Impotence: From 5 Different Sources
Absence of sexual power. Lack of sexual performance in the male whereby erection cannot be maintained to complete a satisfactory orgasm between two people. Or failure to ejaculate. Impotence puts a marriage under serious strain. Marriage Guidance Council: “About a third of sex problems we deal with concern impotence.” Pre-disposing factors: stress, marital problems, diabetes, drugs. Some cases are due to piles, fissures and other diseases of the rectum. First relieve the rectum and the impotence should look after itself. Smoking.
Alternatives. With the use of Ginseng and other plant extracts it is possible for sexual desire not to diminish with age, even in the eighties. Some have children in their seventies. Reduce alcohol to a minimum. Drunk males are poor performers. Aphrodisiacs: Damiana, Siberian Ginseng, Saw Palmetto, Muira-pauama. Inability to achieve or maintain an erection, usually caused by psychological factors, including concerns about performance or relationship difficulties. In about 1 in 10 cases, the cause is a physical disorder, such as diabetes mellitus, or a neurological disorder, such as damage to the spinal cord. Some drugs cause impotence as a side effect, but this reverses when the drugs are stopped.
Treatment may include counselling or sex therapy for psychological problems.
The drug sildenafil is used to treat both organic and psychological impotence, but side effects may occur.
Other treatments include self-administered injections into the penis and a surgical implant, which can produce a sustained erection.
Inability to perform the sexual act due to failure of the reflex mechanism
Inability of the male to perform the sexual act. It may be partial or complete, temporary or permanent. Psychological factors are the most common cause and these include anxiety, ignorance, fear, guilt, weakness of sexual desire or abnormality of such desire. Counselling or sex therapy, preferably with the partner, has a 50-per-cent chance of helping to cure long-term impotence of psychological origin. Among organic causes are lesions (see LESION) of the external genitalia; disturbances of the ENDOCRINE GLANDS, such as diminished activity of the gonads, thyroid gland or pituitary gland; diseases of the central NERVOUS SYSTEM; any severe disturbance of health, such as DIABETES MELLITUS; and addiction to alcohol.
An oral drug for treating erectile function is sildena?l citrate (Viagra®), the ?rst in a new class of drugs called phosphodiesterase type 5 inhibitors, also including tadala?l (Cialis®) and vardena?l (Levitra®). They work by improving blood ?ow to the penis. They can be taken an hour before intercourse (up to 12 hours before, in the case of tadala?l). These drugs are not aphrodisiacs, and side-effects include headache, facial ?ushing and indigestion. There are some suggestions that they may affect retinal function.
Intracavernosal injection or urethral application of alprostadil, a drug which increases local blood supply to the penis, has been used for some years under medical supervision, but success has been variable and oral sildena?l seems to be a more convenient and e?ective treatment for a man with this disorder.

Finasteride is a drug which inhibits the ENZYME that metabolises TESTOSTERONE into the more potent ANDROGEN, dihydrotestosterone. This action results in a reduction of prostate tissue. The drug is used to treat an enlarged PROSTATE GLAND, thus improving urinary ?ow. Its side-effects include reduced LIBIDO, and IMPOTENCE. Finasteride o?ers an alternative to PROSTATECTOMY for some men but a signi?cant minority do not improve. Women of childbearing age should not handle broken or crushed tablets.... finasteride
A colourless liquid, also called ethanol or ethyl-alcohol, produced by the fermentation of carbohydrates by yeast. Medically, alcohol is used as a solvent and an antiseptic; recreationally it is a widely used drug, taken in alcoholic drinks to give a pleasant taste as well as to relax, reduce inhibitions, and increase sociability. Taken to excess, alcohol causes much mental and physical harm – not just to the individual imbibing it, but often to their family, friends, community and work colleagues.
Alcohol depresses the central nervous system and disturbs both mental and physical functioning. Even small doses of alcohol will slow a person’s re?exes and concentration; potentially dangerous effects when, for example, driving or operating machinery. Drunkenness causes slurred speech, muddled thinking, amnesia (memory loss), drowsiness, erectile IMPOTENCE, poor coordination and dulled reactions – thereby making driving or operating machinery especially dangerous. Disinhibition may lead to extreme euphoria, irritability, misery or aggression, depending on the underlying mood at the start of drinking. Severe intoxication may lead to COMA and respiratory failure.
Persistent alcohol misuse leads to physical, mental, social and occupational problems, as well as to a risk of DEPENDENCE (see also ALCOHOL DEPENDENCE). Misuse may follow several patterns: regular but controlled heavy intake, ‘binge’ drinking, and dependence (alcoholism). The ?rst pattern usually leads to mainly physical problems such as gastritis, peptic ulcer, liver disease, heart disease and impotence. The second is most common among young men and usually leads to mainly social and occupational problems – getting into ?ghts, jeopardising personal relationships, overspending on alcohol at weekends, and missing days o? work because of hangovers. The third pattern – alcohol dependence – is the most serious, and can severely disrupt health and social stability.
Many researchers consider alcohol dependence to be an illness that runs in families, with a genetic component which is probably passed on as a vulnerable personality. But it is hard to disentangle genetic, environmental and social factors in such families. In the UK there are estimated to be around a million people suffering from alcohol dependence and a similar number who have di?culty controlling their consumption (together about 1:30 of the population).
Alcohol causes tolerance and both physical and psychological dependence (see DEPENDENCE for de?nitions). Dependent drinkers classically drink early in the morning to relieve overnight withdrawal symptoms. These symptoms include anxiety, restlessness, nausea and vomiting, and tremor. Sudden withdrawal from regular heavy drinking can lead to life-threatening delirium tremens (DTs), with severe tremor, hallucinations (often visual – seeing spiders and monsters, rather than the pink elephants of romantic myth), and CONVULSIONS. This must be treated urgently with sedative drugs, preferably by intravenous drip. Similar symptoms, plus severe INCOORDINATION and double-vision, can occur in WERNICKE’S ENCEPHALOPATHY, a serious neurological condition due to lack of the B vitamin thiamine (whose absorption from the stomach is markedly reduced by alcohol). If not treated urgently with injections of thiamine and other vitamins, this can lead to an irreversible form of brain damage called Korsako?’s psychosis, with severe amnesia. Finally, prolonged alcohol misuse can cause a form of dementia.
In addition to these severe neurological disorders, the wide range of life-threatening problems caused by heavy drinking includes HEPATITIS, liver CIRRHOSIS, pancreatitis (see PANCREAS, DISEASES OF), gastrointestinal haemorrhage, suicide and FETAL ALCOHOL SYNDROME; pregnant women should not drink alcohol as this syndrome may occur with more than a glass of wine or half-pint of beer a day. The social effects of alcohol misuse – such as marital breakdown, family violence and severe debt – can be equally devastating.
Treatment of alcohol-related problems is only moderately successful. First, many of the physical problems are treated in the short term by doctors who fail to spot, or never ask about, heavy drinking. Second, attempts at treating alcohol dependence by detoxi?cation or ‘drying out’ (substituting a tranquillising drug for alcohol and withdrawing it gradually over about a week) are not always followed-up by adequate support at home, so that drinking starts again. Home support by community alcohol teams comprising doctors, nurses, social workers and, when appropriate, probation o?cers is a recent development that may have better results. Many drinkers ?nd the voluntary organisation Alcoholics Anonymous (AA) and its related groups for relatives (Al-Anon) and teenagers (Alateen) helpful because total abstinence from alcohol is encouraged by intensive psychological and social support from fellow ex-drinkers.

