Pediculus humanus var. capitis (head louse) affects the scalp in children or adults, particularly in females. The adult louse may visit many heads in one day, especially in schoolgirls. It lays its eggs on the scalp hair and the resulting ‘nit’ grows out with the hair. Secondary infection owing to scratching is common in severe infestations, causing enlarged lymph glands in the posterior neck and some general debility. The lice and nits can be killed by applications of MALATHION 0.5 per cent lotion or PERMETHRIN 1 per cent lotion. After the hair is washed, application of a conditioner allows nits to be removed with a ?ne nit comb.
Pediculus pubis (crab louse) is broader and shorter than the head louse and less mobile. Usually transmitted sexually, it is found in the pubic area but may infect eyelashes and other body hair. It is easily seen, as are the large nits attached to the public hair: permethrin and malathion lotions are e?ective.... pediculosis
Treatment Twice-daily washing with a salicylic-acid cleanser can help remove the pore-blocking debris, as can daily shampooing. Use only oil-free cosmetics and hide blackheads with a ?esh-tinted acne lotion containing benzoyl peroxide, acid or sulphur. Never squeeze blackheads, however tempting; ask a skin specialist how to do this properly. Other treatments include microdermabrasion, and the antibiotic lotions erythromycin and clindamycin may be e?ective. Tretinoin and adapilene can be used on the skin but are not permitted in pregnancy and may cause problems such as hypersensitivity to sunlight, so medical advice is essential. In resistant cases, long-term suppressive oral therapy with one of the TETRACYCLINES or with ERYTHROMYCIN may be necessary. In females a combined oestrogenantiandrogen ‘pill’ is an alternative. Severe resistant acne can be cleared by a 16- to 24week course of oral isotretinoin, but this drug is teratogenic (see TERATOGENESIS) and can cause many side-effects including depression, so its use requires specialist supervision.

