In developed countries people are living longer, in part because infant and child mortality rates have dropped dramatically over the past 100 years or so. Improved standards of living and more e?ective health care have also contributed to greater longevity: the proportion of people over 65 years of age has greatly increased, and that of the over-75s is still rising. The 2001 census found 336,000 people in the UK aged over 90 and there are 36,000 centenarians in the US. This extreme longevity is attributed to a particular gene (see GENES) slowing the ageing process. Interestingly, those living to 100 often retain the mental faculties of people in their 60s, and examination of centenarians’ brains show that these are similar to those of 60-year-olds. Cancers are classi?ed according to the type of cell from which they are derived as well as the organ of origin. Hence cancers arising within the bronchi, often collectively referred to as ‘lung cancer’, include both adenocarcinomas, derived from epithelium (surface tissue), and carcinomas from glandular tissue. Sarcomas are cancers of connective tissue, including bone and cartilage. The behaviour of cancers and their response to therapy vary widely depending on this classi?cation as well as on numerous other factors such as how large the cancer is, how fast the cells grow and how well de?ned they are. It is entirely wrong to see cancer as a single disease entity with a universally poor prognosis. For example, fewer than one-half of women in whom breast cancer (see BREASTS, DISEASES OF) is discovered will die from the disease, and 75 per cent of children with lymphoblastic LEUKAEMIA can be cured.
Incidence In most western countries, cancer is the second most important cause of death after heart disease and accounts for 20–25 per cent of all deaths. In the United Kingdom in 2003, more than 154,000 people died of malignant disease. There is wide international variation in the most frequently encountered types of cancer, re?ecting the importance of environmental factors in the development of cancer. In the UK as well as the US, carcinoma of the BRONCHUS is the most common. Since it is usually inoperable at the time of diagnosis, it is even more strikingly the leading cause of cancer deaths. In women, breast cancer was for a long time the most common malignant disease, accounting for a quarter of all cancers, but ?gures for the late 1990s show that lung cancer now heads the incidence list – presumably the consequence of a rising incidence of smoking among young women. Other common sites are as follows: males – colon and rectum, prostate and bladder; females – colon and rectum, uterus, ovary and pancreas.
In 2003, of the more than 154,000 people in the UK who died of cancer, over 33,000 had the disease in their respiratory system, nearly 13,000 in the breast, over 5,800 in the stomach and more than 2,000 in the uterus or cervix, while over 4,000 people had leukaemia. The incidence of cancer varies with age; the older a person is, the more likely it is that he or she will develop the disease. The over-85s have an incidence about nine times greater than those in the 25–44 age group. There are also di?erences in incidence between sexes: for example, more men than women develop lung cancer, though the incidence in women is rising as the effects of smoking work through. The death rate from cancer is falling in people under 75 in the UK, a trend largely determined by the cancers which cause the most deaths: lung, breast, colorectal, stomach and prostate.
Causes In most cases the causes of cancer remain unknown, though a family history of cancer may be relevant. Rapid advances have, however, been made in the past two decades in understanding the di?erences between cancer cells and normal cells at the genetic level. It is now widely accepted that cancer results from acquired changes in the genetic make-up of a particular cell or group of cells which ultimately lead to a failure of the normal mechanisms regulating their growth. It appears that in most cases a cascade of changes is required for cells to behave in a truly malignant fashion; the critical changes affect speci?c key GENES, known as oncogenes, which are involved in growth regulation. (See APOPTOSIS.)
Since small genetic errors occur within cells at all times – most but not all of which are repaired – it follows that some cancers may develop as a result of an accumulation of random changes which cannot be attributed to environmental or other causes. The environmental factors known to cause cancer, such as radiation and chemicals (including tar from tobacco, asbestos, etc.), do so by increasing the overall rate of acquired genetic damage. Certain viral infections can induce speci?c cancers (e.g. HEPATITIS B VIRUS and HEPATOMA, EPSTEIN BARR VIRUS and LYMPHOMA) probably by inducing alterations in speci?c genes. HORMONES may also be a factor in the development of certain cancers such as those of the prostate and breast. Where there is a particular family tendency to certain types of cancer, it now appears that one or more of the critical genetic abnormalities required for development of that cancer may have been inherited. Where environmental factors such as tobacco smoking or asbestos are known to cause cancer, then health education and preventive measures can reduce the incidence of the relevant cancer. Cancer can also affect the white cells in the blood and is called LEUKAEMIA.