The counselling and treatment of individuals with psychosexual dysfunction (see SEXUAL DYSFUNCTION). Around half of couples experience some type of sexual problem during their relationships, and for most of them the diffculties are psychological. Sexual therapy is usually given to both partners, but sometimes individual counselling is necessary. Couples may sometimes ?nd that group therapy is helpful. A noncancerous tumour of the pituitary gland that causes overproduction of prolactin. In women, this may result in galactorrhoea, amenorrhoea, or infertility. In men, it may cause impotence and gynaecomastia. In either sex, it may cause headaches, diabetes insipidus, and, if the tumour presses on the optic nerves, loss of the outer visual field. About the Nutrients in This Food
Beer and ale are fermented beverages created by yeasts that convert the sugars in malted barley and grain to ethyl alcohol (a.k.a. “alcohol,” “drink- ing alcohol”).*
The USDA /Health and Human Services Dietary Guidelines for Americans defines one drink as 12 ounces of beer, five ounces of wine, or
1.25 ounces of distilled spirits. One 12-ounce glass of beer has 140 calo- ries, 86 of them (61 percent) from alcohol. But the beverage—sometimes nicknamed “liquid bread”—is more than empty calories. Like wine, beer retains small amounts of some nutrients present in the food from which it was made.
* Because yeasts cannot digest t he starches in grains, t he grains to be used in mak ing beer and ale are allowed to germinate ( “malt” ). Storing This Food
Store beer in a cool place. Beer tastes best when consumed within two months of the day it is made. Since you cannot be certain how long it took to ship the beer to the store or how long it has been sitting on the grocery shelves, buy only as much beer as you plan to use within a week or two.
Protect bottled beer and open bottles or cans of beer from direct sunlight, which can change sulfur compounds in beer into isopentyl mercaptan, the smelly chemical that gives stale beer its characteristic unpleasant odor.
When You Are Ready to Serve This Food
Serve beer only in absolutely clean glasses or mugs. Even the slightest bit of grease on the side of the glass will kill the foam immediately. Wash beer glasses with detergent, not soap, and let them drain dry rather than drying them with a towel that might carry grease from your hands to the glass. If you like a long-lasting head on your beer, serve the brew in tall, tapering glasses to let the foam spread out and stabilize.
For full flavor, serve beer and ales cool but not ice-cold. Very low temperatures immo- bilize the molecules that give beer and ale their flavor and aroma.
What Happens When You Cook This Food
When beer is heated (in a stew or as a basting liquid), the alcohol evaporates but the flavor- ing agents remain intact. Alcohol, an acid, reacts with metal ions from an aluminum or iron pot to form dark compounds that discolor the pot or the dish you are cooking in. To prevent this, prepare dishes made with beer in glass or enameled pots.
Medical Uses and/or Benefits
Reduced risk of heart attack. Data from the American Cancer Society’s Cancer Prevention Study 1, a 12-year survey of more than 1 million Americans in 25 states, shows that men who take one drink a day have a 21 percent lower risk of heart attack and a 22 percent lower risk of stroke than men who do not drink at all. Women who have up to one drink a day also reduce their risk of heart attack. Numerous later studies have confirmed these findings.
Lower risk of stroke. In January 1999, the results of a 677-person study published by researchers at New York Presbyterian Hospital-Columbia University showed that moder- ate alcohol consumption reduces the risk of stroke due to a blood clot in the brain among older people (average age: 70). How the alcohol prevents stroke is still unknown, but it is clear that moderate use of alcohol is a key. Heavy drinkers (those who consume more than seven drinks a day) have a higher risk of stroke. People who once drank heavily, but cut their consumption to moderate levels, can also reduce their risk of stroke. Numerous later studies have confirmed these findings.
Lower cholesterol levels. Beverage alcohol decreases the body’s production and storage of low-density lipoproteins (LDLs), the protein and fat particles that carr y cholesterol into your arteries. As a result, people who drink moderately tend to have lower cholesterol levels and higher levels of high density lipoproteins (HDLs), the fat and protein particles that carr y cholesterol out of the body. The USDA /Health and Human Services Dietar y Guidelines for Americans defines moderation as two drinks a day for a man, one drink a day for a woman.
Stimulating the appetite. Alcoholic beverages stimulate the production of saliva and the gastric acids that cause the stomach contractions we call hunger pangs. Moderate amounts of alcoholic beverages, which may help stimulate appetite, are often prescribed for geriatric
patients, convalescents, and people who do not have ulcers or other chronic gastric problems that might be exacerbated by the alcohol.
Dilation of blood vessels. Alcohol dilates the capillaries (the tiny blood vessels just under the skin), and moderate amounts of alcoholic beverages produce a pleasant flush that temporar- ily warms the drinker. But drinking is not an effective way to warm up in cold weather since the warm blood that flows up to the capillaries will cool down on the surface of your skin and make you even colder when it circulates back into the center of your body. Then an alco- hol flush will make you perspire, so that you lose more heat. Excessive amounts of beverage alcohol may depress the mechanism that regulates body temperature.
Adverse Effects Associated with This Food
Increased risk of breast cancer. In 2008, scientists at the National Cancer Institute released data from a seven-year survey of more than 100,000 postmenopausal women showing that even moderate drinking (one to two drinks a day) may increase by 32 percent a woman’s risk of developing estrogen-receptor positive (ER+) and progesterone-receptor positive (PR+) breast cancer, tumors whose growth is stimulated by hormones. No such link was found between consuming alcohol and the risk of developing ER-/PR- tumors (not fueled by hor- mones). The finding applies to all types of alcohol: beer, wine, and spirits.
Increased risk of oral cancer (cancer of the mouth and throat). Numerous studies confirm the American Cancer Society’s warning that men and women who consume more than two drinks a day are at higher risk of oral cancer than are nondrinkers or people who drink less. Note: The Dietary Guidelines for Americans describes one drink as 12 ounces of beer, five ounces of wine, or 1.5 ounces of distilled spirits.
Increased risk of cancer of the colon and rectum. In the mid-1990s, studies at the University of Oklahoma suggested that men who drink more than five beers a day are at increased risk of rectal cancer. Later studies suggested that men and women who are heavy beer or spirits drinkers (but not those who are heavy wine drinkers) have a higher risk of colorectal cancers. Further studies are required to confirm these findings.
Fetal alcohol syndrome. Fetal alcohol syndrome is a specific pattern of birth defects—low birth weight, heart defects, facial malformations, and mental retardation—first recognized in a study of babies born to alcoholic women who consumed more than six drinks a day while pregnant. Subsequent research has found a consistent pattern of milder defects in babies born to women who consume three to four drinks a day or five drinks on any one occasion while pregnant. To date, there is no evidence of a consistent pattern of birth defects in babies born to women who consume less than one drink a day while pregnant, but two studies at Columbia University have suggested that as few as two drinks a week while preg- nant may raise a woman’s risk of miscarriage. (“One drink” means 12 ounces of beer, five ounces of wine, or 1.25 ounces of distilled spirits.)
Alcoholism. Alcoholism is an addiction disease, the inability to control one’s alcohol consumption. It is a potentially life-threatening condition, with a higher risk of death by accident, suicide, malnutrition, or acute alcohol poisoning, a toxic reaction that kills by para- lyzing body organs, including the heart.
Malnutrition. While moderate alcohol consumption stimulates appetite, alcohol abuse depresses it. In addition, an alcoholic may drink instead of eating. When an alcoholic does eat, excess alcohol in his/her body prevents absorption of nutrients and reduces the ability to synthesize new tissue.
Hangover. Alcohol is absorbed from the stomach and small intestine and carried by the bloodstream to the liver, where it is oxidized to acetaldehyde by alcohol dehydrogenase (ADH), the enzyme our bodies use to metabolize the alcohol we produce when we digest carbohydrates. The acetaldehyde is converted to acetyl coenzyme A and either eliminated from the body or used in the synthesis of cholesterol, fatty acids, and body tissues. Although individuals vary widely in their capacity to metabolize alcohol, on average, normal healthy adults can metabolize the alcohol in one quart of beer in approximately five to six hours. If they drink more than that, they will have more alcohol than the body’s natural supply of ADH can handle. The unmetabolized alcohol will pile up in the bloodstream, interfering with the liver’s metabolic functions. Since alcohol decreases the reabsorption of water from the kidneys and may inhibit the secretion of an antidiuretic hormone, they will begin to urinate copiously, losing magnesium, calcium, and zinc but retaining more irritating uric acid. The level of lactic acid in the body will increase, making them feel tired and out of sorts; their acid-base balance will be out of kilter; the blood vessels in their heads will swell and throb; and their stomachs, with linings irritated by the alcohol, will ache. The ultimate result is a “hangover” whose symptoms will disappear only when enough time has passed to allow their bodies to marshal the ADH needed to metabolize the extra alcohol in their blood.
Changes in body temperature. Alcohol dilates capillaries, tiny blood vessels just under the skin, producing a “flush” that temporarily warms the drinker. But drinking is not an effective way to stay warm in cold weather. Warm blood flowing up from the body core to the surface capillaries is quickly chilled, making you even colder when it circulates back into your organs. In addition, an alcohol flush triggers perspiration, further cooling your skin. Finally, very large amounts of alcohol may actually depress the mechanism that regulates body temperature.
Impotence. Excessive drinking decreases libido (sexual desire) and interferes with the ability to achieve or sustain an erection.
“Beer belly.” Data from a 1995, 12,000 person study at the University of North Carolina in Chapel Hill show that people who consume at least six beers a week have more rounded abdomens than people who do not drink beer. The question left to be answered is which came first: the tummy or the drinking.
Food/Drug Interactions
Acetaminophen (Tylenol, etc.). The FDA recommends that people who regularly have three or more drinks a day consult a doctor before using acetaminophen. The alcohol/acetamino- phen combination may cause liver failure.
Disulfiram (Antabuse). Taken with alcohol, disulfiram causes flushing, nausea, low blood pressure, faintness, respiratory problems, and confusion. The severity of the reaction gener- ally depends on how much alcohol you drink, how much disulfiram is in your body, and how long ago you took it. Disulfiram is used to help recovering alcoholics avoid alcohol. (If taken with alcohol, metronidazole [Flagyl], procarbazine [Matulane], quinacrine [Atabrine], chlorpropamide (Diabinase), and some species of mushrooms may produce a mild disulfi- ramlike reaction.)
Anticoagulants. Alcohol slows the body’s metabolism of anticoagulants (blood thinners) such as warfarin (Coumadin), intensif ying the effect of the drugs and increasing the risk of side effects such as spontaneous nosebleeds.
Antidepressants. Alcohol may increase the sedative effects of antidepressants. Drinking alcohol while you are taking a monoamine oxidase (M AO) inhibitor is especially hazard- ous. M AO inhibitors inactivate naturally occurring enzymes in your body that metabolize tyramine, a substance found in many fermented or aged foods. Tyramine constricts blood vessels and increases blood pressure. If you eat a food containing tyramine while you are taking an M AO inhibitor, you cannot effectively eliminate the tyramine from your body. The result may be a hypertensive crisis. Ordinarily, fermentation of beer and ale does not produce tyramine, but some patients have reported tyramine reactions after drinking some imported beers. Beer and ale are usually prohibited to those using M AO inhibitors.
Aspirin, ibuprofen, ketoprofen, naproxen, and nonsteroidal anti-inflammatory drugs. Like alcohol, these analgesics irritate the lining of the stomach and may cause gastric bleeding. Combining the two intensifies the effect.
