoms recur, together with neck sti?ness and signs of meningeal irritation (see MENINGES). Weakness of individual muscle groups is common, and may progress – to a variable extent, depending on the distribution of the virus – to widespread PARALYSIS. Involvement of the diaphragm and intercostal muscles may lead to respiratory failure and rapid death unless arti?cial respiration is provided. Involvement of the cranial nerves and brain may lead to nystagmus (see under EYE, DISORDERS OF), hoarseness and di?culty in swallowing, and CONVULSIONS may occur in young children. The CEREBROSPINAL FLUID shows an early increase in lymphocytes, followed by a rise in protein concentration.

Treatment There is no e?ective drug treatment for the infection. Treatment involves early bed rest, followed by PHYSIOTHERAPY and orthopaedic measures as required. At the onset of respiratory diffculties a TRACHEOSTOMY and arti?cial ventilation should be started. (In the 1950s, when polio epidemics were occurring, respiratory diffculties were treated by placing patients in an ‘iron lung’ – a large, airtight, cylindrical container in which the air pressure was raised and lowered to simulate normal breathing.) In cases of severe paralysis with persistent wasting of the limbs, surgery may be necessary to minimise the resulting disability.... poliomyelitis

Ribes Nigrum

Linn.

Family: Grossulariaceae.

Habitat: Cold temperate regions extending from Himalayas to northern Asia and Europe.

English: European Black Currant.

Folk: Nabar.

Action: Dried leaves and twigs— a home remedy for coughs. Leaves—diuretic, hypotensive, refrigerant. An infusion is used for inflammatory conditions, sore throat, hoarseness. Fruits— refrigerant, mildly spasmolytic, vasoprotective, anti-inflammatory.

Black currents are very rich in vitamin C (average 150 mg/100g) and contain 0.9-1.7% pectin as calcium pec- tate, also minerals, potassium (372 mg/ 100 g). The acidity of the fruit is mainly due to citric acid; malic acid is present in small amounts. Glucose and fructose are principal sugars; sucrose is a minor component.

The flavonoids in the fruits include kaempferol, quercetin and myricetin. About 0.3% anthocyanosides, concentrated mainly in the skin, consist of glycosides of cyanidol and delphinidol.

The anthocyanosides are reportedly bacteriostatic and exhibit vasopro- tective and anti-inflammatory activity. They are antisecretory against cholera toxin-induced intestinal fluid secretion in vitro.

The leaves contain an anti-inflammatory principle, pycnometol and minute quantities of an essential oil composed mostly of terpenes.

Polyphenolic extract of buds inhibited lipid peroxidation by rat liver mi- crosomes.

Polyphenols present in R. nigrum and R. rubrum (Red Current, Western Himalayas from Kumaon to Kashmir) exhibit free radical scavenging activity. The seed oil lowers VLDL and total cholesterol.

Importance: The sweet flag is an important medhya drug, capable of improving memory power and intellect. It is used in vitiated conditions of vata and kapha, stomatopathy, hoarseness, colic, flatulence, dyspepsia, helminthiasis, amenorrhoea, dismenorrhoea, nephropathy, calculi, strangury, cough, bronchitis, odontalgia, pectoralgia, hepatodynia, otalgia, inflammations, gout, epilepsy, delirium, amentia, convulsions, depression and other mental disorders, tumours, dysentery, hyperdipsia, haemorrhoids, intermittent fevers, skin diseases, numbness and general debility. It is reportedly useful in improving digestion, clearing speech and curing diarrhoea, dysentery, abdominal obstruction and colic. It is also useful in infantile fever, cough bronchitis and asthma. The drug is reported to cure hysteria, insanity and chronic rheumatic complaints. The rhizome is an ingredient of preparations like Vacaditaila, Ayaskrti, Kompancadi gulika, Valiya rasnadi kashaya, etc.

Distribution: The plant is a native of Europe. It is distributed throughout the tropics and subtropics, especially in India and Sri Lanka. It is found in marshes, wild or cultivated, ascending the Himalayas upto 1800m in Sikkim. It is plentiful in marshy tracts of Kashmir and Sirmoor, in Manipur and Naga Hills.