Treatment Many cancers can be cured by surgical removal if they are detected early, before there has been spread of signi?cant numbers of tumour cells to distant sites. Important within this group are breast, colon and skin cancer (melanoma). The probability of early detection of certain cancers can be increased by screening programmes in which (ideally) all people at particular risk of development of such cancers are examined at regular intervals. Routine screening for CERVICAL CANCER and breast cancer (see BREASTS, DISEASES OF) is currently practised in the UK. The e?ectiveness of screening people for cancer is, however, controversial. Apart from questions surrounding the reliability of screening tests, they undoubtedly create anxieties among the subjects being screened.
If complete surgical removal of the tumour is not possible because of its location or because spread from the primary site has occurred, an operation may nevertheless be helpful to relieve symptoms (e.g. pain) and to reduce the bulk of the tumour remaining to be dealt with by alternative means such as RADIOTHERAPY or CHEMOTHERAPY. In some cases radiotherapy is preferable to surgery and may be curative, for example, in the management of tumours of the larynx or of the uterine cervix. Certain tumours are highly sensitive to chemotherapy and may be cured by the use of chemotherapeutic drugs alone. These include testicular tumours, LEUKAEMIA, LYMPHOMA and a variety of tumours occurring in childhood. These tend to be rapidly growing tumours composed of primitive cells which are much more vulnerable to the toxic effects of the chemotherapeutic agents than the normal cells within the body.
Unfortunately neither radiotherapy nor currently available chemotherapy provides a curative option for the majority of common cancers if surgical excision is not feasible. New e?ective treatments in these conditions are urgently needed. Nevertheless the rapidly increasing knowledge of cancer biology will almost certainly lead to novel therapeutic approaches – including probably genetic techniques utilising the recent discoveries of oncogenes (genes that can cause cancer). Where cure is not possible, there often remains much that can be done for the cancer-sufferer in terms of control of unpleasant symptoms such as pain. Many of the most important recent advances in cancer care relate to such ‘palliative’ treatment, and include the establishment in the UK of palliative care hospices.

About the Nutrients in This Food Grapefruit and ugli fruit (a cross between the grapefruit and the tangerine) have moderate amounts of dietary fiber and, like all citrus fruits, are most prized for their vitamin C. Pink or red grapefruits have moderate amounts of vitamin A. One-half medium (four-inch diameter) pink grapefruit has 1.4 g dietary fiber, 1,187 IU vitamin A (51 percent of the R DA for a woman, 40 percent of the R DA for a man), and 44 mg vitamin C (59 percent of the R DA for a woman, 49 percent of the R DA for a man). One half medium (3.75-inch diameter) white grapefruit has 1.3 g dietary fiber, 39 IU vitamin A (2 percent of the R DA for a woman, 1 percent of the R DA for a man), and 39 mg vitamin C (52 percent of the R DA for a woman, 43 percent of the R DA for a man). Pink and red grapefruits also contain lycopene, a red carotenoid (plant pigment), a strong antioxidant that appears to lower the risk of cancer of the prostate. The richest source of lycopene is cooked tom atoes.
The Most Nutritious Way to Serve This Food Fresh fruit or fresh-squeezed juice.
Buying This Food Look for: Firm fruit that is heavy for its size, which means that it will be juicy. The skin should be thin, smooth, and fine-grained. Most grapefruit have yellow skin that, depending on the variety, may be tinged with red or green. In fact, a slight greenish tint may mean that the grapefruit is high in sugar. Ugli fruit, which looks like misshapen, splotched grapefruit, is yellow with green patches and bumpy skin. Avoid: Grapefruit or ugli fruit with puff y skin or those that feel light for their size; the flesh inside is probably dry and juiceless.