Insulin and oral hypoglycemics. Alcohol lowers blood sugar and interferes with the metabo- lism of oral antidiabetics; the combination may cause severe hypoglycemia.
Sedatives and other central nervous system depressants (tranquilizers, sleeping pills, antidepres- sants, sinus and cold remedies, analgesics, and medication for motion sickness). Alcohol inten- sifies sedation and, depending on the dose, may cause drowsiness, respiratory depression, coma, or death.... beer
Diabetes mellitus is a condition characterised by a raised concentration of glucose in the blood due to a de?ciency in the production and/or action of INSULIN, a pancreatic hormone made in special cells called the islet cells of Langerhans.
Insulin-dependent and non-insulindependent diabetes have a varied pathological pattern and are caused by the interaction of several genetic and environmental factors.
Insulin-dependent diabetes mellitus (IDDM) (juvenile-onset diabetes, type 1 diabetes) describes subjects with a severe de?ciency or absence of insulin production. Insulin therapy is essential to prevent KETOSIS – a disturbance of the body’s acid/base balance and an accumulation of ketones in the tissues. The onset is most commonly during childhood, but can occur at any age. Symptoms are acute and weight loss is common.
Non-insulin-dependent diabetes mellitus (NIDDM) (maturity-onset diabetes, type 2 diabetes) may be further sub-divided into obese and non-obese groups. This type usually occurs after the age of 40 years with an insidious onset. Subjects are often overweight and weight loss is uncommon. Ketosis rarely develops. Insulin production is reduced but not absent.
A new hormone has been identi?ed linking obesity to type 2 diabetes. Called resistin – because of its resistance to insulin – it was ?rst found in mice but has since been identi?ed in humans. Researchers in the United States believe that the hormone may, in part, explain how obesity predisposes people to diabetes. Their hypothesis is that a protein in the body’s fat cells triggers insulin resistance around the body. Other research suggests that type 2 diabetes may now be occurring in obese children; this could indicate that children should be eating a more-balanced diet and taking more exercise.
Diabetes associated with other conditions (a) Due to pancreatic disease – for example, chronic pancreatitis (see PANCREAS, DISORDERS OF); (b) secondary to drugs – for example, GLUCOCORTICOIDS (see PANCREAS, DISORDERS OF); (c) excess hormone production
– for example, growth hormone (ACROMEGALY); (d) insulin receptor abnormalities; (e) genetic syndromes (see GENETIC DISORDERS).
Gestational diabetes Diabetes occurring in pregnancy and resolving afterwards.
Aetiology Insulin-dependent diabetes occurs as a result of autoimmune destruction of beta cells within the PANCREAS. Genetic in?uences are important and individuals with certain HLA tissue types (HLA DR3 and HLA DR4) are more at risk; however, the risks associated with the HLA genes are small. If one parent has IDDM, the risk of a child developing IDDM by the age of 25 years is 1·5–2·5 per cent, and the risk of a sibling of an IDDM subject developing diabetes is about 3 per cent.
Non-insulin-dependent diabetes has no HLA association, but the genetic in?uences are much stronger. The risks of developing diabetes vary with di?erent races. Obesity, decreased exercise and ageing increase the risks of disease development. The risk of a sibling of a NIDDM subject developing NIDDM up to the age of 80 years is 30–40 per cent.
Diet Many NIDDM diabetics may be treated with diet alone. For those subjects who are overweight, weight loss is important, although often unsuccessful. A diet high in complex carbohydrate, high in ?bre, low in fat and aiming towards ideal body weight is prescribed. Subjects taking insulin need to eat at regular intervals in relation to their insulin regime and missing meals may result in hypoglycaemia, a lowering of the amount of glucose in the blood, which if untreated can be fatal (see below).
Oral hypoglycaemics are used in the treatment of non-insulin-dependent diabetes in addition to diet, when diet alone fails to control blood-sugar levels. (a) SULPHONYLUREAS act mainly by increasing the production of insulin;
(b) BIGUANIDES, of which only metformin is available, may be used alone or in addition to sulphonylureas. Metformin’s main actions are to lower the production of glucose by the liver and improve its uptake in the peripheral tissues.
Complications The risks of complications increase with duration of disease.
Diabetic hypoglycaemia occurs when amounts of glucose in the blood become low. This may occur in subjects taking sulphonylureas or insulin. Symptoms usually develop when the glucose concentration falls below 2·5 mmol/l. They may, however, occur at higher concentrations in subjects with persistent hyperglycaemia – an excess of glucose – and at lower levels in subjects with persistent hypo-glycaemia. Symptoms include confusion, hunger and sweating, with coma developing if blood-sugar concentrations remain low. Re?ned sugar followed by complex carbohydrate will return the glucose concentration to normal. If the subject is unable to swallow, glucagon may be given intramuscularly or glucose intravenously, followed by oral carbohydrate, once the subject is able to swallow.
Although it has been shown that careful control of the patient’s metabolism prevents late complications in the small blood vessels, the risk of hypoglycaemia is increased and patients need to be well motivated to keep to their dietary and treatment regime. This regime is also very expensive. All risk factors for the patient’s cardiovascular system – not simply controlling hyperglycaemia – may need to be reduced if late complications to the cardiovascular system are to be avoided.
Diabetes is one of the world’s most serious health problems. Recent projections suggest that the disorder will affect nearly 240 million individuals worldwide by 2010 – double its prevalence in 1994. The incidence of insulin-dependent diabetes is rising in young children; they will be liable to develop late complications.
Although there are complications associated with diabetes, many subjects live normal lives and survive to an old age. People with diabetes or their relatives can obtain advice from Diabetes UK (www.diabetes.org.uk).
Increased risks are present of (a) heart disease, (b) peripheral vascular disease, and (c) cerebrovascular disease.
Diabetic eye disease (a) retinopathy, (b) cataract. Regular examination of the fundus enables any abnormalities developing to be detected and treatment given when appropriate to preserve eyesight.
Nephropathy Subjects with diabetes may develop kidney damage which can result in renal failure.
Neuropathy (a) Symmetrical sensory polyneuropathy; damage to the sensory nerves that commonly presents with tingling, numbness of pain in the feet or hands. (b) Asymmetrical motor diabetic neuropathy, presenting as progressive weakness and wasting of the proximal muscles of legs. (c) Mononeuropathy; individual motor or sensory nerves may be affected. (d) Autonomic neuropathy, which affects the autonomic nervous system, has many presentations including IMPOTENCE, diarrhoea or constipation and postural HYPOTENSION.
Skin lesions There are several skin disorders associated with diabetes, including: (a) necrobiosis lipoidica diabeticorum, characterised by one or more yellow atrophic lesions on the legs;
(b) ulcers, which most commonly occur on the feet due to peripheral vascular disease, neuropathy and infection. Foot care is very important.
Diabetic ketoacidosis occurs when there is insu?cient insulin present to prevent KETONE production. This may occur before the diagnosis of IDDM or when insu?cient insulin is being given. The presence of large amounts of ketones in the urine indicates excess ketone production and treatment should be sought immediately. Coma and death may result if the condition is left untreated.
Symptoms Thirst, POLYURIA, GLYCOSURIA, weight loss despite eating, and recurrent infections (e.g. BALANITIS and infections of the VULVA) are the main symptoms.
However, subjects with non-insulindependent diabetes may have the disease for several years without symptoms, and diagnosis is often made incidentally or when presenting with a complication of the disease.
Treatment of diabetes aims to prevent symptoms, restore carbohydrate metabolism to as near normal as possible, and to minimise complications. Concentration of glucose, fructosamine and glycated haemoglobin in the blood are used to give an indication of blood-glucose control.
Insulin-dependent diabetes requires insulin for treatment. Non-insulin-dependent diabetes may be treated with diet, oral HYPOGLYCAEMIC AGENTS or insulin.
Insulin All insulin is injected – mainly by syringe but sometimes by insulin pump – because it is inactivated by gastrointestinal enzymes. There are three main types of insulin preparation: (a) short action (approximately six hours), with rapid onset; (b) intermediate action (approximately 12 hours); (c) long action, with slow onset and lasting for up to 36 hours. Human, porcine and bovine preparations are available. Much of the insulin now used is prepared by genetic engineering techniques from micro-organisms. There are many regimens of insulin treatment involving di?erent combinations of insulin; regimens vary depending on the requirements of the patients, most of whom administer the insulin themselves. Carbohydrate intake, energy expenditure and the presence of infection are important determinants of insulin requirements on a day-to-day basis.
A new treatment for diabetes, pioneered in Canada and entering its preliminary clinical trials in the UK, is the transplantation of islet cells of Langerhans from a healthy person into a patient with the disorder. If the transplantation is successful, the transplanted cells start producing insulin, thus reducing or eliminating the requirement for regular insulin injections. If successful the trials would be a signi?cant advance in the treatment of diabetes.
Scientists in Israel have developed a drug, Dia Pep 277, which stops the body’s immune system from destroying pancratic ? cells as happens in insulin-dependent diabetes. The drug, given by injection, o?ers the possibility of preventing type 1 diabetes in healthy people at genetic risk of developing the disorder, and of checking its progression in affected individuals whose ? cells are already perishing. About the Nutrients in This Food
Spirits are the clear liquids produced by distilling the fermented sugars of grains, fruit, or vegetables. The yeasts that metabolize these sugars and convert them into alcohol stop growing when the concentration of alcohol rises above 12–15 percent. In the United States, the proof of an alcoholic beverage is defined as twice its alcohol content by volume: a beverage with
20 percent alcohol by volume is 40 proof.
This is high enough for most wines, but not high enough for most whiskies, gins, vodkas, rums, brandies, and tequilas. To reach the concentra- tion of alcohol required in these beverages, the fermented sugars are heated and distilled. Ethyl alcohol (the alcohol in beer, wine, and spirits) boils at a lower temperature than water. When the fermented sugars are heated, the ethyl alcohol escapes from the distillation vat and condenses in tubes leading from the vat to a collection vessel. The clear liquid that collects in this vessel is called distilled spirits or, more technically, grain neutral spirits.
Gins, whiskies, cordials, and many vodkas are made with spirits
American whiskeys (which include bourbon, rye, and distilled from grains.
blended whiskeys) and Canadian, Irish, and Scotch whiskies are all made from spirits aged in wood barrels. They get their flavor from the grains and their color from the barrels. (Some whiskies are also colored with caramel.)
Vodka is made from spirits distilled and filtered to remove all flavor. By law, vodkas made in America must be made with spirits distilled from grains. Imported vodkas may be made with spirits distilled either from grains or potatoes and may contain additional flavoring agents such as citric acid or pepper. Aquavit, for example, is essentially vodka flavored with caraway seeds. Gin is a clear spirit flavored with an infusion of juniper berries and other herbs (botanicals). Cordials (also called liqueurs) and schnapps are flavored spirits; most are sweetened with added sugar. Some cordials contain cream.
Rum is made with spirits distilled from sugar cane (molasses). Tequila is made with spirits distilled from the blue agave plant. Brandies are made with spirits distilled from fruit. (Arma- gnac and cognac are distilled from fermented grapes, calvados and applejack from fermented apples, kirsch from fermented cherries, slivovitz from fermented plums.)
Unless they contain added sugar or cream, spirits have no nutrients other than alcohol. Unlike food, which has to be metabolized before your body can use it for energy, alcohol can be absorbed into the blood-stream directly from the gastrointestinal tract. Ethyl alcohol provides 7 calories per gram.