Botany: Acorus calamus Linn. belonging to the family Araceae is a semi -aquatic rhizomatous perennial herb. Rhizome is creeping, much branched, cylindrical or slightly compressed, light brown or pinkish brown externally, white and spongy within. Leaves are bright green, distichous, ensiform, base equitant, thickened in the middle and with wavy margins. Flowers are light brown and densely packed in sessile cylindric spadix. Fruits are oblong, turbinate berries with a pyramidal top. Seeds are few and pendant from the apex of the cells (Warrier et al, 1993).

Another species belonging to the genus Acorus is A. gramineus Soland, the roots of which are used in tonic, antiseptics and insecticidal preparations (Chopra et al, 1956).

Agrotechnology: Acorus is a hardy plant found growing from tropical to subtropical climates. It needs a good and well distributed rainfall throughout the year. It needs ample sunlight during the growth period as well as after harvest for drying the rhizomes. It may be cultivated in any good but fairly moist soil. It is usually grown in areas where paddy can be grown. It comes up well in clayey soils and light alluvial soils of river bank. The field is laid out and prepared exactly as for rice, irrigated sufficiently and after ploughing twice, watered heavily and again ploughed in the puddle. Sprouted rhizome pieces are used for planting and pressed into the mud at a depth of about 5cm at a spacing of 30x30cm. The rhizomes are planted in such a way that the plants in the second row comes in between the plants of the first row and not opposite to them. FYM is to be applied at 25t/ha. Fertilisers are applied at 25:50:60 kg N:P2O5:K2O/ha/yr. Whole of FYM and 1/3 of N, P2O5 and K2O are to be added in the field during March - April as a basal dose. The remaining 2/3 of nutrients is to be given in two equal split doses at 4 months and 8 months after planting. The field is to be regularly irrigated. About 5 cm of standing water is to be maintained in the field in the beginning. Later, it is to be increased to 10 cm as the plant grows. The field is to be regularly weeded. About 8 weedings are to be carried out in all. At each weeding the plants are pressed into the soil. The plant is attacked by mealy bugs. Both shoot and root mealy bugs can be controlled by spraying the shoot and drenching the roots of grown up plants with 10 ml Methyl parathion or 15ml Oxydemeton methyl or 20ml Quinalphos in 10 litres of water. The crop is ready for harvest at the end of first year. The field is to be dried partially so that sufficient moisture is left in the soil to facilitate deep digging. The leaves start turning yellow and dry, indicating maturity. The rhizome will be at a depth of 60cm and having about 30-60cm spread. Therefore, harvesting is to be done carefully. The rhizomes are to be cut into 5-7.5cm long pieces and all the fibrous roots are to be removed. Yield of rhizome is about 10t/ha (Farooqi et al, 1991).

Properties and Activity:Rhizomes, roots and leaves yield essential oil. The important constituents of the Indian oil are asarone and its -isomer. Other constituents are and -pinene, myrcene, camphene, p-cymene, camphor and linalool, sesquiterpenic ketones like asarone, calamone, calacone, acolamone, iso-acolamone, acoragermacrone, epishyobunone, shyobunone and iso- shyobunone. Alcohol present is preisocalamendiol. Sesquiterpene hydrocarbons like elemene, elemane and calarene are also present. Tricyclic sesquiterpenes present are caryophyllene, humulene, guaiene, S-guaizulene, arcurcumene, -cadinene, cadinane, calamenene, calacorene, dihydrocalacorene(calamenene), cadalene and selinene. Roots yield acoric acid as a main constituent in addition to choline. Plant also yields a flavone diglycoside- luteolin 6,8-C-diglucoside.

-asarone is the ma jor constituent of essential oil from rhizome (Dandiya et al, 1958,1959; Raquibuddoula, 1967).

Rhizome is insecticidal, pisicidal, spasmolytic, hypothermic, CNS active and analgesic. Essential oil is anticonvulsant. Rhizome is acrid, bitter, thermogenic, aromatic, intellect promoting, emetic, laxative, carminative, stomachic, anthelmintic, emmenagogue, diuretic, alexeteric, expectorant, anodyne, antispasmodic, aphrodisiac, antiinflammatory, sudorific, antipyretic, sialagogue, insecticidal, tranquillizer, sedative, analgesic, antithermic, antiasthmatic, hypotensive, respiratory depressant, aperitive and tonic.... sweet flag

Tussilago Farfara

Linn.