Examination of the ear includes inspection of the external ear. An auriscope is used to examine the external ear canal and the ear drum. If a more detailed inspection is required, a microscope may be used to improve illumination and magni?cation.
Tuning-fork or Rinne tests are performed to identify the presence of DEAFNESS. The examiner tests whether the vibrating fork is audible at the meatus, and then the foot of the fork is placed on the mastoid bone of the ear to discover at which of the two sites the patient can hear the vibrations for the longest time. This can help to di?erentiate between conductive and nerve deafness.
Hearing tests are carried out to determine the level of hearing. An audiometer is used to deliver a series of short tones of varying frequency to the ear, either through a pair of headphones or via a sound transducer applied directly to the skull. The intensity of the sound is gradually reduced until it is no longer heard and this represents the threshold of hearing, at that frequency, through air and bone respectively. It may be necessary to play a masking noise into the opposite ear to prevent that ear from hearing the tones, enabling each ear to be tested independently.
General symptoms The following are some of the chief symptoms of ear disease: DEAFNESS (see DEAFNESS). EARACHE is most commonly due to acute in?ammation of the middle ear. Perceived pain in this region may be referred from other areas, such as the earache commonly experienced after tonsillectomy (removal of the TONSILS) or that caused by carious teeth (see TEETH, DISORDERS OF). The treatment will depend on the underlying cause. TINNITUS or ringing in the ear often accompanies deafness, but is sometimes the only symptom of ear disease. Even normal people sometimes experience tinnitus, particularly if put in soundproofed surroundings. It may be described as hissing, buzzing, the sound of the sea, or of bells. The intensity of the tinnitis usually ?uctuates, sometimes disappearing altogether. It may occur in almost any form of ear disease, but is particularly troublesome in nerve deafness due to ageing and in noise-induced deafness. The symptom seems to originate in the brain’s subcortical regions, high in the central nervous system. It may be a symptom of general diseases such as ANAEMIA, high blood pressure and arterial disease, in which cases it is often synchronous with the pulse, and may also be caused by drugs such as QUININE, salicylates (SALICYLIC ACID and its salts, for example, ASPIRIN) and certain ANTIBIOTICS. Treatment of any underlying ear disorder or systemic disease, including DEPRESSION, may reduce or even cure the tinnitis, but unfortunately in many cases the noises persist. Management involves psychological techniques and initially an explanation of the mechanism and reassurance that tinnitus does not signify brain disease, or an impending STROKE, may help the person. Tinnitus maskers – which look like hearing aids – have long been used with a suitably pitched sound helping to ‘mask’ the condition.
Diseases of the external ear
WAX (cerumen) is produced by specialised glands in the outer part of the ear canal only. Impacted wax within the ear canal can cause deafness, tinnitis and sometimes disturbance of balance. Wax can sometimes be softened with olive oil, 5-per-cent bicarbonate of soda or commercially prepared drops, and it will gradually liquefy and ‘remove itself’. If this is ineffective, syringing by a doctor or nurse will usually remove the wax but sometimes it is necessary for a specialist (otologist) to remove it manually with instruments. Syringing should not be done if perforation of the tympanic membrane (eardrum) is suspected. FOREIGN BODIES such as peas, beads or buttons may be found in the external ear canal, especially in children who have usually introduced them themselves. Live insects may also be trapped in the external canal causing intense irritation and noise, and in such cases spirit drops are ?rst instilled into the ear to kill the insect. Except in foreign bodies of vegetable origin, where swelling and pain may occur, syringing may be used to remove some foreign bodies, but often removal by a specialist using suitable instrumentation and an operating microscope is required. In children, a general anaesthetic may be needed. ACUTE OTITIS EXTERNA may be a di?use in?ammation or a boil (furuncle) occurring in the outer ear canal. The pinna is usually tender on movement (unlike acute otitis media – see below) and a discharge may be present. Initially treatment should be local, using magnesium sulphate paste or glycerine and 10-per-cent ichthaminol. Topical antibiotic drops can be used and sometimes antibiotics by mouth are necessary, especially if infection is acute. Clotrimazole drops are a useful antifungal treatment. Analgesics and locally applied warmth should relieve the pain.
CHRONIC OTITIS EXTERNA producing pain and discharge, can be caused by eczema, seborrhoeic DERMATITIS or PSORIASIS. Hair lotions and cosmetic preparations may trigger local allergic reactions in the external ear, and the chronic disorder may be the result of swimming or use of dirty towels. Careful cleaning of the ear by an ENT (Ear, Nose & Throat) surgeon and topical antibiotic or antifungal agents – along with removal of any precipitating cause – are the usual treatments. TUMOURS of the ear can arise in the skin of the auricle, often as a result of exposure to sunlight, and can be benign or malignant. Within the ear canal itself, the commonest tumours are benign outgrowths from the surrounding bone, said to occur in swimmers as a result of repeated exposure to cold water. Polyps may result from chronic infection of the ear canal and drum, particularly in the presence of a perforation. These polyps are soft and may be large enough to ?ll the ear canal, but may shrink considerably after treatment of the associated infection.
Diseases of the middle ear
OTITIS MEDIA or infection of the middle ear, usually occurs as a result of infection spreading up the Eustachian tubes from the nose, throat or sinuses. It may follow a cold, tonsillitis or sinusitis, and may also be caused by swimming and diving where water and infected secretions are forced up the Eustachian tube into the middle ear. Primarily it is a disease of children, with as many as 1.5 million cases occurring in Britain every year. Pain may be intense and throbbing or sharp in character. The condition is accompanied by deafness, fever and often TINNITUS.
In infants, crying may be the only sign that something is wrong – though this is usually accompanied by some localising manifestation such as rubbing or pulling at the ear. Examination of the ear usually reveals redness, and sometimes bulging, of the ear drum. In the early stages there is no discharge, but in the later stages there may be a discharge from perforation of the ear drum as a result of the pressure created in the middle ear by the accumulated pus. This is usually accompanied by an immediate reduction in pain.
Treatment consists of the immediate administration of an antibiotic, usually one of the penicillins (e.g. amoxicillin). In the majority of cases no further treatment is required, but if this does not quickly bring relief then it may be necessary to perform a myringotomy, or incision of the ear drum, to drain pus from the middle ear. When otitis media is treated immediately with su?cient dosage of the appropriate antibiotic, the chances of any permanent damage to the ear or to hearing are reduced to a negligible degree, as is the risk of any complications such as mastoiditis (discussed later in this section). CHRONIC OTITIS MEDIA WITH EFFUSION or glue ear, is the most common in?ammatory condition of the middle ear in children, to the extent that one in four children in the UK entering school has had an episode of ‘glue ear’. It is characterised by a persistent sticky ?uid in the middle ear (hence the name); this causes a conductive-type deafness. It may be associated with enlarged adenoids (see NOSE, DISORDERS OF) which impair the function of the Eustachian tube. If the hearing impairment is persistent and causes problems, drainage of the ?uid, along with antibiotic treatment, may be needed – possibly in conjunction with removal of the adenoids. The insertion of grommets (ventilation tubes) was for a time standard treatment, but while hearing is often restored, there may be no long-term gain and even a risk of damage to the tympanic membrane, so the operation is less popular than it was a decade or so ago. MASTOIDITIS is a serious complication of in?ammation of the middle ear, the incidence of which has been dramatically reduced by the introduction of antibiotics. In?ammation in this cavity usually arises by direct spread of acute or chronic in?ammation from the middle ear. The signs of this condition include swelling and tenderness of the skin behind the ear, redness and swelling inside the ear, pain in the side of the head, high fever, and a discharge from the ear. The management of this condition in the ?rst instance is with antibiotics, usually given intravenously; however, if the condition fails to improve, surgical treatment is necessary. This involves draining any pus from the middle ear and mastoid, and removing diseased lining and bone from the mastoid.
Diseases of the inner ear
MENIÈRE’S DISEASE is a common idiopathic disorder of ENDOLYMPH control in the semicircular canals (see EAR), characterised by the triad of episodic VERTIGO with deafness and tinnitus. The cause is unknown and usually one ear only is affected at ?rst, but eventually the opposite ear is affected in approximately 50 per cent of cases. The onset of dizziness is often sudden and lasts for up to 24 hours. The hearing loss is temporary in the early stages, but with each attack there may be a progressive nerve deafness. Nausea and vomiting often occur. Treatment during the attacks includes rest and drugs to control sickness. Vasodilator drugs such as betahistine hydrochloride may be helpful. Surgical treatment is sometimes required if crippling attacks of dizziness persist despite these measures. OTOSCLEROSIS A disorder of the middle ear that results in progressive deafness. Often running in families, otosclerosis affects about one person in 200; it customarily occurs early in adult life. An overgrowth of bone ?xes the stapes (the innermost bone of the middle ear) and stops sound vibrations from being transmitted to the inner ear. The result is conductive deafness. The disorder usually affects both ears. Those affected tend to talk quietly and deafness increases over a 10–15 year period. Tinnitus often occurs, and occasionally vertigo.
Abnormal hearing tests point to the diagnosis; the deafness may be partially overcome with a hearing aid but surgery is eventually needed. This involves replacing the stapes bone with a synthetic substitute (stapedectomy). (See also OTIC BAROTRAUMA.)... ear, diseases of