All spirits except unflavored vodkas contain volatile molecules that give the beverage its characteristic taste and smell. Warming the liquid excites these molecules and intensifies the flavor and aroma, which is the reason we serve brandy in a round glass with a narrower top that captures the aromatic molecules as they rise toward the air when we warm the glass by holding it in our hands. Whiskies, too, though traditionally served with ice in America, will have a more intense flavor and aroma if served at room temperature.
What Happens When You Cook This Food
The heat of cooking evaporates the alcohol in spirits but leaves the flavoring intact. Like other alcoholic beverages, spirits should be added to a recipe near the end of the cooking time to preserve the flavor while cooking away any alcohol bite.
Alcohol is an acid. If you cook it in an aluminum or iron pot, it will combine with metal ions to form dark compounds that discolor the pot and the food you are cooking. Any recipe made with spirits should be prepared in an enameled, glass, or stainless-steel pot.
Medical Uses and/or Benefits
Reduced risk of heart attack. Data from the American Cancer Society’s Cancer Prevention Study 1, a 12-year survey of more than 1 million Americans in 25 states, shows that men who take one drink a day have a 21 percent lower risk of heart attack and a 22 percent lower risk of stroke than men who do not drink at all. Women who have up to one drink a day also reduce their risk of heart attack. Numerous later studies have confirmed these findings.
Lower cholesterol levels. Beverage alcohol decreases the body’s production and storage of low density lipoproteins (LDLs), the protein and fat particles that carry cholesterol into your arteries. As a result, people who drink moderately tend to have lower cholesterol levels and higher levels of high density lipoproteins (HDLs), the fat and protein particles that carry cholesterol out of the body. Numerous later studies have confirmed these findings.
Lower risk of stroke. In January 1999, the results of a 677-person study published by researchers at New York Presbyterian Hospital-Columbia University showed that moderate alcohol consumption reduces the risk of stroke due to a blood clot in the brain among older people (average age: 70). How alcohol prevents stroke is still unknown, but it is clear that moderate use is a key. Heavy drinkers (those who consume more than seven drinks a day) have a higher risk of stroke. People who once drank heavily, but cut their consumption to moderate levels, reduce their risk of stroke.
Stimulating the appetite. Alcoholic beverages stimulate the production of saliva and the gastric acids that cause the stomach contractions we call hunger pangs. Moderate amounts of alcoholic beverages, which may help stimulate appetite, are often prescribed for geriatric patients, convalescents, and people who do not have ulcers or other chronic gastric problems that might be exacerbated by the alcohol.
Dilation of blood vessels. Alcoholic beverages dilate the tiny blood vessels just under the skin, bringing blood up to the surface. That’s why moderate amounts of alcoholic beverages (0.2–1 gram per kilogram of body weight, or two ounces of whiskey for a 150-pound adult) temporarily warm the drinker. But the warm blood that flows up to the surface of the skin will cool down there, making you even colder when it circulates back into the center of your body. Then an alcohol flush will make you perspire, so you lose more heat. Excessive amounts of beverage alcohol may depress the mechanism that regulates body temperature.
Adverse Effects Associated with This Food
Alcoholism. Alcoholism is an addiction disease, the inability to control one’s alcohol consumption. It is a potentially life-threatening condition, with a higher risk of death by accident, suicide, malnutrition, or acute alcohol poisoning, a toxic reaction that kills by para- lyzing body organs, including the heart.
Fetal alcohol syndrome. Fetal alcohol syndrome is a specific pattern of birth defects—low birth weight, heart defects, facial malformations, learning disabilities, and mental retarda- tion—first recognized in a study of babies born to alcoholic women who consumed more than six drinks a day while pregnant. Subsequent research has found a consistent pattern of milder defects in babies born to women who drink three to four drinks a day or five drinks on any one occasion while pregnant. To date there is no evidence of a consistent pattern of birth defects in babies born to women who consume less than one drink a day while preg- nant, but two studies at Columbia University have suggested that as few as two drinks a week while pregnant may raise a woman’s risk of miscarriage. (One drink is 12 ounces of beer, five ounces of wine, or 1.25 ounces of distilled spirits.)
Increased risk of breast cancer. In 2008, scientists at the National Cancer Institute released data from a seven-year survey of more than 100,000 postmenopausal women showing that even moderate drinking (one to two drinks a day) may increase by 32 percent a woman’s risk of developing estrogen-receptor positive (ER+) and progesterone-receptor positive (PR+) breast cancer, tumors whose growth is stimulated by hormones. No such link was found between consuming alcohol and the risk of developing ER-/PR- tumors (not fueled by hor- mones). The finding applies to all types of alcohol: beer, wine, and distilled spirits.
Increased risk of oral cancer (cancer of the mouth and throat). Numerous studies confirm the A merican Cancer Societ y’s warn ing that men and women who consume more than t wo drinks a day are at higher risk of oral cancer than are nondrinkers or people who drink less.
Increased risk of cancer of the colon and rectum. In the mid-1990s, studies at the University of Oklahoma suggested that men who drink more than five beers a day are at increased risk of rectal cancer. Later studies suggested that men and women who are heavy beer or spirits drinkers (but not those who are heavy wine drinkers) have a higher risk of colorectal cancers. Further studies are required to confirm these findings.
Malnutrition. While moderate alcohol consumption stimulates appetite, alcohol abuses depresses it. In addition, an alcoholic may drink instead of eating. When an alcoholic does eat, excess alcohol in his/her body prevents absorption of nutrients and reduces the ability to synthesize new tissue.
Hangover. Alcohol is absorbed from the stomach and small intestine and carried by the bloodstream to the liver, where it is oxidized to acetaldehyde by alcohol dehydrogenase (ADH), the enzyme our bodies use every day to metabolize the alcohol we produce when we digest carbohydrates. The acetaldehyde is converted to acetyl coenzyme A and either eliminated from the body or used in the synthesis of cholesterol, fatty acids, and body tis- sues. Although individuals vary widely in their capacity to metabolize alcohol, an adult of average size can metabolize the alcohol in four ounces (120 ml) whiskey in approximately five to six hours. If he or she drinks more than that, the amount of alcohol in the body will exceed the available supply of ADH. The surplus, unmetabolized alcohol will pile up in the bloodstream, interfering with the liver’s metabolic functions. Since alcohol decreases the reabsorption of water from the kidneys and may inhibit the secretion of an antidiuretic hormone, the drinker will begin to urinate copiously, losing magnesium, calcium, and zinc but retaining uric acid, which is irritating. The level of lactic acid in the body will increase, making him or her feel tired and out of sorts; the acid-base balance will be out of kilter; the blood vessels in the head will swell and throb; and the stomach, its lining irritated by the alcohol, will ache. The ultimate result is a hangover whose symptoms will disappear only when enough time has passed to allow the body to marshal the ADH needed to metabolize the extra alcohol in the person’s blood.
Changes in body temperature. Alcohol dilates capillaries, tiny blood vessels just under the skin, producing a “flush” that temporarily warms the drinker. But drinking is not an effective way to stay warm in cold weather. Warm blood flowing up from the body core to the surface capillaries is quickly chilled, making you even colder when it circulates back into your organs. In addition, an alcohol flush triggers perspiration, further cooling your skin. Finally, very large amounts of alcohol may actually depress the mechanism that regulates body temperature.
Impotence. Excessive drinking decreases libido (sexual desire) and interferes with the ability to achieve or sustain an erection.
Migraine headache. Some alcoholic beverages contain chemicals that inhibit PST, an enzyme that breaks down certain alcohols in spirits so that they can be eliminated from the body. If they are not broken down by PST, these alcohols will build up in the bloodstream and may trigger a migraine headache. Gin and vodka appear to be the distilled spirits least likely to trigger headaches, brandy the most likely.
Food/Drug Interactions
Acetaminophen (Tylenol, etc.). FDA recommends that people who regularly have three or more drinks a day consult a doctor before using acetaminophen. The alcohol/acetaminophen combination may cause liver failure.
Anti-alcohol abuse drugs (disulfiram [Antabuse]). Taken concurrently with alcohol, the anti- alcoholism drug disulfiram can cause flushing, nausea, a drop in blood pressure, breathing difficulty, and confusion. The severity of the symptoms, which may var y among individu- als, generally depends on the amount of alcohol consumed and the amount of disulfiram in the body.
Anticoagulants. Alcohol slows the body’s metabolism of anticoagulants (blood thinners), intensif ying the effect of the drugs and increasing the risk of side effects such as spontane- ous nosebleeds.
Antidepressants. Alcohol may strengthen the sedative effects of antidepressants.
Aspirin, ibuprofen, ketoprofen, naproxen and nonsteroidal anti-inflammatory drugs. Like alco- hol, these analgesics irritate the lining of the stomach and may cause gastric bleeding. Com- bining the two intensifies the effect.
Insulin and oral hypoglycemics. Alcohol lowers blood sugar and interferes with the metabo- lism of oral antidiabetics; the combination may cause severe hypoglycemia.
Sedatives and other central nervous system depressants (tranquilizers, sleeping pills, antide- pressants, sinus and cold remedies, analgesics, and medication for motion sickness). Alcohol intensifies the sedative effects of these medications and, depending on the dose, may cause drowsiness, sedation, respiratory depression, coma, or death.
MAO inhibitors. Monoamine oxidase (M AO) inhibitors are drugs used as antidepressants or antihypertensives. They inhibit the action of natural enzymes that break down tyramine, a substance formed naturally when proteins are metabolized. Tyramine is a pressor amine, a chemical that constricts blood vessel and raises blood pressure. If you eat a food that contains tyramine while you are taking an M AO inhibitor, the pressor amine cannot be eliminated from your body and the result may be a hypertensive crisis (sustained elevated blood pressure). Brandy, a distilled spirit made from wine (which is fermented) contains tyramine. All other distilled spirits may be excluded from your diet when you are taking an M AO inhibitor because the spirits and the drug, which are both sedatives, may be hazard- ous in combination.... distilled spirits
The rigid state of the PENIS when it responds to sexual stimulus. An erection is necessary for e?ective penetration of the VAGINA. As a result of sexual arousal, the three cylinders of erectile tissue in the penis become engorged with blood, lengthening, raising and hardening the penis. Muscles surrounding the blood vessels contract and retain the blood in the penis. Erections also occur during sleep and in young boys. Inability to have or maintain an erection is one cause of IMPOTENCE (see also SILDENAFIL CITRATE).... erection
These widely used drugs include a range of different preparations which relieve DEPRESSION. All the antidepressants available at the time of writing are more or less equally e?ective. In studies where patients agree to take either antidepressants or identical dummy PLACEBO pills (without knowing which), at least two-thirds of those who receive antidepressants feel much better within three months, while fewer than one-third of those on placebos recover naturally in the same period. In general these drugs are useful for severe and moderate depression including postnatal illness; they are not e?ective in milder forms of depression although they may be tried for a short time if other therapies have failed.
The most widely prescribed type of antidepressants are the tricyclics, so-called because their molecular structure includes three rings. The other commonly used types are named after the actions they have on chemicals in the brain: the SELECTIVE SEROTONIN-REUPTAKE INHIBITORS (SSRIS) and the MONOAMINE OXIDASE INHIBITORS (MAOIS) – see also below. All types of antidepressant work in similar ways. Tricyclic antidepressants have cured depression in millions of people, but they can cause unpleasant side-effects, particularly in the ?rst couple of weeks. These include SEDATION, dry mouth, excessive sweating, CONSTIPATION, urinary problems, and impotence (inability to get an erection). Up to half of all people prescribed tricyclic drugs cannot tolerate the side-effects and stop treatment before their depression is properly treated. More seriously, tricyclics can upset the rhythm of the heart in susceptible people and should never be given in the presence of heart disease.
The SSRIs are newer, coming into wide use in the late 1980s. They increase the levels in the brain of the chemical messenger SEROTONIN, which is thought to be depleted in depression. Indeed, the SSRIs are as e?ective as tricyclics and, although they can cause nausea and excessive sweating at ?rst, they generally have fewer side-effects. Their main disadvantage, however, is that they cost much more than the most commonly used tricyclic, amitriptyline. On the other hand, they are more acceptable to many patients and they cause fewer drop-outs from treatment – up to a quarter rather than a half. The money saved by completed, successful treatment may outweigh the prescribing costs. SSRIs have been reported as associated with an increased risk of suicide.
Another group of antidepressants, the MAOIs, have been in use since the late 1950s.
They are stimulants, rather than sedatives, and are particularly helpful for people who are physically and mentally slowed by depression. They work well but have one big disadvantage – a dangerous interaction with certain foods and other drugs, causing a sudden and very dangerous increase in blood pressure. People taking them must carry an information card explaining the risk and listing the things that they should avoid. Because of this risk, MAOIs are not used much now, except when other treatments have failed. A new MAOI, moclobemide, which is less likely to interact and so cause high blood pressure, is now available.
LITHIUM CARBONATE is a powerful antidepressant used for intractable depression. It should be used under specialist supervision as the gap between an e?ective dose and a toxic one is narrow.
St John’s Wort is a popular herbal remedy which may be e?ective, but which is handicapped by di?erences of strength between di?erent preparations or batches. It can interact with a number of conventional drugs and so needs to be used cautiously and with advice.
In general, antidepressants work by restoring the balance of chemicals in the brain. Improved sleep and reduced anxiety are usually the ?rst signs of improvement, particularly among people taking the more sedative tricyclic drugs. Improvement in other symptoms follow, with the mood starting to lift after about two weeks of treatment. Most people feel well by three months, although a few residual symptoms, such as slowness in the mornings, may take longer to clear up. People taking antidepressants usually want to stop them as soon as they feel better; however, the risk of relapse is high for up to a year and most doctors recommend continuing the drugs for around 4–6 months after recovery, with gradual reduction of the dose after that.
Withdrawal reactions may occur including nausea, vomiting, headache, giddiness, panic or anxiety and restlessness. The drugs should be withdrawn gradually over about a month or longer (up to six months in those who have been on maintenance treatment).