Family: Compositae; Asteraceae.

Habitat: Western Himalayas from Kashmir to Nepal at 1,500-3,500 m.

English: Coughwort, Coltsfoot, Asses' Foot.

Unani: Fanjiyun.

Action: Leaves and flowers— anticatarrhal, antitussive, expectorant, antispasmodic, demulcent, anti-inflammatory. Used for dry, unproductive, irritative cough, smoker's cough, whooping cough, bronchial asthma (effect shortlived), laryngitis.

Key application: In acute catarrh of the respiratory tract with cough and hoarseness; acute, mild inflammation of the oral and pharyngeal mucosa. Contraindicated during pregnancy and nursing. (German Commission E.)

The leaves and flowers contain flavo- noids including rutin, hyperoside and isoquercetin; pyrrolizidine alkaloids including senkirkine and tussilagine (about 0.015%); mucilage (about 8%) consisting of polysaccharides based on glucose, galactose, fructose, arabinose and xylose; inulin, tannins.

Polysaccharides are antiinflammatory and immuno-stimulating, as well as demulcent. Flavonoids also have anti-inflammatory and antispasmod- ic action. Pyrrolizidine alkaloids have caused hepatotoxicity in rats fed daily on high doses, but not on daily low dose regimes. These are largely destroyed when the herb is boiled.

The leaf contains an inhibitor of platelet activating factor (PAF). The PAF inhibitor can account for Coltsfoot's efficacy in asthma. The constituent, tussilagone has respiratory stimulant and cardiovascular (including pressor) activities. Cancer - Bronchial Carcinoma

The most common form of cancer throughout the world. Five year survival: 10 per cent. Its association with cigarette smoking is now established beyond doubt. Other causes include such occupational poisons as asbestos, arsenic, chromium, diesel fumes, etc. The squamous cell carcinoma is the most common of the four types.

Diagnosis is confirmed by sputum test, chest X-ray, bronchoscopy or biopsy. Earliest symptoms are persistent cough, pain in the chest, hoarseness of voice and difficulty of breathing. Physical examination is likely to reveal sensitivity and swelling of lymph nodes under arms.

Symptoms. Tiredness, lack of energy, possible pains in bones and over liver area. Clubbing of finger-tips indicate congestion of the lungs. Swelling of arms, neck and face may be obvious. A haematologist may find calcium salts in the blood. The supportive action of alteratives, eliminatives and lymphatic agents often alleviate symptoms where the act of swallowing has not been impaired.

Broncho-dilators (Lobelia, Ephedra, etc) assist breathing. Mullein has some reputation for pain relief. To arrest bleeding from the lesion (Blood root).

According to Dr Madaus, Germany, Rupturewort is specific on lung tissue. To disperse sputum (Elecampane, Red Clover). In advanced cases there may be swollen ankles and kidney breakdown for which Parsley root, Parsley Piert or Buchu may be indicated. Cough (Sundew, Irish Moss). Soft cough with much sputum (Iceland Moss). To increase resistance (Echinacea).

Alternatives. Secondary to primary treatment. Of possible value.

Teas. Violet leaves, Mullein leaves, Yarrow leaves, Gotu Kola leaves, White Horehound leaves. Flavour with a little Liquorice if unpalatable.

Tablets/capsules. Lobelia, Iceland Moss, Echinacea, Poke root.

Formula. Equal parts: Violet, Red Clover, Garden Thyme, Yarrow, Liquorice. Dose: Powders: 750mg (three 00 capsules or half a teaspoon. Liquid Extracts: 1-2 teaspoons. Tinctures: 1-3 teaspoons. Thrice daily, and during the night if relief is sought.

Practitioner. Tinctures BHP (1983). Ephedra 4; Red Clover 4, Yellow Dock 2; Bugleweed 2; Blood root quarter; Liquorice quarter (liquid extract). Mix. Start low: 30-60 drops in water before meals and at bedtime increasing to maximum tolerance level.

Aromatherapy. Oils: Eucalyptus or Thyme on tissue to assist breathing. Inhale.

