Strongyloidiasis: From 3 Different Sources


An infestation of the intestines by the parasitic worm STRONGYLOIDES STERCORALIS. It is widespread in the tropics. Strongyloidiasis is contracted in affected areas by walking barefoot on soil contaminated with faeces. Larvae penetrate the soles, migrating via the lungs and throat to the intestine. Here they develop into adults and produce larvae. Most larvae are passed in the faeces, but some enter the skin around the anus to begin a new cycle. A person may be infested for more than 40 years.

The larvae cause itching and red weals where they enter the skin. In the lungs they may cause asthma or pneumonia. Heavy intestinal infestation may cause swelling of the abdomen and diarrhoea. Occasionally, an infected person with reduced immunity dies of complications, such as septicaemia or meningitis.

Treatment with an anthelmintic drug, usually tiabendazole, kills the worms.

This infection is caused by nematode worms of the genus Strongyloides spp. – the great majority being from S. stercoralis. This helminth is present throughout most tropical and subtropical countries; a single case report has been made in England – about an individual who had not been exposed to such an environment. Larvae usually penetrate intact skin, especially the feet (as with hookworm infection). Unlike hookworm infection, eggs mature and hatch in the lower gastrointestinal tract; thus larvae can immediately re-enter the circulation in the colo-rectum or perianal region, setting up an auto-infection cycle. Therefore, infection can continue for the remaining lifespan of the individual. Severe malnutrition may be a predisposing factor to infection, as was the case in prisoners of war in south-east Asia during World War II.

Whilst an infected patient is frequently asymptomatic, heavy infection can cause jejunal mucosal abnormalities, and an absorptive defect, with weight loss. During the migratory phase an itchy linear rash (larva currens) may be present on the lower abdomen, buttocks, and groins; this gives rise to recurrent transient itching. In an immunosuppressed individual, the ‘hyperinfection syndrome’ may ensue; migratory larvae invade all organs and tissues, including the lungs and brain. Associated with this widespread infection, the patient may develop an Enterobacteriacae spp. SEPTICAEMIA; this, together with S. stercoralis larvae, produces a MENINGOENCEPHALITIS. There is no evidence that this syndrome is more common in patients with HIV infection.

Diagnosis consists of visualisation of S. stercoralis (larvae or adults) in a jejunal biopsy-section or aspirate. Larvae may also be demonstrable in a faecal sample, especially following culture. Eosinophilia may be present in peripheral blood, during the invasive stage of infection. Chemotherapy consists of albendazole. The formerly used benzimidazole compound, thiabendazole, is now rarely prescribed in an uncomplicated infection due to unpleasant side-effects; even so, in the ‘hyperinfection syndrome’ it probably remains the more e?ective of the two compounds.

(strongyloidosis) n. an infestation of the small intestine with the parasitic nematode worm Strongyloides stercoralis, common in humid tropical regions. Larvae, present in soil contaminated with human faeces, penetrate the skin of a human host and may produce an itching rash. They migrate to the lungs, where they cause tissue destruction and bleeding, and then via the windpipe and gullet to the intestine. Adult worms burrow into the intestinal wall and may cause ulceration, diarrhoea, abdominal pain, nausea, anaemia, and weakness. Treatment involves use of ivermectin or albendazole.

Albendazole

A drug adjunct to surgery in the treatment of hydatid cysts (see under CYSTS) caused by Taenia echinococcus, a small tapeworm (see TAENIASIS). If surgery is not possible, albendazole can be used on its own. The drug is also used to treat STRONGYLOIDIASIS.... albendazole

Creeping eruption is a skin condition caused by the invasion of the skin by the larvae of various species of nematode worms. Thiabendazole

The drug of choice for adults infected with the intestinal parasite Strongyloides stercoralis (see STRONGYLOIDIASIS). Its side-effects, including ANOREXIA, nausea, vomiting, diarrhoea, abdominal pain, itching and drowsiness, are more troublesome in elderly patients.... thiabendazole

Tropical Diseases

Technically, those diseases occurring in the area of the globe situated between the Tropic of Cancer and the Tropic of Capricorn: pertaining to the sun. They include many ‘exotic’ infections – many of them parasitic in origin – which fall under the umbrella of ‘TROPICAL MEDICINE’. However, disease in the tropics is far broader than this and includes numerous other infections, many of them with a viral or bacterial basis: for example, the viral hepatidises, streptococcal and pneumococcal infections, and tuberculosis. The prevalence of other diseases, such as rheumatic cardiac disease, cirrhosis, heptocellular carcinoma (‘hepatoma’), and various nutrition-related problems, is also much increased in most areas of the tropics. With people from developed countries increasingly travelling to worldwide destinations for business and holiday, the ‘importation’ of tropical diseases to temperate climates should be borne in mind when people fall ill.

Tiabendazole

Ivermectin

n. a drug used in the treatment of *onchocerciasis. It acts by killing the immature forms (*microfilariae) of the parasite. Ivermectin is also used to treat creeping eruption, strongyloidiasis, and scabies. Side-effects, which are mild, include itching and swollen lymph nodes.... ivermectin

Strongyloides

The virus enters the bloodstream and infects cells that have a particular receptor, known as the CD4 receptor, on their surface. These cells include a type of white blood cell (a CD4 lymphocyte) responsible for fighting infection and cells in other tissues such as the brain. The virus reproduces within the infected cells, which then die, releasing more virus particles into the blood. If the infection is left untreated, the number of CD4 lymphocytes falls, resulting in greater susceptibility to certain infections and some types of cancer.

Some people experience a short-lived illness similar to infectious mononucleosis when they are first infected with. Many individuals have no obvious symptoms; some have only vague complaints, such as weight loss, fevers, sweats, or unexplained diarrhoea, described as -related complex.

Minor features of infection include skin disorders such as seborrhoeic dermatitis. More severe features include persistent herpes simplex infections, oral candidiasis (thrush), shingles, tuberculosis, and shigellosis. may also affect the brain, causing a variety of neurological disorders, including dementia.

Certain conditions, known as AIDSdefining illnesses, are characteristic of full-blown. These include cancers (Kaposi’s sarcoma and lymphoma of the brain), and various infections (pneumocystis pneumonia, cytomegalovirus infection, toxoplasmosis, diarrhoea due to CRYPTOSPORIDIUM or ISOSPORA, candidiasis, disseminated strongyloidiasis, and cryptococcosis), many of which are described as opportunistic infections.

Confirmation of infection involves testing a blood sample for the presence of antibodies to. Diagnosis of fullblown is based on a positive test along with the presence of an AIDSdefining illness.

The risk of infection with can be reduced by practising safer sex. Intravenous drug users should not share needles. There is a small risk to health workers handling infected blood products or needles, but this risk can be minimized by safe practices.

Treatment of infection with a combination of antiviral drugs can slow the disease’s progress, and may prevent the development of full-blown. The 2 main types of antiviral drug used are protease inhibitors, such as indinavir, and reverse transcriptase inhibitors such as zidovudine.