About the Nutrients in This Food
Grapefruit and ugli fruit (a cross between the grapefruit and the tangerine) have moderate amounts of dietary fiber and, like all citrus fruits, are most prized for their vitamin C. Pink or red grapefruits have moderate amounts of vitamin A.
One-half medium (four-inch diameter) pink grapefruit has 1.4 g dietary fiber, 1,187 IU vitamin A (51 percent of the R DA for a woman, 40 percent of the R DA for a man), and 44 mg vitamin C (59 percent of the R DA for a woman, 49 percent of the R DA for a man). One half medium (3.75-inch diameter) white grapefruit has 1.3 g dietary fiber, 39 IU vitamin A (2 percent of the R DA for a woman, 1 percent of the R DA for a man), and 39 mg vitamin C (52 percent of the R DA for a woman, 43 percent of the R DA for a man).
Pink and red grapefruits also contain lycopene, a red carotenoid (plant pigment), a strong antioxidant that appears to lower the risk of cancer of the prostate. The richest source of lycopene is cooked tom atoes.

Buying This Food
Look for: Firm fruit that is heavy for its size, which means that it will be juicy. The skin should be thin, smooth, and fine-grained. Most grapefruit have yellow skin that, depending on the variety, may be tinged with red or green. In fact, a slight greenish tint may mean that the grapefruit is high in sugar. Ugli fruit, which looks like misshapen, splotched grapefruit, is yellow with green patches and bumpy skin.
Avoid: Grapefruit or ugli fruit with puff y skin or those that feel light for their size; the flesh inside is probably dry and juiceless.
Storing This Food
Store grapefruit either at room temperature (for a few days) or in the refrigerator.
Refrigerate grapefruit juice in a tightly closed glass bottle with very little air space at the top. As you use up the juice, transfer it to a smaller bottle, again with very little air space at the top. The aim is to prevent the juice from coming into contact with oxygen, which destroys vitamin C. (Most plastic juice bottles are oxygen-permeable.) Properly stored and protected from oxygen, fresh grapefruit juice can hold its vitamin C for several weeks.
Preparing This Food
Grapefruit are most flavorful at room temperature, which liberates the aromatic molecules that give them their characteristic scent and taste.
Before cutting into the grapefruit, rinse it under cool running water to flush debris off the peel.
To section grapefruit, cut a slice from the top, then cut off the peel in strips—starting at the top and going down—or peel it in a spiral fashion. You can remove the bitter white membrane, but some of the vitamin C will go with it. Finally, slice the sections apart. Or you can simply cut the grapefruit in half and scoop out the sections with a curved, serrated grapefruit knife.
What Happens When You Cook This Food
Broiling a half grapefruit or poaching grapefruit sections reduces the fruit’s supply of vitamin C, which is heat-sensitive.
How Other Kinds of Processing Affect This Food
Commercially prepared juices. How well a commercially prepared juice retains its vitamin C depends on how it is prepared, stored, and packaged. Commercial flash-freezing preserves as much as 95 percent of the vitamin C in fresh grapefruit juices. Canned juice stored in the refrigerator may lose only 2 percent of its vitamin C in three months. Prepared, pasteurized “fresh” juices lose vitamin C because they are sold in plastic bottles or waxed-paper cartons that let oxygen in.
Commercially prepared juices are pasteurized to stop the natural enzyme action that would otherwise turn sugars to alcohols. Pasteurization also protects juices from potentially harmful bacterial and mold contamination. Following several deaths attributed to unpas- teurized apple juices containing E. coli O157:H7, the FDA ruled that all fruit and vegetable juices must carry a warning label telling you whether the juice has been pasteurized. Around the year 2000, all juices must be processed to remove or inactivate harmful bacteria.
Medical Uses and/or Benefits
Antiscorbutic. All citrus fruits are superb sources of vitamin C, the vitamin that prevents or cures scurvy, the vitamin C-deficiency disease.
Increased absorption of supplemental or dietary iron. If you eat foods rich in vitamin C along with iron supplements or foods rich in iron, the vitamin C will enhance your body’s ability to absorb the iron.
Wound healing. Your body needs vitamin C in order to convert the amino acid proline into hydroxyproline, an essential ingredient in collagen, the protein needed to form skin, ten- dons, and bones. As a result people with scurvy do not heal quickly, a condition that can be remedied with vitamin C, which cures the scurvy and speeds healing. Whether taking extra vitamin C speeds healing in healthy people remains to be proved.
Possible inhibition of virus that causes chronic hepatitis C infection. In Januar y 2008, research- ers at Massachusetts General Hospital Center for Engineering in Medicine (Boston) published a report in the medical journal Hepatology detailing the effect of naringenin, a compound in grapefruit, on the behavior of hepatitis viruses in liver cells. In laborator y studies, naringenin appeared to inhibit the ability of the virus to multiply and/or pass out from the liver cells. To date, there are no studies detailing the effect of naringenin in human beings with hepatitis C.
Adverse Effects Associated with This Food
Contact dermatitis. The essential oils in the peel of citrus fruits may cause skin irritation in sensitive people.
Food/Drug Interactions
Aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen, naproxen and others. Taking aspirin or NSAIDs with acidic foods such as grapefruit may intensif y the drug’s ability to irritate your stomach and cause gastric bleeding.
Antihistamines, anticoagulants, benzodiazepines (tranquilizers or sleep medications), calcium channel blockers (blood pressure medication), cyclosporine (immunosuppressant drug used in organ transplants), theophylline (asthma drug). Drinking grapefruit juice with a wide variety of drugs ranging from antihistamines to blood pressure medication appears to reduce the amount of the drug your body metabolizes and eliminates. The “grapefruit effect” was first identified among people taking the antihypertensive drugs felodipine (Plendil) and nifedip- ine (Adalat, Procardia). It is not yet known for certain exactly what the active substance in the juice is. One possibility, however, is bergamottin, a naturally occurring chemical in grapefruit juice known to inactivate cytochrome P450 3A4, a digestive enzyme needed to convert many drugs to water-soluble substances you can flush out of your body. Without an effective supply of cytochrome P450 3A4, the amount of a drug circulating in your body may rise to dangerous levels. Reported side effects include lower blood pressure, increased heart rate, headache, flushing, and lightheadedness.