Cancer

An invasive growth which gradually emerges into life and, undisciplined, eats its way into neighbouring tissues. Malignancy is the growth of abnormal cells with the ability to form a primary lesion from which cells may be bloodborne to other parts of the body (metastasis). Growth usually follows the line of the lymph vessels (Violet leaves have an affinity for lymph vessels).

Course of the disease is unpredictable, cases surviving for many years on primary or supportive herbal treatment. Suspected malignancy should be referred to modern hospital treatment immediately. Early detection is vital.

Common signs calling attention are: (1) Unusual bleeding or discharge. (2) Tired feeling all the time. (3) Thickening or lump in breast or elsewhere. (4) Sudden change in hair texture and colour. (5) Irritable cough or hoarseness. (6) Extreme mental depression. (7) Obvious change in a mole or wart. (8) Muscle weakness and cramps. (9) A sore that does not heal. (10) Change in bowel or bladder habit. (11) Sudden weakness of the eyes. (12) Difficulties in swallowing; indigestion. (13) Excess wind in stomach or bowel.

Tumour-killing effect of chemotherapy may be intensified and side-effects minimised (loss of weight, and of white blood cells) when certain neoplastic herbs are prescribed. Cytotoxic drugs inhibit the ability of Vitamin C to stimulate the body’s defences. Herbs enhance the body’s self-healing ability to eliminate. An inoperable cancer would appear to be good grounds for herbal medication which often relieves pain and preserves a man’s dignity in his hour of extremity.

A series of medical trials in Finland revealed that terminal cases had 12 per cent lower mean serum selenium concentration than controls. Other similar trials point to the need for selenium supplements. Those with both low selenium and low Vitamin E levels are especially at risk.

Plants with a special reference to cancer include: Blue Flag, Burdock, Clivers, Condurango, Echinacea, Guaiacum, Houseleek, Poke root. There are many more referred to in medical literature.

Poke Root. John Bartram reported in the late 18th century that from his experience among the Mohawk Indians, Poke root (Phytolacca decandra) was a “cure” for cancer. (American Indian Medicine, Virgil J. Vogel)

Blood Root. For internal or external bleeding of cancer.

Calendula (Marigold). For the same purpose.

Mistletoe. Dr Alfred Vogel advises an extract of the plant (Loranthus europaeus) as grown on the Oak tree: dose: 10-15 drops.

Almonds. Edgar Cayce, Virginia Beach, USA, with some successes to his credit, advised eating three almonds a day to counter any tendency towards the disease.

Laetrile. From Apricot kernels that contain cyanogenic glucosides. Though competent physicians have reported positive results in some terminal cases without prior surgery or radiation, the remedy has been withdrawn from general practice because of possible toxicity.

Much needless suffering may be incurred because of out-moded resistance of doctors and governments against prescribing morphine early in cancer patients. It is estimated that 50-80 per cent of patients do not receive satisfactory pain-relief because doctors fear tolerance of the drug would increase, necessitating a higher dosage. From the beginning of time the Opium Poppy has been the most effective analgesic for the terminal condition. Morphine is a respiratory depressant and some authorities believe it should be given before the final stages in continuous doses for adequate pain control. Risks must be balanced with benefits. Way of Life. Herbal medication of malignant disease involves the patient with his treatment. Here is something he or she can do to regain some control over their life. It can give them the satisfaction of knowing that in some way they are ‘fighting back’ thus influencing the quality of life and a sense of well- being.

If improvement in cancer is not possible maybe the condition can be stabilised and the patient helped to cope with the very unpleasant side-effects of chemotherapy and radiation. Thus, may be restored the body’s natural balance and a possible extension of lifespan.

For this, patients and practitioners may need information and support. That is why suggestions for malignant disease are included in this book. Moreover, well-meaning friends and relatives may exert pressure on the patient ‘to leave no stone unturned’ in search of a cure. Thus every possible secondary treatment should be considered since any one may prove to contribute towards recovery. Cancer – Larynx

Chiefly due to continued inflammation from faulty use of the voice, smoking, drugs or infection.

Symptoms. Cough, hoarseness, difficult swallowing. ‘Always clearing the throat.’ Differs from a polyp or papilloma on the vocal chords which are benign.

Of possible value:– Teas. Balm, Chamomile, Gotu Kola, Red Clover, Red Sage, Yarrow. Plantain (Arthur Hyde MNIMH)

Tea (mild analgesic). Mix equal parts: Balm and German Chamomile. 1 heaped teaspoon to each cup boiling water; infuse 5 minutes. 1 cup freely.