Storing This Food Store grapefruit either at room temperature (for a few days) or in the refrigerator. Refrigerate grapefruit juice in a tightly closed glass bottle with very little air space at the top. As you use up the juice, transfer it to a smaller bottle, again with very little air space at the top. The aim is to prevent the juice from coming into contact with oxygen, which destroys vitamin C. (Most plastic juice bottles are oxygen-permeable.) Properly stored and protected from oxygen, fresh grapefruit juice can hold its vitamin C for several weeks.
Preparing This Food Grapefruit are most flavorful at room temperature, which liberates the aromatic molecules that give them their characteristic scent and taste. Before cutting into the grapefruit, rinse it under cool running water to flush debris off the peel. To section grapefruit, cut a slice from the top, then cut off the peel in strips—starting at the top and going down—or peel it in a spiral fashion. You can remove the bitter white membrane, but some of the vitamin C will go with it. Finally, slice the sections apart. Or you can simply cut the grapefruit in half and scoop out the sections with a curved, serrated grapefruit knife.
What Happens When You Cook This Food Broiling a half grapefruit or poaching grapefruit sections reduces the fruit’s supply of vitamin C, which is heat-sensitive.
How Other Kinds of Processing Affect This Food Commercially prepared juices. How well a commercially prepared juice retains its vitamin C depends on how it is prepared, stored, and packaged. Commercial flash-freezing preserves as much as 95 percent of the vitamin C in fresh grapefruit juices. Canned juice stored in the refrigerator may lose only 2 percent of its vitamin C in three months. Prepared, pasteurized “fresh” juices lose vitamin C because they are sold in plastic bottles or waxed-paper cartons that let oxygen in. Commercially prepared juices are pasteurized to stop the natural enzyme action that would otherwise turn sugars to alcohols. Pasteurization also protects juices from potentially harmful bacterial and mold contamination. Following several deaths attributed to unpas- teurized apple juices containing E. coli O157:H7, the FDA ruled that all fruit and vegetable juices must carry a warning label telling you whether the juice has been pasteurized. Around the year 2000, all juices must be processed to remove or inactivate harmful bacteria.
Medical Uses and/or Benefits Antiscorbutic. All citrus fruits are superb sources of vitamin C, the vitamin that prevents or cures scurvy, the vitamin C-deficiency disease. Increased absorption of supplemental or dietary iron. If you eat foods rich in vitamin C along with iron supplements or foods rich in iron, the vitamin C will enhance your body’s ability to absorb the iron. Wound healing. Your body needs vitamin C in order to convert the amino acid proline into hydroxyproline, an essential ingredient in collagen, the protein needed to form skin, ten- dons, and bones. As a result people with scurvy do not heal quickly, a condition that can be remedied with vitamin C, which cures the scurvy and speeds healing. Whether taking extra vitamin C speeds healing in healthy people remains to be proved. Possible inhibition of virus that causes chronic hepatitis C infection. In Januar y 2008, research- ers at Massachusetts General Hospital Center for Engineering in Medicine (Boston) published a report in the medical journal Hepatology detailing the effect of naringenin, a compound in grapefruit, on the behavior of hepatitis viruses in liver cells. In laborator y studies, naringenin appeared to inhibit the ability of the virus to multiply and/or pass out from the liver cells. To date, there are no studies detailing the effect of naringenin in human beings with hepatitis C.
Adverse Effects Associated with This Food Contact dermatitis. The essential oils in the peel of citrus fruits may cause skin irritation in sensitive people.