P. 84-85o C) and 23- hydroxy tricontan-2-one (M. P. 109-110 o C) were isolated from the rhizome by Misra et al (1984). They also isolated 21-hydroxy tetracontan-20-one and 4-methyl heptade canoic acid from the root stock. Misra et al (1990) isolated a new natural triterpene alcohol-Curculigol charactrised as 24-methy cycloart-7-en-3-beta-20-diol. A novel pentacyclic triterpene has been isolated from the rhizomes of C.orchioides and characterised as 31- methyl-3-oxo-20-ursen-28-oic acid (Metha and Gawarikar,1991). Xu and Xu (1992) and Xu et al (1992 a, b) have isolated 13 cycloartane type. Triterpene glycosides from C. orchioides rhizome and characterised them as curculigo saponin A-M.The root stock are mucilaginous, sweet, cooling, bitter, emollient, diuretic, aphrodisiac, depurative, alternative, appetiser, carminative, viriligenic, antipyretic and tonic (Sivarajan and Indira, 1994; Warrier et al, 1994).The uterine stimulant activity of the flavone glycoside extracted from C. orchioides has been studied by Dhawan and Saxena (1958), Sharma et al (1975) and Dhar et al (1979).The plant extract of C. orchioides showed hypoglycaemic, spasmolytic and anticancer properties (Dhar et al,1968). Phagocytic activity (Kubo et al, 1983) and immunoadjuvant activity (Oru et al, 1982) of phenolic glycosides, curculigoside isolated from the rhizome of the plant have been reported. Porwal and Mehta (1985) discussed the medicinal importance of the plant and its use in indigenous system of medicine as a tonic. Sharma et al (1991) reported the influence of MAK an ayurvedic food supplement constituting C. orchioides against Dimethyl benz anthracene induced mammary tumours in rats. Samanta (1992) reported the modulation of male infertility by Ayurvedic drug, which constitutes C. orchioides. Immunostimulant activity of C. orchioides has been demonstrated by Saxena (1992). Immunological activites of curculigo saponin G were assayed in mice and the results showed that it promoted proliferation of spleen lymphocyctes very significantly and increased the weight of the thymus in vivo in mice (Xu et al,1992).Pharmacological studies in China, on the alcoholic extract obtained from the plant showed several active effects such as adaptogenic, anti-inflammatory, anticonvulsant, sedative, androgenic and immunopromoting activities (Xu et al, 1992).Curculigo orchioides is distributed widely throughout the country. The demand of the raw materials and derivatives of the plant for the indigenous drug industries are satisfied mainly from the wild source, depleting the natural population and thus the species have become extinct or endangered. Ansari (1993) have reported C. orchioides as a threatened plant from Madhulia forest of Garakhpur. Augustin and Souza (1995) also considered the plant as an endangered species. About Muira Puama Tea
The main ingredient of the muira puama tea is, of course, the muira puama herbal plant. It is a flowering plant with two species (Benth and Anselmino). Its origin can be found in the Amazonian rainforests, although at present it is grown in Europe, as well. The trees grow up to 4 meters, sometimes even taller. They have short-petioled leaves which are light green on upper surface and dark brown on lower surface. It has small, white flowers that have a similar scent to those of jasmine.

A new alkaloid 15-hydroxy strychnine has been isolated from the seeds and the structure of the alkaloid established by spectroscopic data (Galeffi et al, 1979). Cai et al (1990 b) studied the changes in the alkaloid composition of the seeds during drug processing. Saily et al (1994) determined the mineral elements in Strychnos nux-vomica. Corsaro et al (1995) reported polysaccharides from the seeds of Strychnos species.Seeger and Neumann (1986) reviewed the physico-chemical characteristics, occurrence, identification, utilisation, poisoning, toxicity, kinetics, differential diagnosis and therapeutic uses of strychnine and brucine. Aspergillus niger, A. flavus and Pencillium citrinum showed regular association with Strychnos seeds and effectively deteriorated the alkaloid content of the seeds (Dutta, 1988; Dutta and Roy, 1992). Nicholson (1993) described the history, structure and synthesis of strychnine which occur in the seeds of S. nux-vomica. Rawal and Michoud (1991) developed a general solution for the synthesis of 2- azabicyclo (3.3.1) nonane substructure of Strychnos alkaloids.Villar et al (1984) and Hayakawa et al (1984) developed HPLC method for the analysis of strychnine and brucine. Shukla et al (1985) evaluated the efficacy of Rasnadigugglu compound consisting of S. nux-vomica, on rheumatoid arthritis and found to be effective in reducing inflammatory oedoma and rheumatoid arthritis. It also exhibited analgesic activity. A compound Unani formulation containing S. nux-vomica significantly attenuated withdrawal intensity in morphine dependent rats (Zatar et al, 1991). Shahana et al (1994) studied the effect of Unani drug combination (UDC) having Strychnos nux-vomica on the abstinence syndrome in moderately and severely morphine dependent rats. The UDC strikingly suppressed the abstinence syndrome was seen to possess central depressant and analgesic action.Melone et al (1992) reported brucine-lethality in mice. Preparations: Average dose, 1-2 grams or equivalent. Thrice daily. An older generation of herbalists prepared their tinctures and extracts from the green flowering unripe wild Oats as the effective constituent is unstable. Taken hot, effects are more immediate.
Tea: oatstraw: 1-2 teaspoons to each cup boiling water; infuse 15 minutes. Drink freely.
Tincture BHP (1983) 1 part to 5 parts 45 per cent alcohol. Macerate and shake daily for 8 days; dose, 1- 5ml.
Extracts, groats and oatmeal products are all beneficial but are not of the same efficacy as the fresh green plant.
Oatstraw bath: 2-3 handfuls oatstraw. Simmer in 2-3 litres water for 5 minutes; strain; add to bathwater. Liquid Extract: dose: 1-2ml in water.