Tablets/capsules. Blue Flag root, Echinacea, Poke root.

Formula. Echinacea 2; Mullein 2; Goldenseal quarter. Mix. Dose: Powders: 750mg (three 00 capsules or half a teaspoon). Liquid extracts: 1-2 teaspoons. Tinctures: 1-3 teaspoons. Thrice daily and at bedtime. Diet. Slippery Elm gruel.

Horse Radish

Cochlearia armoracia L. Armoracia rusticana, Gaertn. Part used: root.

Constituents: asparagine, B vitamins, Vitamin C, sinigrin and other glucosinolates, resin.

Hoarseness (1 teaspoon juice in honey). Rheumatic joints (poultice). Common cold, influenza and early stages of fever: cup of Horse Radish tea every 2-3 hours. Tea: 1 teaspoon grated fresh root in each cup boiling water; infuse 20 minutes. Half-1 cup in sips, freely. Horse Radish vinegar. 1oz scraped fresh root to 1 pint cider vinegar. 1-2 teaspoons in water for catarrh, sinusitis, poor circulation or as a male tonic.

Steeping slices of the fresh root in cider produces a copious discharge of urine in dropsy.

Laryngitis, Acute

 Inflammation of the vocal cords. May be associated with the common cold, influenza, and other viral or bacterial infections.

Causes: smoking, mis-use of the voice in talking or singing (Ginseng).

Symptoms: voice husky or absent (aphonia). Talking causes pain. Self-limiting.

Treatment. Stop talking for 2 days. Care is necessary: neglect or ineffective treatment may rouse infection and invade the windpipe and bronchi resulting in croup.

Differential: croup is alerted by high fever and characteristic cough, requiring hospital treatment. Alternatives. Teas: Red Sage. Garden Sage. Thyme, wild or garden.

Effective combination: equal parts, Sage and Raspberry leaves. Used also as a gargle.

Tablets/capsules. Poke root. Lobelia. Iceland Moss.

Cinnamon. Tincture, essence or oil of: 3-5 drops in teaspoon honey.

Horseradish. 1oz freshly scraped root to steep in cold water for two hours. Add 2 teaspoons runny honey. Dose: 2-3 teaspoons every two hours.

Topical. Equal parts water/cider vinegar cold pack round throat. Renew when dry.

Laryngitis, Chronic

 The main symptom is hoarseness or loss of voice from malfunction of the vocal cords by disease, stroke, stress, or nerve disorder. Pain on speaking. “Raw throat.”

Constitutional disturbance: fever, malaise.

Many causes, including: drugs, drinking spirits. Gross mis-use of voice (singing or talking) may produce nodules (warts) on the cords. The smoker has inflammatory changes. Nerve paralysis in the elderly. Carcinoma of the larynx. Voice changes during menstruation are associated with hormonal changes (Agnus Castus). Professional singers, members of choirs benefit from Irish Moss, Iceland Moss, Slippery Elm or Poke root.

Alternatives. Cayenne, Caraway seed, Balm of Gilead, Lungwort, Queen’s Delight, Thyme, Wild Indigo, Marsh Cudweed, Mullein, Marshmallow.

For most infections: Equal parts, Tinctures Goldenseal and Myrrh: 3-5 drops in water 3-4 times daily; use also as a spray or gargle.

Tea. Formula. Equal parts: Mullein, Marshmallow root, Liquorice. 2 teaspoons to each cup water brought to boil; vessel removed on boiling. Drink freely.

To prevent voice damage. The voice should not be strained by talking too much, shouting or singing – especially with a cold. Try not to cough or keep clearing the throat but instead, swallow firmly. Do not whisper – it will strain the voice.

A common cause of laryngitis is growth of a nodule, cyst or polyp on the vocal cords. They are visible on use of an endoscope. There are two vocal cords which, in speech, come together and vibrate like a reed in a musical instrument. In formation of a nodule they cannot meet, air escapes and the voice becomes hoarse. Budesonide

An inhaled corticosteroid drug used in the treatment of bronchial asthma to prevent asthma attacks. It is administered using an inhaler. Side effects of budesonide, which include hoarseness, throat irritation and, rarely, fungal infections, can be reduced by rinsing the mouth after administration.