During a bite by the female mosquito, one or more sporozoites – a stage in the life-cycle of the parasite – are injected into the human circulation; these are taken up by the hepatocytes (liver cells). Following division, merozoites (minute particles resulting from the division) are liberated into the bloodstream where they invade red blood cells. These in turn divide, releasing further merozoites. As merozoites are periodically liberated into the bloodstream, they cause the characteristic fevers, rigors, etc.
Malaria occurs in many tropical and subtropical countries; P. falciparum is, however, con?ned very largely to Africa, Asia and South America. Malaria is present in increasingly large areas; in addition, the parasites are developing resistance to various preventative and treatment drugs. The disease constitutes a signi?cant problem for travellers, who must obtain sound advice on chemoprophylaxis before embarking on tropical trips – especially to a rural area where intense transmission can occur. Transmission has also been recorded at airports, and following blood transfusion.
The World Health Organisation (WHO) has listed malaria as one of Europe’s top ten infectious diseases. In 1992, 20,000 cases were reported: this had risen to more than 200,000 by the late 1990s. The resurgence of malaria has been worldwide, in part the result of the development of resistant strains of the disease, and in part because many countries have failed (or been unable) to implement environmental measures to eliminate mosquitoes. Nearly 40 years ago the WHO forecast that by 1980 only four million people would be affected worldwide; now, at the beginning of the 21st century, around 500 million people a year are contracting malaria with about 3,000 people a day dying from the infection – as many as 70 per cent of them children under the age of ?ve, according to WHO ?gures. The apparently steady advance of global warming means that countries with temperate climates may well warm up su?ciently to enable malaria to become established as an ENDEMIC disease. In any case, the great increase in international air travel has exposed many more people to the risk of malaria, and infected individuals may not exhibit symptoms until they are back home. Doctors seeing a recent traveller with unexplained pyrexia and illness should consider the possibility of malarial infection.
Diagnosis is by demonstration of trophozoites – a stage in the parasite’s life-cycle that takes place in red blood cells – in thick/thin blood-?lms of peripheral blood. Serological tests are of value in deciding whether an individual has had a past infection, but are of no value in acute disease.
P. vivax and P. ovale infections cause less severe disease than P. falciparum (see below), although overall there are many clinical similarities; acute complications are unusual, but chronic ANAEMIA is often present. Primaquine is necessary to eliminate the exoerythrocytic cycle in the hepatocyte (liver cell).
P. falciparum Complications of P. falciparum infection include cerebral involvement (see BRAIN – Cerebrum), due to adhesion of immature trophozoites on to the cerebral vascular endothelium; these lead to a high death rate when inadequately treated. Renal involvement (frequently resulting from HAEMOGLOBINURIA), PULMONARY OEDEMA, HYPOTENSION, HYPOGLYCAEMIA, and complications in pregnancy are also important. In complicated disease, HAEMODIALYSIS and exchange TRANSFUSION have been used. No adequate controlled trial using the latter regimen has been carried out, however, and possible bene?ts must be weighed against numerous potential side-effects – for instance, the introduction of a wide range of infections, overload of the circulatory system with infused ?uids, and other complications.
P. malariae usually produces a chronic infection, and chronic renal disease (nephrotic syndrome) is an occasional sequel, especially in tropical Africa.
Gross SPLENOMEGALY (hyper-reactive malarious splenomegaly, or tropical splenomegaly syndrome) can complicate all four human Plasmodium spp. infections. The syndrome responds to long-term malarial chemoprophylaxis. BURKITT’S LYMPHOMA is found in geographical areas where malaria infection is endemic; the EPSTEIN BARR VIRUS is aetiologically involved.
Prophylaxis Malaria specialists in the United Kingdom have produced guidance for residents travelling to endemic areas for short stays. Drug choice takes account of:
risk of exposure to malaria;
extent of drug resistance;
e?cacy of recommended drugs and their side-effects;
criteria relevant to the individual (e.g. age, pregnancy, kidney or liver impairment). Personal protection against being bitten by
mosquitoes is essential. Permethrinimpregnated nets are an e?ective barrier, while skin barrier protection and vaporised insecticides are helpful. Lotions, sprays or roll-on applicators all containing diethyltoluamide (DEET) are safe and work when put on the skin. Their e?ect, however, lasts only for a few hours. Long sleeves and trousers should be worn after dark.
Drug prophylaxis should be started at least a week before travelling into countries where malaria is endemic (two or three weeks in the case of me?oquine). Drug treatment should be continued for at least four weeks after leaving endemic areas. Even if all recommended antimalarial programmes are followed, it is possible that malaria may occur any time up to three months afterwards. Medical advice should be sought if any illness develops. Chloroquine can be used as a prophylactic drug where the risk of resistant falciparum malaria is low; otherwise, me?oquine or proguanil hydrochloride should be used. Travellers to malaria-infested areas should seek expert advice on appropriate prophylactic treatment well before departing.
Treatment Various chemoprophylactic regimes are widely used. Those commmonly prescribed include: chloroquine + paludrine, me?oquine, and Maloprim (trimethoprim + dapsone); Fansidar (trimethoprim + sulphamethoxazole) has been shown to have signi?cant side-effects, especially when used in conjunction with chloroquine, and is now rarely used. No chemotherapeutic regimen is totally e?ective, so other preventive measures are again being used. These include people avoiding mosquito bites, covering exposed areas of the body between dusk and dawn, and using mosquito repellents.
Chemotherapy was for many years dominated by the synthetic agent chloroquine. However, with the widespread emergence of chloroquine-resistance, quinine is again being widely used. It is given intravenously in severe infections; the oral route is used subsequently and in minor cases. Other agents currently in use include me?oquine, halofantrine, doxycycline, and the artemesinin alkaloids (‘qinghaosu’).

Phytophotodermatitis is a streaky, blistering photodermatosis typically seen on the limbs of children playing in grassy meadows in summer. The phototoxic reaction is caused by psoralens in weeds.
Berlocque dermatitis is a pattern of streaky pigmentation usually seen on women’s necks, caused by a reaction to psoralens in perfumes.
Certain rare metabolic diseases may lead to photosensitisation. They include the PORPHYRIAS and PELLAGRA. Other skin diseases such as lupus erythematosus (see under LUPUS) and ROSACEA may be aggravated by light exposure. Sometimes, in the absence of any of these factors, some people spontaneously develop a sensitivity to light causing various patterns of DERMATITIS or URTICARIA. The most common pattern is ‘polymorphic light eruption’ which typically appears within a day or two of arrival at a sunny holiday destination and persists until departure. Continuously exposed areas, such as the hands and face, may be ‘hardened’ and unaffected.
Treatment Appropriate clothing and headgear, sunscreen creams and lotions are the main preventative measures.... photodermatoses
Uses Zinc chloride is a powerful caustic and astringent which, combined with zinc sulphate, is used as an astringent mouthwash. Zinc sulphate is also used in the form of eyedrops in the treatment of certain forms of conjunctivitis (see under EYE, DISORDERS OF).
Zinc oxide, zinc stearate, and zinc carbonate are made up in dusting powders, in ointments, in paste bandages or suspended in water as lotions for the astringent action they exert upon abraded surfaces of the skin. Zinc and castor oil ointment of the British Pharmacopoeia is a well-tried treatment for nappy rash.