Persistent, painful erection of the penis without sexual arousal. Priapism occurs when blood does not drain from the spongy tissue of the penis, thus keeping the penis erect. This may be caused by clotting in the blood vessels due to a blood disorder or as a result of treatment for impotence. Urgent treatment is needed in order to avoid permanent damage. A rare degenerative disorder of unknown cause that progressively damages the autonomic nervous system. It begins gradually at age 60–70 and is more common in men. Myristica fragrans
FAMILY: Myristicaceae
SYNONYMS: M. officinalis, M. aromata, Nux moschata, myristica (oil), mace (husk), macis (oil).
GENERAL DESCRIPTION: An evergreen tree up to 20 metres high with a greyish-brown smooth bark, dense foliage and small dull-yellow flowers. ‘Mace’ is the name given to the bright red netlike aril or husk surrounding the nutmeg shell and seed, which is contained within the fleshy fruit.
DISTRIBUTION: Native to the Moluccas and nearby islands; cultivated in Indonesia, Sri Lanka and the West Indies, especially Grenada.
EXTRACTION: Essential oil by steam (or water) distillation from 1. the dried worm eaten nutmeg seed (the worms eat away all the starch and fat content); 2. the dried orangebrown aril or husk – mace; and 3. an oleoresin is also produced in small quantities by solvent extraction from mace. The oil is also distilled in the USA and Europe from the imported nutmegs.
OTHER SPECIES: Indonesia and Sri Lanka produce the so-called ‘East Indian’ nutmeg which is considered superior, while Grenada produces the ‘West Indian’ nutmeg – see also Botanical Classification.
HERBAL/FOLK TRADITION: Nutmeg and mace are widely used as domestic spices in the East and West. They have been used for centuries as a remedy mainly for digestive and kidney problems. In Malaysia they are used during pregnancy to strengthen and tone the uterine muscles. Grated nutmeg with lard is used for piles. A fixed oil of nutmeg is also used in soap and candle making.
Nutmeg is current in the British Herbal Pharmacopoeia indicated for flatulent dyspepsia, nausea, diarrhoea, dysentery, and topically for rheumatism.
ACTIONS: Analgesic, anti-emetic, antioxidant, antirheumatic, antiseptic, antispasmodic, aphrodisiac, carminative, digestive, emmenagogue, gastric secretory stimulant, larvicidal, orexigenic, prostaglandin inhibitor, stimulant, tonic.
CHARACTERISTICS: 1. A water-white or pale yellow mobile liquid with a sweet, warm-spicy odour and a terpeney top-note. 2. A water white or pale yellow mobile liquid with a sweet, warm-spicy scent. 3. An orange-brown viscous liquid with a fresh, spicy-warm, balsamic fragrance. It has good masking power.
They blend well with oakmoss, lavandin, bay leaf, Peru balsam, orange, geranium, clary sage, rosemary, lime, petitgrain, mandarin, coriander and other spice oils.
PRINCIPAL CONSTITUENTS: Mainly monoterpene hydrocarbons (88 per cent approx.): camphene, pinene, dipentene, sabinene, cymene, with lesser amounts of geraniol, borneol, linalol, terpineol, myristicin (4–8 per cent), safrol and elemincin, among others. Mace oil contains similar constituents but contains more myristicin.
SAFETY DATA: Both nutmeg and mace are generally non-toxic, non-irritant and non sensitizing. However, used in large doses they show signs of toxicity such as nausea, stupor and tachycardia, believed to be due to the myristicin content. ‘Large quantities are hallucinogenic and excitant to the motor cortex.’. On this basis nutmeg (especially the West Indian type) is probably safer to use than mace. Use in moderation, and with care in pregnancy.

Solidago canadensis
FAMILY: Asteraceae
SYNONYMS: S. canadensis var. gilvocanescens, S. altissima, S. canadensis var. salebrosa, Canadian goldenrod, meadow goldenrod, common goldenrod, giant goldenrod, tall goldenrod, rough goldenrod.
GENERAL DESCRIPTION: Solidago canadensis is a herbaceous, perennial plant of the daisy family often found growing as a wildflower, commonly forming colonies. The stem is stiff and erect, growing up to 7 feet high with numerous branches and lance-shaped leaves. The attractive small yellow or golden flowers are held above the foliage in clusters, which bloom late in the season. Goldenrod is often unfairly blamed for causing hay fever in humans although the pollen causing these allergy problems is mainly produced by ragweed, another wild plant which blooms at the same time. Goldenrods have a reputation of being difficult to control due to their aggressive rhizomatous growth, which enables them to rapidly colonize large areas.
DISTRIBUTION: Native to North America, the herb occurs in most US states and Canadian provinces. It is found in a variety of habitats, and it typically is one of the first plants to colonize an area after disturbance (such as fire) but rarely persists once shrubs and trees become established and is found neither in very dry locations nor in waterlogged ones. In many parts of Europe, Japan and China, it is considered an invasive weed, often endangering local fauna.
OTHER SPECIES: The genus Solidago comprises over 140 varieties, many of which are very similar; it can even crossbreed with other plants! Nearly all are known simply as goldenrod, though some have specific folk names such as woundwort, Aaron’s rod and Blue Mountain tea. Most of the goldenrod species are native to North America although a few are native to Europe, Asia, North Africa and South America. The European goldenrod (S. virgaurea), was known as woundwort in England as it was used as an astringent remedy for treating wounds and bleeding. The sweet goldenrod (S. odora), a native of the US and Canada, is the species most frequently seen in herb gardens and is also used to produce an essential oil used by the perfumery industry. Sweet goldenrod also has a long history of medicinal use by Native Americans and was exported for tea in the nineteenth century.
HERBAL/FOLK TRADITION: The genus name Solidago derives from the Latin soldare, which means ‘to make whole’. Goldenrod has a long history of medicinal use especially by the Native Americans who used the plant for a wide range of conditions: infusions and compounds of the flowers and roots were used as emetics, sedatives and to counteract love potions, treat diarrhoea, fevers and the ’flu. The Shuswap used the plant in baths for women giving birth and the Zunis chewed crushed flowers to treat sore throats and drank infusions of the flowers for body pain. People of the Gosiute tribe ate the seeds and the Navajo people ate the roots. The Navajo also smoked the root with other plants and used the plant as a charm for success in gambling. The ground flowers were used on bee stings and a tea made to treat snakebite. During the Boston Tea Party, colonists drank goldenrod tea instead of English tea, which gave it the nick-name ‘Liberty Tea’. The tea is aromatic with a pleasing anise-like flavour as well as being a mildly astringent, carminative, diaphoretic, diuretic, febrifuge and stimulant.
Traditionally the herb has also been used for respiratory conditions and urinary tract infections. In the Swiss Pharmacopoeia, Goldenrod essential oil is indicated for supporting the circulatory system including cardiovascular problems like tachycardia as well as urinary tract and liver function. Goldenrod has also been approved by the German Commission as a natural anti-inflammatory, diuretic and antispasmodic in the treatment of urinary tract conditions. It is also reputed to be a lymphatic system conditioner, a regulator of the autonomic nervous system, with relaxing and calming effects valuable for hypertension.
According to the University of Montreal in Canada, goldenrod shows great potential in dealing with impotence. Although a little-known aromatherapy essential oil, it may have value due to its anti-allergy properties, for conditions like hay fever, allergic reactions, coughs and respiratory infections.
ACTIONS: Analgesic, anti-allergenic, antibacterial, antifungal, anti-infectious, anti-inflammatory, anti-oxidant, antihypertensive, antispasmodic, antiviral, astringent, diuretic, expectorant, febrifuge, immuno-stimulant, liver support/stimulant, mucolytic, sedative.
EXTRACTION: The essential oil is steam distilled from the goldenrod flower.
CHARACTERISTICS: A yellowish liquid with a strong fresh, balsamic-peppery and slightly bitter aroma with a woody undertone. It blends well with ravintsara, peru balsam, rosemary, pine, ginger, spruce and spice oils.
PRINCIPAL CONSTITUENTS: Mainly pinene, bornyl acetate, benzoates, myrcene, methyl chavicol, borneol and limonene. NB: Essential oil from a sample of S. odora contained up to 75 per cent methyl chavicol (estragole), which is known to be moderately toxic
SAFETY DATA: Generally considered non-toxic, non-irritating and non-sensitizing: possible skin sensitivity. Avoid during pregnancy and for children due to high levels of estragole in some chemotypes.