Buerger’s disease A rare disorder, also called thromboangiitis obliterans, in which the arteries, nerves, and veins in the legs, and sometimes those in the arms, become severely inflamed. Blood supply to the toes and fingers becomes cut off, eventually causing gangrene. The disease is most common in men under the age of 45 who smoke heavily. bulimia An illness that is characterized by bouts of overeating usually followed by self-induced vomiting or excessive laxative use. Most sufferers are girls or women between the ages of 15 and 30. In some cases, the symptoms coexist with those of anorexia nervosa. Repeated vomiting can lead to dehydration and loss of potassium, causing weakness and cramps, and tooth damage due to the gastric acid in vomit. Pharynx, Cancer Of

A cancerous tumour of the pharynx. Pharyngeal cancer usually develops in the mucous membrane lining. In the West, almost all cases of pharyngeal cancer are related to smoking and to drinking alcohol. The incidence rises with age, and the disorder is more common in men.

Cancerous tumours of the oropharynx (the middle section of the pharynx) usually cause difficulty swallowing, often with a sore throat and earache. Bloodstained sputum may be coughed up. Sometimes there is only the feeling of a lump in the throat or a visible enlarged lymph node in the neck. Cancer of the laryngopharynx (the lowermost part of the pharynx) initially causes a sensation of incomplete swallowing, then a muffled voice, hoarseness, and increased difficulty in swallowing. Tumours of the nasopharynx have different causes.Diagnosis of cancer of the pharynx is made by biopsy, often in conjunction with laryngoscopy, bronchoscopy, or oesophagoscopy.

Speech Disorders

Defects or disturbances in speech that lead to an inability to communicate effectively.

Damage to the language centres of the brain (usually due to a stroke, head injury, or brain tumour) leads to aphasia, which may impair speech.

Disorders of articulation may be caused by damage to nerves that go to muscles in the larynx, mouth or lips, due to stroke, head injury, multiple sclerosis, or Parkinson’s disease. A structural abnormality of the mouth, such as a cleft lip and palate, can also be a cause.

Disorders of voice production include hoarseness and inappropriate pitch or loudness. In many cases, the cause is a disorder affecting closure of the vocal cords (see larynx, disorders of). A voice that is too high or low or too loud or soft may be caused by a hormonal or psychiatric disturbance or by hearing loss.

Disorders of fluency include stuttering, which is marked by hesitant speech and repetition of sounds.

Delayed speech development in a child may be due to hearing loss (see deafness), slow maturation of the nervous system, poor tongue and lip control, lack of stimulation, or emotional disturbance (see developmental delay).

Trachea

The air passage, also called the windpipe, that runs from immediately below the larynx to behind the upper part of the sternum, where it divides to form the bronchi. The trachea is made of fibrous and elastic tissue and smooth muscle. It also contains about 20 rings of cartilage, which keep it open. The lining of the trachea has cells (goblet cells) that secrete mucus and cells with cilia,which beat the mucus upwards to help keep the lungs and airways clear. tracheitis Inflammation of the trachea. Tracheitis is usually caused by a viral infection and is aggravated by inhaled fumes, especially tobacco smoke. Greenland Moss

Rhododendron groenlandicum

FAMILY: Ericaceae

SYNONYMS: Ledum, Labrador tea, marsh tea, swamp tea, bog Labrador tea, rusty Labrador tea, Hudson’s Bay tea (formerly Ledum groenlandicum).

GENERAL DESCRIPTION: This beautiful, hardy plant is a shrub belonging to the heather family, which can reach one metre in height. It is recognizable by its thick, leathery evergreen leaves whose edges coil under and are quite unique, being deep green on top with a downy-fuzz beneath. New leaves have a woolly mat of white hairs underneath; mature leaves have reddish hairs. All leaves are dotted with resinous glands and are fragrant, with a pungent scent, when crushed. The fluffy white flowers, which are borne in spring, are also strongly aromatic.

DISTRIBUTION: The plant is native to North America, from Greenland and Labrador across to Alaska, as far north as the treeline. It is absent from the far North and the dry prairies. The Latin name groenlandicum refers to the fact that it grows in Greenland where it is still widespread, often growing in dense colonies.