Herbal alternatives exist but may be of limited efficacy: menstrual pain (Helonias root), muscular rheumatism (Black Cohosh), tension headaches (Ginkgo), Angina pectoris (Cramp bark), eye-strain (Eyebright), facial neuralgia (German Chamomile), swollen glands (Poke root), chest pain (Elecampane), cough (Iceland Moss), simple abdominal pain (Fenugreek).
Cures for relief of painful limbs run into hundreds. Gentle massage to release endorphins which block pain waves offers an external approach. Arthritic knees are less painful on application of Castor oil packs or one of the many preparations commended under poultices, liniments, lotions, etc.
During trials at Long Island University, USA, 189 cases of rheumatic knees and elbows were rendered painless by pollen poultices. Dissolve one tablespoon bee pollen pellets in warm water; immerse small handtowel; squeeze out excess moisture; bind over affected joint.

When the discharge is yellow, indicating pus, an infection is suspected which may develop into an abscess. Herbal treatment can be effective but if, after a week, the condition has not improved surgical exploration may be necessary to remove the affected duct.

Liniment. 10 drops oil of Camphor to egg-cup Olive oil. Massage for relief of lumbago, fibrositis, neuralgia, chest and muscle pain.
Inhalant: Inhale the fumes for respiratory oppression with difficult breathing, heart failure, collapse, shock from injury, hypothermia, tobacco habit.
Camphor locket. A small square is sometimes hung in a small linen bag round the neck for prevention of infection, colds.
Camphorated oil: 1oz (30g) Flowers of Camphor to 4oz (125g) peanut oil. Dissolve in gentle heat. Camphor lotion. Dissolve teaspoon (4-6g) Camphor flowers in 4oz Cider vinegar.

Camphor Drops. At one time a bottle brandy with a knob of Camphor at the bottom was kept in every pantry to restore vitality and warmth to those suffering from exposure to cold and damp. Atopic eczema. Allergic eczema. May run in families together with hay fever, asthma or inflamed nasal membrane. May appear anywhere but prefers elbows, knees (flexures), ankles or face. Often seen in infants. May return again and again throughout adult life. Scratching exacerbates.
As regards babies, some paediatricians believe breast-feeding to be protective. A stronger case follows investigation into pollutants from the atmosphere or as additives in food. Industrial chemicals find their way into breast milk that may not be easily excreted but stored in fat.
Cow’s milk is particularly suspect because of exposure of the animal to herbicides and pesticides. For this reason, goat’s milk has met with some success in treatment of this condition, as has Soya milk. Now known that food plays an important part in effective treatment. Chief allergy-stimulators: dairy produce, eggs, cow’s milk. Each individual case must identify those foods that are responsible.
Seborrhoeic eczema leads to scaling of the scalp and redness of the ears, eyebrows, side of the nose and possibly armpits and groin.
Stasis eczema (or varicose eczema) may arise from varicose vein problems, usually limited to the lower third of the leg.
Discoid eczema has coin-shaped patches preferring extensor surfaces of arms and legs.
Contact eczema may be caused by washing-up detergents, etc. See: CONTACT DERMATITIS.
While emotional or psychic disturbance may worsen, eczema is seldom a psychosomatic disorder arising from stressful situations. Contact with water may worsen. Hairdressers and those allergic to dyes may require patch tests.
Eczema patients, especially atopic, have a metabolic deficiency of linoleic acid (a dietary fatty acid) to y-linolenic acid, which is found in Evening Primrose oil. Eczema may develop in bottle-fed babies due to absence of GLA (gamma-linolenic acid) in commercial powdered milk. GLA is present in Evening Primrose.
A cross-over trial in 99 patients (adults and children) by Bristol (England) dermatologists found Evening Primrose oil (Efamol capsules) produced an overall 43 per cent improvement in eczema severity: doses – 4 to 6 capsules twice daily (adults); 2 capsules twice daily (children). Lower doses were not effective.
Alternatives. Formula: equal parts, Burdock root, Yellow Dock root, Valerian root. Dose. Liquid Extracts, 1 teaspoon. Tinctures, 1-2 teaspoons. Powders, two 00 capsules or one-third teaspoon. Thrice daily, before meals. Practitioner: specific medication.
Dry eczema. Equal parts, tinctures: Yarrow, Dandelion, Calendula, Echinacea.
Weeping eczema. Combine tinctures: Barberry 1; Clivers 2; Echinacea 2.
Seborrhoeic eczema. Combine tinctures: Blue Flag root 1; Meadowsweet 2; Boneset 1.
Discoid eczema. Combine tinctures: Yellow Dock 2; Mountain Grape 1; Echinacea 1.
Varicose eczema. Combine tinctures: Echinacea 2; Calendula (Marigold) 1; Hawthorn 1.
Dosage for the above: One to two 5ml teaspoons in water thrice daily before meals.
Skin Care. May reduce necessity for steroid creams. It is best to avoid: lanolin and Coconut oil compounds that may contain coal tar. Wash in soft water (rain water) or water not containing chemical softeners.

Chinese herbs. A study has shown herbal treatment to be far superior to placebo in clinical trials. British children with (dry) atopic eczema responded favourably to treatment which included the following herbs known as Formula PSE101.

External: Mexican men still apply the oil to their eye-brows and hair for growth while their women use it to dress their braids and tresses. To the scalp the oil removes excess sebum, moistens dry skin and expels dandruff. Used for minor skin disorders, acne, sunburn, minor burns, chapped skin, nappy-rash, soft fingernails and facial blemishes.
Preparations: Nuts – eaten freely by Mexicans, their children and farm animals. Roasted to make coffee. Meal left over after oil extraction contains no less than 17 amino acids. With its 35 per cent protein Jojoba is a valuable nutrient. Oil is obtained from Mexican beans by cold pressing to ensure that properties provided by nature are not destroyed by chemical processing. It has now replaced some animal oils, especially whale oil (spermaceti) as a base for ointments, creams, bath oils, suntan lotions, and other cosmetic preparations. Uses, internal. While positive properties for the healing of wounds are present in a decoction of the leaves, it is for its oil that the tree is universally known. Taken for constipation and lead colic. Pin worms in children: 1 teaspoon daily for one month. Orally, the oil forms a barrier on the surface of the stomach thus arresting secretion of gastric juice. For this purpose it has been used with success for gastric and duodenal ulcer. Cases are on record of daily drinking a dessertspoonful of the oil to prevent heart disease and arteriosclerosis, and to alleviate muscular pain.
Olive oil is beneficial for increasing high-density lipoprotein (HDL) and to decrease low density lipoprotein (LDL) which can have a detrimental effect upon the blood when in excess.
Uses, external. In some Levantine counries it is still the belief that rubbing the body with the oil prevents rheumatism, gout and kindred conditions. It is a common ingredient in liniments and lotions for aches and pains of the muscles.
The oil should be expressed by the ‘cold press’ method to preserve its active constituents. Cases are on record where the swallowing of a single black Olive stone (pit) has relieved serious low back pain within hours.