Food/Drug Interactions Aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen, naproxen and others. Taking aspirin or NSAIDs with acidic foods such as grapefruit may intensif y the drug’s ability to irritate your stomach and cause gastric bleeding. Antihistamines, anticoagulants, benzodiazepines (tranquilizers or sleep medications), calcium channel blockers (blood pressure medication), cyclosporine (immunosuppressant drug used in organ transplants), theophylline (asthma drug). Drinking grapefruit juice with a wide variety of drugs ranging from antihistamines to blood pressure medication appears to reduce the amount of the drug your body metabolizes and eliminates. The “grapefruit effect” was first identified among people taking the antihypertensive drugs felodipine (Plendil) and nifedip- ine (Adalat, Procardia). It is not yet known for certain exactly what the active substance in the juice is. One possibility, however, is bergamottin, a naturally occurring chemical in grapefruit juice known to inactivate cytochrome P450 3A4, a digestive enzyme needed to convert many drugs to water-soluble substances you can flush out of your body. Without an effective supply of cytochrome P450 3A4, the amount of a drug circulating in your body may rise to dangerous levels. Reported side effects include lower blood pressure, increased heart rate, headache, flushing, and lightheadedness. An antimicrobial constituent, mas- linic acid, alongwith gallic acid and pyrogallol, has been isolated from the leaves. Alcoholic extract of the leaves completely inhibited Micrococcus pyo- genes, but was inactive against E. coli. The aqueous extract, however, showed partial inhibition against E. coli. The aqueous extract also showed potent inhibitory activity towards lens aldose re- ductase, which plays an important role in the management of diabetic complications. The activity is attributed to a flavonoid, isoscutelarein.Bixin, the main constituent of seed coat, shows cytostatic effect on the growth of human lymphoma cells. About the Nutrients in This Food Apples are a high-fiber fruit with insoluble cellulose and lignin in the peel and soluble pectins in the flesh. Their most important vitamin is vitamin C. One fresh apple, 2.5 inches in diameter, has 2.4 g dietary fiber and 4.6 mg vitamin C (6 percent of the R DA for a woman, 5 percent of the R DA for a man). The sour taste of all immature apples (and some varieties, even when ripe) comes from malic acid. As an apple ripens, the amount of malic acid declines and the apple becomes sweeter. Apple seeds contain amygdalin, a naturally occurring cyanide/sugar compound that degrades into hydrogen cyanide. When an apple is damaged the injured cells release polyphenoloxi- dase, an enzyme that hastens the oxidation of phenols in the apple, producing brownish pigments that darken the fruit. It’s easy to check loose apples; if you buy them packed in a plastic bag, turn the bag upside down and examine the fruit.
Storing This Food Store apples in the refrigerator. Cool storage keeps them from losing the natural moisture that makes them crisp. It also keeps them from turning brown inside, near the core, a phe- nomenon that occurs when apples are stored at warm temperatures. Apples can be stored in a cool, dark cabinet with plenty of circulating air. Check the apples from time to time. They store well, but the longer the storage, the greater the natural loss of moisture and the more likely the chance that even the crispest apple will begin to taste mealy.

What Happens When You Cook This Food When you cook an unpeeled apple, insoluble cellulose and lignin will hold the peel intact through all normal cooking. The flesh of the apple, though, will fall apart as the pectin in its cell walls dissolves and the water inside its cells swells, rupturing the cell walls and turning the apples into applesauce. Commercial bakers keep the apples in their apple pies firm by treating them with calcium; home bakers have to rely on careful timing. To prevent baked apples from melting into mush, core the apple and fill the center with sugar or raisins to absorb the moisture released as the apple cooks. Cloudy apple juice is made simply by chopping or shredding apples and then pressing out and straining the juice. Clear apple juice is cloudy juice filtered to remove solid particles and then treated with enzymes to eliminate starches and the soluble fiber pectin. Medical Uses and/or Benefits As an antidiarrheal. The pectin in apple is a natural antidiarrheal that helps solidif y stool. Shaved raw apple is sometimes used as a folk remedy for diarrhea, and purified pectin is an ingredient in many over-the-counter antidiarrheals. Lower cholesterol levels. Soluble fiber (pectin) may interfere with the absorption of dietary fats, including cholesterol. The exact mechanism by which this occurs is still unknown, but one theory is that the pectins in the apple may form a gel in your stomach that sops up fats and cholesterol, carrying them out of your body as waste. Potential anticarcinogenic effects. A report in the April 2008 issue of the journal Nutrition from a team of researchers at the Universit y of Kaiserslautern, in Germany, suggests that several natural chemicals in apples, including but yrate (produced naturally when the pectin in apples and apple juice is metabolized) reduce the risk of cancer of the colon by nourishing and protecting the mucosa (lining) of the colon.