OTHER SPECIES: Ledum is a genus name, which includes 8 species of evergreen shrubs native to cool temperate and sub-arctic regions of the Northern Hemisphere, commonly known as Labrador tea. The common name Ledum is also applied to Rhododendron tomentsum subsp. subarcticum (formerly L. decumbens) which is known as Northern Labrador tea. This species, which is similar but slightly smaller, grows farther north on tundra at up to 1,800 metres and contains toxic alkaloids known to be poisonous to livestock. It lacks the characteristic fuzz on the underside of the mature leaves and the flowers of L. groenlandicum.

HERBAL/FOLK TRADITION: This strongly aromatic herb has been used in folk medicine for centuries. Brewed as a medicinal beverage known as Labrador tea, it was used by practically all Canada’s First Nations peoples as a tonic and to treat certain respiratory, digestive and kidney ailments; as a remedy for headaches and various types of rheumatism; and to facilitate childbirth. The herbal tea also served to clean wounds and was applied to insect bites. It is said that the plant was used for over 5,000 years by the native people of North America, to protect themselves from scurvy, and the Cree used it for fevers and colds: indeed it was regarded as a ‘cure-all’ by the indigenous people. In the fur-trading era, the French Canadian coureurs-de-bois used Labrador tea to extend their supplies of black tea: it thus became a substitute for unaffordable Chinese tea during times of economic crisis. However, like other plants in the heather family, Greenland moss contains an andromedo-toxin that can cause poisoning if used in excess.

According to recent clinical trials, Greenland moss essential oil has a natural affinity for the immune system and can be an effective immune system supporter. The oil also helps counteract blood toxicity and aids liver regeneration, valuable in cases of liver intoxication originating from circulation disorders, viral hepatitis, enteritis and cirrhosis (fatty liver). Clinical research suggests that the essential oil functions like an enzyme in the liver, digesting toxic waste and fat molecules. It is also indicated for obesity, oedema, water retention and thyroid regulation. PRINCIPAL CONSTITUENTS: The main chemical constituents are limonene (up to 35 per cent), sabinene, selinene, bornyl acetate with other monoterpenes and sesquiterpenes.

SAFETY DATA: Possible skin sensitization: always dilute for topical use. Avoid during pregnancy and by children. NB: Abusive consumption of the tea derived from its leaves may cause indigestion, and may even have a toxic effect due to the high level of tannins that it contains.

Jasmine

Jasminum officinale

FAMILY: Oleaceae

SYNONYMS: Jasmin, jessamine, common jasmine, poet’s jessamine.

GENERAL DESCRIPTION: An evergreen shrub or vine up to 10 metres high with delicate, bright green leaves and star-shaped very fragrant white flowers.

DISTRIBUTION: Native to China, northern India and west Asia; cultivated in the Mediterranean region, China and India (depending on the exact species). The concrete is produced in Italy, France, Morocco, Egypt, China, Japan, Algeria and Turkey; the absolute is mainly produced in France.

OTHER SPECIES: There are many species of jasmine used for medicine and perfumery work. Apart from the common jasmine, the most widespead varieties are the royal or Italian jasmine (J. grandiflorum) which is grown in the Mediterranean region, and its Eastern counterpart J. officinale var. grandiflorum or J. auriculatum. HERBAL/FOLK TRADITION: In China the flowers of J. officinale var. grandiflorum are used to treat hepatitis, liver cirrhosis and dysentery; the flowers of J. sambac are used for conjunctivitis, dysentery, skin ulcers and tumours. The root is used to treat headaches, insomnia, pain due to dislocated joints and rheumatism.

In the West, the common jasmine was said to ‘warm the womb ... and facilitate the birth; it is useful for cough, difficulty of breathing, etc. It disperses crude humours, and is good for cold and catarrhous constitutions, but not for the hot.’ It was also used for hard, contracted limbs and problems with the nervous and reproductive systems.

ACTIONS: Analgesic (mild), antidepressant, anti-inflammatory, antiseptic, antispasmodic, aphrodisiac, carminative, cicatrisant, expectorant, galactagogue, parturient, sedative, tonic (uterine).

EXTRACTION: A concrete is produced by solvent extraction; the absolute is obtained from the concrete by separation with alcohol. An essential oil is produced by steam distillation of the absolute.