Many skin conditions, including chickenpox, urticaria (nettle rash), and eczema, produce an itchy rash. Generalized skin itchiness can be a result of diabetes mellitus, kidney failure, jaundice, and thyroid disorders.
Pruritus ani (itching around the anal region) occurs with haemorrhoids and anal fissure. Threadworm infestation is the most likely cause of anal itching in children. Pruritus vulvae (itching of the external genitalia in women) may be due to candidiasis, hormonal changes, or to use of spermicides or vaginal ointments and deodorants. Insect bites, lice, and scabies infestations cause intense itching.
Specific treatment for itching depends on the underlying cause. Cooling lotions, such as calamine, relieve irritation; emollients reduce dryness.... itching
The nails are susceptible to damage through injury, or by bacterial or fungal infections, especially tinea and candidiasis. Sometimes they become abnormally thick and curved: a condition known as onychogryphosis. Nail abnormalities may be a sign of skin disease, such as alopecia areata, psoriasis, and lichen planus, or of more generalized disease, for example iron-deficiency anaemia. Unusual nail colour may indicate disease.

FAMILY: Myrtaceae
SYNONYMS: Myrcia acris, Pimenta acris, myrcia, bay, bay rum tree, wild cinnamon, bayberry, bay leaf (oil).
GENERAL DESCRIPTION: A wild-growing tropical evergreen tree up to 8 metres high, with large leathery leaves and aromatic fruits.
DISTRIBUTION: Native to the West Indies, particularly Dominica where the essential oil is produced.
OTHER SPECIES: There are several other varieties, for example the anise-scented and lemon-scented bay, the oils of which have a totally different chemical composition. Not to be confused with bay laurel, the common household spice, nor with the North American bayberry or wax myrtle (Myrcia cerifera) well known for its wax yielding berries.
HERBAL/FOLK TRADITION: The West Indian bay tree is often grown in groves together with the allspice or pimento bush, then the fruits of both are dried and powdered for the preparation of the household allspice. The so-called bay rum tree also provides the basic ingredient for the famous old hair tonic, which is made from the leaves by being distilled in rum. ‘A hair application with both fragrant and tonic virtues … useful for those who suffer from greasy hair and need a spirit-based, scalp-stimulating lotion to help them to control their locks!’9

EXTRACTION: Essential oil by water or steam distillation from the leaves. An oleoresin is also produced in small quantities.
CHARACTERISTICS: A dark yellow mobile liquid with a fresh-spicy top note and a sweet-balsamic undertone. It blends well with lavander, lavandin, rosemary, geranium, ylang ylang, citrus and spice oils.
PRINCIPAL CONSTITUENTS: Eugenol (up to 56 per cent), myrcene, chavicol and, in lesser amounts, methyl eugenol, linalol, limonene, among others.
SAFETY DATA: Moderately toxic due to high eugenol content; also a mucous membrane irritant – use in moderation only. Unlike bay laurel, however, it does not appear to cause dermal irritation or sensitization.

SYNONYMS: A. officinalis, European angelica, garden angelica.
GENERAL DESCRIPTION: A large hairy plant with ferny leaves and umbels of white flowers. It has a strong aromatic scent and a large rhizome.
DISTRIBUTION: Native to Europe and Siberia, cultivated mainly in Belgium, Hungary and Germany.
OTHER SPECIES: There are over thirty different types of angelica but this is the most commonly used medicinally. see Botanical Classification section.
HERBAL/FOLK TRADITION: This herb has been praised for its virtues since antiquity.
It strengthens the heart, stimulates the circulation and the immune system in general. It has been used for centuries in Europe for bronchial ailments, colds, coughs, indigestion, wind and to stimulate the appetite. As a urinary antiseptic it is helpful in cystitis and is also used for rheumatic inflammation. The Chinese employ at least ten kinds of angelica, well known for promoting fertility, fortifying the spirit and for treating female disorders generally; it has a reputation second only to ginseng. It is current in the British Herbal Pharmacopoeia as a specific for bronchitis associated with vascular deficiency. Candied Angelica stalks are popular in France and Spain.
ACTIONS: Antispasmodic, carminative, depurative, diaphoretic, digestive, diuretic, emmenagogue, expectorant, febrifuge, nervine, stimulant, stomachic, tonic. Reported to have bactericidal and fungicidal properties.
EXTRACTION: Essential oil produced by steam distillation from the 1. roots and rhizomes, and, 2. fruit or seed. An absolute is also produced on a small scale, from the roots.
CHARACTERISTICS: 1. A colourless or pale yellow oil which turns yellowy-brown with age, with a rich herbaceous-earthy bodynote. 2. The seed oil is a colourless liquid with a fresher, spicy top note. It blends well with patchouli, opopanax, costus, clary sage, oakmoss, vetiver and with citrus oils.
PRINCIPAL CONSTITUENTS: Root and seed oil contain phellandrene, pinene, limonene, linalol and borneol; rich in coumarins including osthol, angelicin, bergapten and imperatorin; also contains plant acids.
SAFETY DATA: Both root and seed oil are non toxic and non-irritant. The root oil (not the seed oil) is phototoxic, probably due to higher levels of bergapten. Not to be used during pregnancy or by diabetics.