Adverse Effects Associated with This Food Intestinal gas. For some children, drinking excess amounts of apple juice produces intestinal discomfort (gas or diarrhea) when bacteria living naturally in the stomach ferment the sugars in the juice. To reduce this problem, the American Academy of Pediatrics recommends that children ages one to six consume no more than four to six ounces of fruit juice a day; for children ages seven to 18, the recommended serving is eight to 12 ounces a day. Two dried apricot halves provide 0.6 g dietary fiber, 252 IU vitamin A (11 percent of the R DA for a woman, 8 percent of the R DA for a man), no vitamin C, and 2 mg iron (11 percent of the R DA for a woman, 25 percent of the R DA for a man). The bark, leaves, and inner stony pit of the apricot all contain amyg- dalin, a naturally occurring compound that degrades to release hydrogen cyanide (prussic acid) in your stomach. Apricot oil, treated during processing to remove the cyanide, is marked FFPA to show that it is “free from prussic acid.” Cases of fatal poisoning from apricot pits have been reported, including one in a three-year-old girl who ate 15 apricot kernels (the seed inside the pit). Extract of apricot pits, known medically as Laetrile, has been used by some alternative practitioners to treat cancer on the theory that the cyanide in amygdalin is released only when it comes in contact with beta-glucuronidase, an enzyme common to tumor cells. Scientifically designed tests of amygdalin have not shown this to be true. Laetrile is illegal in the United States.

Buying This Food Look for: Firm, plump orange fruit that gives slightly when you press with your thumb. Avoid: Bruised apricots. Like apples and potatoes, apricots contain polyphenoloxidase, an enzyme that combines with phenols in the apricots to produce brownish pigments that discolor the fruit. Drying. Five pounds of fresh apricots produce only a pound of dried ones. Drying removes water, not nutrients; ounce for ounce, dried apricots have 12 times the iron, seven times the fiber, and five times the vitamin A of the fresh fruit. Three and a half ounces of dried apricots provide 12,700 IU vitamin A, two and a half times the full daily requirement for a healthy adult man, and 6.3 mg of iron, one-third the daily requirement for an adult woman. In some studies with laboratory animals, dried apricots have been as effective as liver, kidneys, and eggs in treating iron-deficiency anemia. To keep them from turning brown as they dry, apricots may be treated with sulfur dioxide. This chemical may cause serious allergic reactions, including anaphylactic shock, in people who are sensitive to sulfites.

Saponin content varies with age. Saikos- aponins are analgesic, antipyretic as well as antitussive; anti-inflammatory on oral administration. In Japan and China, roots have been used traditionally in auto-immune diseases. Saikos- aponins form an ingredient of anti- tumour pharmaceuticals. This effect is due to the sennosides, specifically, due to their active metabolite in the colon, rheinanthrone. Its tuberous roots are an exclusive source of a diterpenoid forskolin which has the unique property of activating almost all hormone sensitive adenylate cyclase enzymes in a biological system. It is useful in the treatment of congestive heart failure, glaucoma, asthma, cancer and in preventing immature greying of hair (Hegde,1997).Agrotechnology: The Coleus group of plants grows in tropical to subtropical situations and in warm temperate climatic zone on mountains of India, Nepal, Burma, Sri Lanka, Thailand and Africa. It comes up well on the sun exposed dry hill slopes from 300m to 1800m altitude. A well drained medium fertile soil is suitable for its cultivation. it is propagated vegetatively through stem and root cuttings. Vine cuttings to a length of 10-15cm from the top portion are most ideal for planting. The land is ploughed or dug to a depth of 15-20cm and ridges are formed 30cm apart. MDMA is structurally similar to endogenous CATECHOLAMINES and produces central and peripheral sympathetic stimulation of alpha and beta ADRENERGIC RECEPTORS. Several fatalities in young people have been attributed to adverse reactions resulting from MDMA use/abuse and possibly accompanying alcohol consumption. The principal effects are increase