Rosa centifolia
FAMILY: Rosaceae
SYNONYMS: Rose maroc, French rose, Provence rose, hundred-leaved rose, Moroccan otto of rose (oil), French otto of rose (oil), rose de mai (absolute or concrete).
GENERAL DESCRIPTION: The rose which is generally used for oil production is strictly a hybrid between R. centifolia, a pink rose, and R. gallica, a dark red rose. This variety, known as rose de mai, grows to a height of 2.5 metres and produces an abundance of flowers with large pink or rosy-purple petals. There are two subspecies – one is more spiny than the other.
DISTRIBUTION: The birthplace of the cultivated rose is believed to be ancient Persia; now cultivated mainly in Morocco, Tunisia, Italy, France, Yugoslavia and China. The concrete, absolute and oil are mainly produced in Morocco; the absolute in France, Italy and China.
OTHER SPECIES: There are over 10,000 types of cultivated rose! There are several subspecies of R. centifolia, depending on the country of origin. Other therapeutic species are the red rose or apothecary rose (R. gallica) of traditional Western medicine, the oriental or tea rose (R. indica), the Chinese or Japanese rose (R. rugosa) and the Turkish or Bulgarian rose (R. damascena) which is also extensively cultivated for its oil. Recently rosehip seed oil from R. rubiginosa has been found to be a very effective skin treatment; it promotes tissue regeneration and is good for scars, burns and wrinkles. The wild rose (Eubatus rubus) is now also being used to produce a rare absolute with a rich, spicy-earthy fragrance. See also entry on damask rose and the Botanical Classification section.
HERBAL/FOLK TRADITION: The healing virtues of the rose have been known since antiquity and although roses are rarely used in herbal practice nowadays, up to the Middle Ages they played an essential part in the materia medica, and still
fulfil an important role in Eastern medicine. They were used for a wide range of disorders, including digestive and menstrual problems, headaches and nervous tension, liver congestion, poor circulation, fever (plague), eye infections and skin complaints. ‘The symbolism connected with the rose is perhaps one of the richest and most complex associated with any plant ... traditionally associated with Venus, the Goddess of love and beauty, and in our materialistic age the Goddess is certainly alive and well in the cosmetics industry for rose oil (mainly synthetic) is found as a component in 46% of men’s perfumes and 98% of women’s fragrances.’.
The French or Moroccan rose possesses narcotic properties and has the reputation for being aphrodisiac (more so than the Bulgarian type), possibly due to the high percentage of phenyl ethanol in the former. For further distinctions between the different properties of rose types, see damask rose.

EXTRACTION: 1. Essential oil or otto by water or steam distillation from the fresh petals. (Rose water is produced as a byproduct of this process.) 2. Concrete and absolute by solvent extraction from the fresh petals. (A rose leaf absolute is also produced in small quantities in France.)
CHARACTERISTICS: 1. The oil is a pale yellow liquid with a deep, sweet, rosy-floral, tenacious odour. 2. The absolute is a reddish orange viscous liquid with a deep, rich, sweet, rosy-spicy, honeylike fragrance. It blends well with jasmine, cassie, mimosa, neroli, geranium, bergamot, lavender, clary sage, sandalwood, guaiacwood, patchouli, benzoin, chamomile, Peru balsam, clove and palmarosa.
PRINCIPAL CONSTITUENTS: It has over 300 constituents, some in minute traces. Mainly citronellol (18–22 per cent), phenyl ethanol (63 per cent), geraniol and nerol (10–15 per cent), stearopten (8 per cent), farnesol (0.2–2 per cent), among others.
SAFETY DATA: Non-toxic, non-irritant, non-sensitizing.

Skin care: Broken capillaries, conjunctivitis (rose water), dry skin, eczema, herpes, mature and sensitive complexions, wrinkles.

Salvia sclarea
FAMILY: Lamiaceae (Labiatae)
SYNONYMS: Clary, clary wort, muscatel sage, clear eye, see bright, common clary, clarry, eye bright.
GENERAL DESCRIPTION: Stout biennial or perennial herb up to 1 metre high with large, hairy leaves, green with a hint of purple, and small blue flowers.
DISTRIBUTION: Native to southern Europe; cultivated worldwide especially in the Mediterranean region, Russia, the USA, England, Morocco and central Europe. The French, Moroccan and English clary are considered of superior quality for perfumery work.
OTHER SPECIES: Closely related to the garden sage (S. officinalis) and the Spanish sage (S. lavendulaefolia), which are both used to produce essential oils. Other types of sage include meadow clary (S. pratensis) and vervain sage (S. verbenaca). Clary sage should not be confused with the common wayside herb eyebright (Euphrasia).
HERBAL/FOLK TRADITION: This herb, highly esteemed in the Middle Ages, has now largely fallen out of use. It was used for digestive disorders, kidney disease, uterine and menstrual complaints, for cleansing ulcers and as a general nerve tonic. The mucilage from the seeds was used for treating tumours and for removing dust particles from the eyes.
Like garden sage, it cools inflammation and is especially useful for throat and respiratory infections.

CHARACTERISTICS: A colourless or pale yellowy-green liquid with a sweet, nutty herbaceous scent. It blends well with juniper, lavender, coriander, cardamon, geranium, sandalwood, cedarwood, pine, labdanum, jasmine, frankincense, bergamot and other citrus oils.
PRINCIPAL CONSTITUENTS: Linalyl acetate (up to 75 per cent), linalol, pinene, myrcene and phellandrene, among others. Constituents vary according to geographical origin – there are several different chemotypes.
SAFETY DATA: Non-toxic, non-irritant, nonsensitizing. Avoid during pregnancy. Do not use clary sage oil while drinking alcohol since it can induce a narcotic effect and exaggerate drunkenness. Clary sage is generally used in preference to the garden sage in aromatherapy due to its lower toxicity level.

Cananga odorata var. genuina
FAMILY: Annonaceae
SYNONYMS: Unona odorantissimum, flower of flowers.
GENERAL DESCRIPTION: A tall tropical tree up to 20 metres high with large, tender, fragrant flowers, which can be pink, mauve or yellow. The yellow flowers are considered best for the extraction of essential oil.
DISTRIBUTION: Native to tropical Asia, especially Indonesia and the Philippines. Major oil producers are Madagascar, Reunion and the Comoro Islands.
OTHER SPECIES: Very closely related to cananga (C. odoratum var. macrophylla), although the oil produced from the ylang ylang is considered of superior quality for perfumery work, having a more refined quality.
HERBAL/FOLK TRADITION: In Indonesia, the flowers are spread on the beds of newly married couples on their wedding night. In the Molucca Islands, an ointment is made from ylang ylang and cucuma flowers in a coconut oil base for cosmetic and hair care, skin diseases, to prevent fever (including malaria) and fight infections.
In the Victorian age, the oil was used in the popular hair treatment Macassar oil, due to its stimulating effect on the scalp, encouraging hair growth. The oil was also used to soothe insect bites, and is thought to have a regulating effect on cardiac and respiratory rhythm.
ACTIONS: Aphrodisiac, antidepressant, anti infectious, antiseborrhoeic, antiseptic, euphoric, hypotensive, nervine, regulator, sedative (nervous), stimulant (circulatory), tonic.
EXTRACTION: Essential oil by water or steam distillation from the freshly picked flowers. The first distillate (about 40 per cent) is called ylang ylang extra, which is the top grade. There are then three further successive distillates, called Grades 1, 2 and 3. A ‘complete’ oil is also produced which represents the total or ‘unfractionated’ oil, but this is sometimes constructed by blending ylang ylang 1 and 2 together, which are the two least popular grades. (An absolute and concrete are also produced by solvent extraction for their long-lasting floral-balsamic effect.)
CHARACTERISTICS: Ylang ylang extra is a pale yellow, oily liquid with an intensely sweet, soft, floral-balsamic, slightly spicy scent – a good oil has a creamy rich topnote. A very intriguing perfume oil in its own right, it also blends well with rosewood, jasmine, vetiver, opopanax, bergamot, mimosa, cassie, Peru balsam, rose, tuberose, costus and others. It is an excellent fixative. The other grades lack the depth and richness of the ylang ylang extra.
PRINCIPAL CONSTITUENTS: Methyl benzoate, methyl salicylate, methyl paracretol, benzyl acetate, eugenol, geraniol, linalol and terpenes: pinene, cadinene, among others.
SAFETY DATA: Non-toxic, non-irritant, a few cases of sensitization reported. Use in moderation, since its heady scent can cause headaches or nausea.

Skin care: Acne, hair growth, hair rinse, insect bites, irritated and oily skin, general skin care.
Circulation muscles and joints: High blood pressure, hyperpnoea (abnormally fast breathing), tachycardia, palpitations.
Nervous system: Depression, frigidity, impotence, insomnia, nervous tension and stress-related disorders – ‘The writer, working with odorous materials for more than twenty years, long ago noticed that ... ylang ylang soothes and inhibits anger born of frustration.’.
OTHER USES: Extensively used as a fragrance component and fixative in soaps, cosmetics and perfumes, especially oriental and floral types; ylang ylang extra tends to be used in high-class perfumes, ylang ylang 3 in soaps, detergents, etc. Used as a flavour ingredient, mainly in alcoholic and soft drinks, fruit flavours and desserts.... ylang ylang