SYNONYMS: B. alba var. pubescens, B. odorata, B. pendula, European white birch, silver birch.
GENERAL DESCRIPTION: Decorative tree, up to 15–20 metres high, with slender branches, silvery-white bark broken into scales, and light green oval leaves. The male catkins are 2–5 cms long, the female up to 15 cms long.
DISTRIBUTION: Native to the northern hemisphere; found throughout Eastern Europe, Russia, Germany, Sweden, Finland, the Baltic coast, northern China and Japan.
OTHER SPECIES: Many cultivars exist of this species of birch. The paper birch (B. papyrifera) and B. verrucosa are also used for the production of birch bud oil and/or birch tar. NB Should not be confused with the oil from the sweet birch (B. lenta) which is potentially toxic.
HERBAL/FOLK TRADITION: Birch buds were formerly used as a tonic in hair preparations. Birch tar is used in Europe for all types of chronic skin complaints: psoriasis, eczema, etc. In Scandinavia the young birch leaflets and twigs are bound into bundles and used in the sauna to tone the skin and promote the circulation. The sap is also tapped in the spring and drunk as a tonic. Buds, leaves and bark are used for ‘rheumatic and arthritic conditions, especially where kidney functions appear to need support … oedematous states; urinary infections and calculi.’.
ACTIONS: Anti-inflammatory, antiseptic, cholagogue, diaphoretic, diuretic, febrifuge, tonic.
EXTRACTION: 1. Essential oil by steam distillation from the leaf-buds. 2. Crude birch tar is extracted by slow destructive distillation from the bark; this is subsequently steam distilled to yield a rectified birch tar oil.
CHARACTERISTICS: 1. Pale yellow, viscous oil with a woody-green balsamic scent. It crystallizes at low temperatures. 2. The crude tar is an almost black, thick oily mass. The rectified oil is a brownish-yellow, clear oily liquid with a smoky, tar-like, ‘Russian leather’ odour. It blends well with other woody and balsamic oils.
PRINCIPAL CONSTITUENTS: 1. Mainly betulenol and other sesquiterpenes. 2. In the tar oil: phenol, cresol, xylenol, guaiacol, creosol, pyrocatechol, pyrobetulin (which gives the ‘leather’ scent).

GENERAL DESCRIPTION: A small tree, about 4.5 metres high with smooth oval leaves, bearing small round fruit which ripen from green to yellow, much like a miniature orange in appearance.
DISTRIBUTION: Native to tropical Asia. Extensively cultivated in Calabria in southern Italy and also grown commercially on the Ivory Coast.
OTHER SPECIES: Not to be confused with the herb bergamot or bee balm (Monarda didyma).
HERBAL/FOLK TRADITION: Named after the Italian city of Bergamo in Lombardy, where the oil was first sold. The oil has been used in Italian folk medicine for many years, primarily for fever (including malaria) and worms; it does not feature in the folk tradition of any other countries. However, due to recent research in Italy, bergamot oil is now known to have a wide spectrum of applications, being particularly useful for mouth, skin, respiratory and urinary tract infections.

EXTRACTION: Essential oil by cold expression of the peel of the nearly ripe fruit. (A rectified or terpeneless oil is produced by vacuum distillation or solvent extraction.)
CHARACTERISTICS: A light greenish-yellow liquid with a fresh sweet-fruity, slightly spicy-balsamic undertone. On ageing it turns a brownish-olive colour. It blends well with lavender, neroli, jasmine, cypress, geranium, lemon, chamomile, juniper, coriander and violet.
PRINCIPAL CONSTTTUENTS Known to have about 300 compounds present in the expressed oil: mainly linalyl acetate (30–60 per cent), linalol (11–22 per cent) and other alcohols, sesquiterpenes, terpenes, alkanes and furocoumarins (including bergapten, 0.30–0.39 per cent).
SAFETY DATA: Certain furocoumarins, notably bergapten, have been found to be phototoxic on human skin; that is, they cause sensitization and skin pigmentation when exposed to direct sunlight (in concentration and in dilution even after some time!). Extreme care must be taken when using the oil in dermal applications – otherwise a rectified or ‘bergapten-free’ oil should be substituted. Available information indicates it to be otherwise non-toxic and relatively non-irritant.

FAMILY: Cupressaceae
SYNONYMS: Juniper tar, prickly cedar, medlar tree, prickly juniper.
GENERAL DESCRIPTION: A large evergreen shrub up to 4 metres high, with long dark needles and brownish-black berries about the size of hazelnuts.
DISTRIBUTION: Native to southern France; now common throughout Europe and North Africa. The tar is produced mainly in Spain and Yugoslavia.
OTHER SPECIES: There are many varieties of juniper which are used commercially apart from the prickly juniper: J. communis produces juniper oil, J. virginiana produces Virginian cedarwood oil, and in Yugoslavia an oil is produced from the fruits and twigs of J. smreka.
HERBAL/FOLK TRADITION: Used in the treatment of cutaneous diseases, such as chronic eczema, parasites, scalp disease, hair loss, etc. especially in France and other continental countries. It is also used as an antiseptic wound dressing and for toothache.
ACTIONS: Analgesic, antimicrobial, antipruritic, antiseptic, disinfectant, parasiticide, vermifuge.
EXTRACTION: The crude oil or tar is obtained by destructive distillation from the branches and heartwood (usually in the form of shavings or chips). A rectified oil is produced from the crude by steam or vacuum distillation. In addition, an oil is occasionally produced from the berries by steam distillation.
CHARACTERISTICS: The rectified oil is an orange-brown, oily liquid with a woody, smoky, leatherlike odour. It blends well with thyme, origanum, clove, cassia, tea tree, pine and medicinal-type bases.
PRINCIPAL CONSTITUENTS: Cadinene, cadinol, p-cresol, guaiacol, among others.
SAFETY DATA: Non-toxic, non-irritant, possible sensitization problems. Use with care, especially when treating inflammatory or allergic skin conditions. Turpentine (terebinth) oil makes a useful alternative, with less possibility of an allergic reaction.