in pulse, blood pressure, temperature and respiratory rate. Additional complications such as cardiac ARRHYTHMIA, heatstroke-type syndrome, HYPONATRAEMIA and brain haemorrhage may occur. Along with other therapeutic applications, The Ayurvedic Pharmacopoeia of India recommends the aerial root in lipid disorders.Phytosterolin, isolated from the roots, given orally to fasting rabbits at a dose of 25 mg/kg, produced maximum fall in blood sugar level equivalent to 81% of the tolbutamide standard after 4 h. The root bark showed antidiabetic activity in pituitary diabetes and alloxan-induced diabetes.The alcoholic extract of the stem bark also exhibited antidiabetic activity on alloxan-induced diabetes in albino rats, and brought down the level of serum cholesterol and blood urea. This activity is attributed to a glucoside, bengalenoside and the flavonoid glycosides, leucocyanidin and leucopelargonidin. Bengalenoside is half as potent as tolbutamide. The leucopelargonidin glycoside is practically nontoxic and may be useful in controlling diabetes with hyperlipi- demia. Fruits had highest atropine content (0.95%) and shoots the highest hyoscyamine content (0.33%).Flavonoids are active against E. coli, Staphylococcus aureus and Candida albicans (quercetin does not show activity against Candida albicans).The polysaccharide extract from fruits showed antiageing, immunos- timulatory and antiproliferatory activities. The polysaccharide acts as an an- tioxidant and prevented CCl4-induced increases in lipid peroxidases in liver. It can also protect against genetic damage from mutagenic and genotoxic compounds. This activity leads to its potential use in preventing the adverse effects of chemotherapeutic agents.The fruit contain beta-carotene (8 mg/100 g dry weight), also free amino acids (1.0-2.6%); major amino acid is proline.The dried fruit and root bark reduce cholesterol level by preventing its absorption in gastrointestinal tract. A constituent of the root bark, kuko- amine exhibits cholesterol lowering, antihypertensive and hypoglycaemic effects. Hepatoprotective activity is attributed to a cerebroside constituent found in the fruit. The Ayurvedic Pharmacopoeia ofIn- dia indicated the use of the dried stem bark in increased frequency and turbidity of urine, skin diseases, nausea, emesis, asthma, gastroenteritis, giddiness and vertigo.The bitter constituents are present exclusively in the pericarp, not in the kernel as in the case of Neem fruit. Bakayanin has been isolated from the pericarp (bitter in dilutions of 1 in 10,000).The heartwood also yielded bakaya- nin and a lactone, bakalactone. Leaves gave quercitrin and rutin and tetranor- triterpenoids, salanin and vilasinin.An infusion of the bark is effective against ascariasis. The activity resides in the inner bark which is bitter but not astringent (outer bark contains tannins and is astringent).The ethanolic extract of the leaves is fungicidal and antibacterial. The activity is attributed to azadrine and me- liotannic acid.The fruits are considered poisonous to man and animals; contain melianon- inol, melianol, melianone, meliandi- ol, vanillin and vanillic acid. Vanillic acid analogues show micro- and macro-filaricidal activity.Gedunin, present in the plant, inhibits Plasmodium falciparum, while the seed extract does not show anti- malarial activity against P. berghei.The plant exhibited sedative and psychostimulant properties. Antitu- mour and antiviral activities have also been reported. Intraperitoneal administration of partially purified extracts of fresh green leaves reduced the spread of Tacaribe virus (that causes typical encephalitis) to kidneys, liver and brain in inoculated neonatal mice.Dosage: Stem bark—5-10 g (API, Vol. IV.); leaf, seed, root—50- 100 ml decoction; 3-5 g powder. (German Commission E.)The herb contains iridoid glyco- sides, foliamenthin, dihydrofoliamen- thin, menthiafolin and loganin; pyri- dine alkaloids including gentianine; coumarins (scopoletin); phenolic acids (caffeic, with protocatechuic, ferulic, sinapic, vanillic including others; fla- vonoids including rutin, hyperoside.Choleretic action of the herb is attributed to the synergistic action of caffeic and ferulic acids and iridoid glycosides.Scoparone and scopoletin (cou- marins isolated from the aerial parts) exhibit