CHARACTERISTICS: The absolute is a dark orange-brown, viscous liquid with an intensely rich, warm, floral scent and a tealike undertone. It blends well with rose, sandalwood, clary sage, and all citrus oils. It has the ability to round off any rough notes and blend with virtually everything.

PRINCIPAL CONSTITUENTS: There are over 100 constituents in the oil including benzyl acetate, linalol, phenylacetic acid, benzyl alcohol, farnesol, methyl anthranilate, cis jasmone, methyl jasmonate, among others.

SAFETY DATA: Non-toxic, non-irritant, generally non-sensitizing. (An allergic reaction has been known to occur in some individuals.)

AROMATHERAPY/HOME: USE

Skin care: Dry, greasy, irritated, sensitive skin.

Orris

Iris pallida

FAMILY: Iridaceae

SYNONYMS: Orris root, iris, flag iris, pale iris, orris butter (oil).

GENERAL DESCRIPTION: A decorative perennial plant up to 1.5 metres high, with sword-shaped leaves, a creeping fleshy rootstock and delicate, highly scented, pale blue flowers.

DISTRIBUTION: Native to the eastern Mediterranean region; also found in northern India and North Africa. Most commercial orris is produced in Italy where it grows wild. The oil is mainly produced in France and Morocco and to lesser extent in Italy and the USA.

OTHER SPECIES: There are many species of iris; cultivation has also produced further types. In Italy the pale iris (I. pallida) is collected indiscriminately with the Florentine orris (I. florentina) which has white flowers tinged with pale blue, and the common or German iris (I. germanica) which has deep purple flowers with a yellow beard. Other species which have been used medicinally include the American blue flag (I. versicolor), and the yellow flag iris (I. pseudacorus).

HERBAL/FOLK TRADITION: In ancient Greece and Rome orris root was used extensively in perfumery, and its medicinal qualities were held in high esteem by Dioscorides. The juice of the root was used for cosmetic purposes, and the root bruised in wine was employed for dropsy, bronchitis, coughs, hoarseness, chronic diarrhoea and congested headaches. In Russia the root was used to make a tonic drink with honey and ginger.

Iris is little used medicinally these days, but it still appears in the British Herbal Pharmacopoeia as being formerly used in upper respiratory catarrh, coughs, and for diarrhoea in infants.

ACTIONS: Dried Root – antidiarrhoeal, demulcent, expectorant. Fresh Root – diuretic, cathartic, emetic.

EXTRACTION: 1. An essential oil (often called a ‘concrete’) by steam distillation from the rhizomes which have been peeled, washed, dried and pulverized. The rhizomes must be stored for a minimum of three years prior to extraction otherwise they have virtually no scent! 2. An absolute produced by alkali washing in ethyl ether solution to remove the myristic acid from the ‘concrete’ oil. 3. A resin or resinoid by alcohol extraction from the peeled rhizomes.

CHARACTERISTICS: 1. The oil solidifies at room temperature to a cream-coloured mass with a woody, violet-like scent and a soft, floral-fruity undertone. 2. The absolute is a water-white or pale yellow oily liquid with a delicate, sweet, floral-woody odour. 3. The resin is a brown or dark orange viscous mass with a deep, woody sweet, tobacco-like scent – very tenacious.

Orris blends well with cedarwood, sandalwood, vetiver, cypress, mimosa, labdanum, bergamot, clary sage, rose, violet and other florals.

PRINCIPAL CONSTITUENTS: Myristic acid, an odourless substance which makes the ‘oil’ solid (85–90 per cent), alpha-irone and oleic acid.

SAFETY DATA: The fresh root causes nausea and vomiting in large doses. The oil and absolute are much adulterated or synthetic – ‘true’ orris absolute is three times the price of jasmine.

AROMATHERAPY/HOME: USE None. However, the powdered orris, which is a common article, may be used as a dry shampoo, a body powder, a fixative for pot pourris, and to scent linen.

OTHER USES: The powder is used to scent dentifrices, toothpowders, etc. The resin is used in soaps, colognes and perfumes; the absolute and ‘concrete’ oil are reserved for high-class perfumery work. Occasionally used on the Continent for confectionery and fruit flavours.... orris