FAMILY: Apiaceae (Umbelliferae)
SYNONYMS: F. gummosa, galbanum gum, galbanum resin, ‘bubonion’.
GENERAL DESCRIPTION: A large perennial herb with a smooth stem, shiny leaflets and small flowers. It contains resin ducts which exude a milky juice, a natural oleoresin. The dried resinous exudation is collected by making incisions at the base of the stem.
DISTRIBUTION: Native to the Middle East and western Asia; cultivated in Iran, Turkey, Afghanistan and Lebanon. Distillation usually takes place in Europe or the USA.
OTHER SPECIES: There are two distinct types: Levant galbanum which is liquid or soft, and Persian galbanum which is solid or hard. Other Ferula species also yield galbanum gum, such as the muskroot; see also Botanical Classification section.
HERBAL/FOLK TRADITION: It was used by the ancient civilizations as an incense, and in Egypt for cosmetics and in the embalming process. It is generally used in the east in a similar way to asafetida: for treating wounds, inflammations and skin disorders and also for respiratory, digestive and nervous complaints. Zalou root (F. hermonic) is used in Beirut as an aphrodisiac.
ACTIONS: Analgesic, anti-inflammatory, antimicrobial, antiseptic, antispasmodic, aphrodisiac, balsamic, carminative, cicatrisant, digestive, diuretic, emmenagogue, expectorant, hypotensive, restorative, tonic.
EXTRACTION: Essential oil by water or steam distillation from the oleoresin or gum – only the Levant or soft type is used for oil production. A partially deterpenized oil is produced, known as ‘galbanol’. (A resinoid is also produced, mainly for use as a fixative.)
CHARACTERISTICS: Crude – A dark amber or brown viscous liquid with a green-woody scent and a soft balsamic undertone. Oil – A colourless, or pale yellow or olive liquid with a fresh green topnote and woody-dry balsamic undertone. It blends well with hyacinth, violet, narcissus, lavender, geranium, oakmoss, opopanax, pine, fir, styrax and oriental bases.
PRINCIPAL CONSTITUENTS: Pinene, cadinol, cadinene and myrcene, among others.
SAFETY DATA: Non-toxic, non-irritant, non-sensitizing.

FAMILY: Cistaceae
SYNONYMS: Cistus (oil), gum cistus, ciste, cyste (absolute), labdanum gum, ambreine, European rock rose.
GENERAL DESCRIPTION: A small sticky shrub up to 3 metres high with lance-shaped leaves which are white and furry on the underside, and fragrant white flowers. Labdanum gum, a dark brown solid mass, is a natural oleoresin which is obtained by boiling the plant material in water.
DISTRIBUTION: Native to the Mediterranean mountainous regions and the Middle East. Now found throughout the Mediterranean region, especially southern France, Spain, Portugal, Greece, Morocco, Cyprus and Yugoslavia. The oil is mainly produced in Spain.
OTHER SPECIES: Labdanum gum is also obtained from other Cistus species, notably C. incanus, and other subspecies: see Botanical Classification section.
HERBAL/FOLK TRADITION: One of the early aromatic substances of the ancient world. The gum was used formerly for catarrh, diarrhoea, dysentery and to promote menstruation; externally it was used in plasters. The oil from the closely related plant frostwort (Helianthemum canadense), also known as cistus, also has many medicinal qualities and is said to be useful for scrofulous skin conditions, ulcers and tumours, including cancer.
ACTIONS: Antimicrobial, antiseptic, antitussive, astringent, balsamic, emmenagogue, expectorant, tonic.
EXTRACTION: 1. A resinoid or resin concrete and absolute by solvent extraction from the crude gum. 2. An essential oil by steam distillation from the crude gum, the absolute, or from the leaves and twigs of the plant directly.
CHARACTERISTICS: 1. Absolute – a semi solid green or amber mass with a rich, sweet, herbaceous-balsamic odour. 2. Oil – a dark yellow or amber viscous liquid with a warm, sweet, dry-herbaceous musky scent. It blends well with oakmoss, clary sage, pine, juniper, calamus, opopanax, lavender, lavandin, bergamot, cypress, vetiver, sandalwood, patchouli, olibanum, chamomile maroc and oriental bases.
PRINCIPAL CONSTITUENTS: It contains over 170 pinenes, including camphene, sabinene, myrcene, phellandrene, limonene, cymene, cineol, borneol, nerol, geraniol, fenchone, etc. Exact constituents vary according to source.
SAFETY DATA: Generally non-toxic, non-irritant, non-sensitizing. Avoid during pregnancy.

SYNONYMS: L. vera, L. officinalis, garden lavender, common lavender
GENERAL DESCRIPTION: An evergreen woody shrub, up to 1 metre tall, with pale green, narrow, linear leaves and flowers on blunt spikes of a beautiful violet-blue colour. The whole plant is highly aromatic.
DISTRIBUTION: Indigenous to the Mediterranean region, now cultivated all over the world. The oil is produced mainly in France, also Spain, Italy, England, Australia, Tasmania, Yugoslavia, Turkey, Russia, Bulgaria, Greece, etc.
OTHER SPECIES: There are many varieties of lavender; L. angustifolia is divided into two subspecies – L. delphinensis and L. fragrans. French lavender (L. stoechas) is a smaller shrub with dark violet flowers; see also entries on spike lavender, lavandin and the Botanical Classification section. The so-called cotton lavender (Santolina chamaecyparissus) and the sea lavender (Statice caroliniana) belong to different botanical families.
HERBAL/FOLK TRADITION: Lavender has a well-established tradition as a folk remedy, and its scent is still familiar to almost everyone. It was used to ‘comfort the stomach’ but above all as a cosmetic water, an insect repellent, to scent linen, and as a reviving yet soothing oil ‘The essential oil, or a spirit of lavender made from it, proves admirably restorative and tonic against faintness, palpitations of a nervous sort, weak giddiness, spasms and colic ... A few drops of lavender in a hot footbath has a marked influence in relieving fatigue. Outwardly applied, it relieves toothache, neuralgia, sprains and rheumatism. In hysteria, palsy and similar disorders of debility and lack of nerve power, lavender will act as a powerful stimulant.’.