antihepatotoxic, choleretic and cholagogue properties.The rhizomes contain dihydrofolia- menthin, loganin, menthiafolin and a triterpenoid saponin menyantho- side. Nutmeg is used in flatulency, diarrhoea, nausea and vomiting. Mace is used in rheumatism, chronic bowel complaints and asthma. When roasted, both nutmeg and mace are used for diarrhoea, colic, flatulence and dyspepsia.Key application: Dried seed and aril—included among unapproved herbs by German Commission E. Following actions have been considered: antispasmodic, MAO inhibition, inhibition of prostaglandin synthesis.The Ayurvedic Pharmacopoeia of India recommends the kernel of the fruit in spermatorrhoea.An aqueous extract of nutmeg is reported to show anti-secretory activity against E. coli heat-labile enterotoxin; the hexane soluble fraction of the alcoholic extract inhibited the heat-labile and heat-stable-enterotoxin-induced secretory response in animal studies.The hexane extract contains myris- ticin, an anti-inflammatory principle, and licarin-B and dehydro di- isoeugenol which exhibited CNS depressant properties. The extracts of nutmeg decreased kidney prostaglan- din levels in rats. They also inhibited platelet aggregation (due to eugenol and isoeugenol). The anti-inflammatory activity observed in carrage- enan-induced oedema in rats and enhanced vascular permeability in mice, are attributed to myristicin present in mace.Mace also activates hepatic detoxification process. Monomeric and dimer- ic phenyl propanoids (myristicin, de- hydro diisoeugenol) from mace, on p.o. administration in mice, produced suppression of lipid peroxidation in liver.Seeds contain about 0.24% myris- ticin, whereas volatile oil about 3.12%.The resorcinols, malabaricones B and C, isolated from the seed coat (mace) exhibited strong antibacterial and antifungal activities. Neoplasm inhibitors, phenylpropyl derivatives, have been isolated from pulverized mace.Dosage: Endosperm of dried seed (kernel of fruit)—0.5-1.0 g powder. (API, Vol. I.)... myristica fragransNearly three-quarters of neonatal deaths occur during the ?rst week of life. For this reason, increasing emphasis is being laid on this initial period of life. In Britain, in the last four decades of the 20th century, the number of deaths in the ?rst week of life fell dramatically from 13.2 to just over 2.7 per 1,000 live births. The chief causes of deaths in this period are extreme prematurity (less than 28 weeks’ gestation), birth asphyxia with oxygen lack to the brain, and congenital abnormalities. Catnip contains iridoids, tannins and volatile oil, major components being alpha- and beta-nepetalactone (up to 42%), citronellol and geraniol.The catnip response in the domestic cat is being attributed to iridoid lac- tones, nepetalactone, dihydronepeta- lactone, iso-dihydronepetalactone and neonepetalactone. Its reputation as a hallucinogen has been disputed, but a few studies have shown behavioural effects, although weak, in young chicks, rodents and cats. Causes Whatever the causes of obesity, the fact remains that energy intake (in the form of food and drink) must exceed energy output (in the form of activity and exercise) over a suf?ciently long period of time.
Obesity tends to aggregate in families. This has led to the suggestion that some people inherit a ‘thrifty’ gene which predisposes them to obesity in later life by lowering their energy output. Indeed, patients often attribute their obesity to such a metabolic defect. Total energy output is made up of the resting metabolic rate (RMR), which represents about 70 per cent of the total; the energy cost of physical activity; and thermogenesis, i.e. the increase in energy output in response to food intake, cold exposure, some drugs and psychological in?uences. In general, obese people are consistently found to have a higher RMR and total energy output, per person – and also when expressed against fat-free mass – than do their lean counterparts. Most obese people do not appear to have a reduced capacity for thermogenesis. Although a genetic component to obesity remains a possibility, it is unlikely to be great or to prevent weight loss from being possible in most patients by reducing energy intake. Environmental in?uences are believed to be more important in explaining the familial association in obesity.