CHARACTERISTICS: 1. The oil is a colourless to pale yellow liquid with a sweet, floral herbaceous scent and balsamic-woody undertone; it has a more fragrant floral scent compared to spike lavender. It blends well with most oils, especially citrus and florals; also cedarwood, clove, clary sage, pine, geranium, labdanum, oakmoss, vetiver, patchouli, etc. 2. The absolute is a dark green viscous liquid with a very sweet herbaceous, somewhat floral odour.
PRINCIPAL CONSTITUENTS: Over 100 constituents including linalyl acetate (up to 40 per cent), linalol, lavandulol, lavandulyl acetate, terpineol, cineol, limonene, ocimene, caryophyllene, among others. Constituents vary according to source: high altitudes generally produce more esters.

SYNONYMS: M. officinalis, M. aromata, Nux moschata, myristica (oil), mace (husk), macis (oil).
GENERAL DESCRIPTION: An evergreen tree up to 20 metres high with a greyish-brown smooth bark, dense foliage and small dull-yellow flowers. ‘Mace’ is the name given to the bright red netlike aril or husk surrounding the nutmeg shell and seed, which is contained within the fleshy fruit.
DISTRIBUTION: Native to the Moluccas and nearby islands; cultivated in Indonesia, Sri Lanka and the West Indies, especially Grenada.
EXTRACTION: Essential oil by steam (or water) distillation from 1. the dried worm eaten nutmeg seed (the worms eat away all the starch and fat content); 2. the dried orangebrown aril or husk – mace; and 3. an oleoresin is also produced in small quantities by solvent extraction from mace. The oil is also distilled in the USA and Europe from the imported nutmegs.
OTHER SPECIES: Indonesia and Sri Lanka produce the so-called ‘East Indian’ nutmeg which is considered superior, while Grenada produces the ‘West Indian’ nutmeg – see also Botanical Classification.
HERBAL/FOLK TRADITION: Nutmeg and mace are widely used as domestic spices in the East and West. They have been used for centuries as a remedy mainly for digestive and kidney problems. In Malaysia they are used during pregnancy to strengthen and tone the uterine muscles. Grated nutmeg with lard is used for piles. A fixed oil of nutmeg is also used in soap and candle making.
Nutmeg is current in the British Herbal Pharmacopoeia indicated for flatulent dyspepsia, nausea, diarrhoea, dysentery, and topically for rheumatism.
ACTIONS: Analgesic, anti-emetic, antioxidant, antirheumatic, antiseptic, antispasmodic, aphrodisiac, carminative, digestive, emmenagogue, gastric secretory stimulant, larvicidal, orexigenic, prostaglandin inhibitor, stimulant, tonic.
CHARACTERISTICS: 1. A water-white or pale yellow mobile liquid with a sweet, warm-spicy odour and a terpeney top-note. 2. A water white or pale yellow mobile liquid with a sweet, warm-spicy scent. 3. An orange-brown viscous liquid with a fresh, spicy-warm, balsamic fragrance. It has good masking power.
They blend well with oakmoss, lavandin, bay leaf, Peru balsam, orange, geranium, clary sage, rosemary, lime, petitgrain, mandarin, coriander and other spice oils.
PRINCIPAL CONSTITUENTS: Mainly monoterpene hydrocarbons (88 per cent approx.): camphene, pinene, dipentene, sabinene, cymene, with lesser amounts of geraniol, borneol, linalol, terpineol, myristicin (4–8 per cent), safrol and elemincin, among others. Mace oil contains similar constituents but contains more myristicin.
SAFETY DATA: Both nutmeg and mace are generally non-toxic, non-irritant and non sensitizing. However, used in large doses they show signs of toxicity such as nausea, stupor and tachycardia, believed to be due to the myristicin content. ‘Large quantities are hallucinogenic and excitant to the motor cortex.’. On this basis nutmeg (especially the West Indian type) is probably safer to use than mace. Use in moderation, and with care in pregnancy.

SYNONYMS: Terebinth, therebentine, gum thus, gum turpentine, turpentine balsam, spirit of turpentine (oil).
GENERAL DESCRIPTION: ‘Gum turpentine’ is a term loosely applied to the natural oleoresin formed as a physiological product in the trunks of various Pinus, Picea and Abies species. Turpentine refers both to the crude oleoresin (a mixture of oil and resin) and to the distilled and rectified essential oils.
DISTRIBUTION: All over the world. The largest producer is the USA, also Mexico, France, Portugal, Spain, Greece, Scandinavia, New Zealand, Tasmania, India, China, the USSR, etc.
OTHER SPECIES: Apart from the longleaf pine (Pinus palustris), which is the leading source of American gum turpentine, other sources in the USA include the slash pine (P. elliottii) and the Mexican white pine (P. ayacahuite). In India the chir pine (P. roxburghii); in Tasmania the lodgepole pine (P. contorta var. latifolia); in China the masson or Southern red pine (P. massoniana); in Europe and Scandinavia the Scotch pine (P. sylvestris) and the sea pine (P. pinaster), as well as many others. HERBAL/FOLK TRADITION: Known to Galen and Hippocrates for its many applications, especially with regard to pulmonary and genito-urinary infections, digestive complaints and externally as a treatment for rheumatic or neuralgic pain and skin conditions. In China the oleoresin has been used (both internally and externally) for centuries for excess phlegm, bronchitis, rheumatism, stiff joints, toothache, boils, sores, ringworm and dermatitis.
The turpentine essence or spirit of turpentine is said to be four times more active than the crude turpentine.
ACTIONS: Analgesic, antimicrobial, antirheumatic, antiseptic, antispasmodic, balsamic, diuretic, cicatrisant, counter-irritant, expectorant, haemostatic, parasiticide, rubefacient, stimulant, tonic, vermifuge.
EXTRACTION: Essential oil by steam (or water) distillation from the crude oleoresin, then rectified. ‘It has to be purified because it is viscous, coloured and acidic.’.
CHARACTERISTICS: A colourless, water white mobile liquid with a fresh, warm balsamic, familiar odour.
PRINCIPAL CONSTITUENTS: Mainly alphapinene (approx. 50 per cent), betapinene (25–35 per cent) and carene (20–60 per cent) in the American oils. In European oils the alpha pinene can constitute up to 95 per cent – constituents vary according to source.
SAFETY DATA: Environmental hazard – marine pollutant. Relatively non-toxic and non-irritant; possible sensitization in some individuals. Avoid therapeutic use or employ in moderation only.