An inactive lifestyle plays a minor role in the development of obesity, but it is unclear whether people are obese because they are inactive or are inactive because they are obese. For the majority of obese people, the explanation must lie in an excessive energy intake. Unfortunately, it is di?cult to demonstrate this directly since the methods used to assess how much people eat are unreliable. For most obese people it seems likely that the defect lies in their failure to regulate energy intake in response to a variety of cognitive factors (e.g. ease of ?tting of clothes) in the long term.
Unfortunately, it can be possible to identify by the time of their ?rst birthday, many of the children destined to be obese.
Rarely, obesity has an endocrine basis and is caused by hypothyroidism (see under THYROID GLAND, DISEASES OF), HYPOPITUITARISM, HYPOGONADISM or CUSHING’S SYNDROME.
Symptoms Obesity has adverse effects on MORBIDITY and mortality (see DEATH RATE) which are greatest in young adults and increase with the severity of obesity. It is associated with an increased mortality and/or morbidity from cardiovascular disease, non-insulin-dependent diabetes mellitus, diseases of the gall-bladder, osteoarthritis, hernia, gout and possibly certain cancers (i.e. colon, rectum and prostate in men, and breast, ovary, endometrium and cervix in women). Menstrual irregularities and ovulatory failure are often experienced by obese women. Obese people are also at greater risk when they undergo surgery. With the exception of gallstone formation, weight loss will reduce these health risks.
Treatment Creation of an energy de?cit is essential for weight loss to occur, so the initial line of treatment is a slimming diet. An average de?cit of 1,000 kcal/day (see CALORIE) will produce a loss of 1 kg of fat/week and should be aimed for. Theoretically, this can be achieved by increasing energy expenditure or reducing energy intake. In practice, a low-energy diet is the usual form of treatment since attempts to increase energy expenditure, either by physical exercise or a thermogenic drug, are relatively ine?ective.
Anorectic drugs, gastric stapling and jaw-wiring are sometimes used to treat severe obesity. They are said to aid compliance with a low-energy diet by either reducing hunger (anorectic drugs) or limiting the amount of food the patient can eat. Unfortunately, the long-term e?ectiveness of gastric stapling is not known, and it is debatable whether the modest reduction in weight achieved by use of anorectic drugs is worthwhile – although a new drug, ORLISTAT, is becoming available that reduces the amount of fat absorbed from food in the gastrointestinal tract. For some grossly obese patients, jaw-wiring can be helpful, but a regain of weight once the wires are removed must be prevented. These procedures carry a risk, so should be done only if an individual’s health is in danger.... obesity

The essential oil from leaves contained carvacrol, euge- nol, chavicol, allyl catechol, cineole, estragol, caryophyllene, cardinene, p- cymene and eugenol methyl ether.Administration of the leaf extract resulted in decreased tumour burden and tumour incidence and a delay in the onset of mammary tumour in Wistar rats.The alcoholic extract of the leaf stalk is reported to show antispermatogenic and antiandrogenic effect in male albino rats.The essential oil exhibited hypoten- sive, cardiac as well as respiratory depressant and cardiotonic properties.The leaf showed antifungal and antibacterial activity. The Himalayan plants yielded an essential oil (0.26%) with alpha-pinene, dipentene, linalool, cineol, methyl sal- icylate, decyl aldehyde, anisaldehyde, bergapten, eugenol, indole and salicylic and benzoic acids as major constituents. The oil is applied topically in sprains, bruises, sciatica, rheumatism, chest affections and in certain skin diseases.The narcotic action of the bark is attributed to the presence of a yellow oleoresin. Alpha-D-glucan was separated from this mucilage.Leaves gave aucubin and esculetin, in addition to polysaccharides. The whole plant yielded rhamnosidoglyco- side of caffeic acid. Seeds contain 1.1% aucubin. Aucubin exhibits antibacterial activity.
