An epidemic disease is usually infectious from person to person, but not necessarily so since many persons in a locality may simply be exposed to the same cause at one time; for example, outbreaks of lead-poisoning are epidemic in this sense.
The conditions which govern the outbreak of epidemics are poorly understood, but include infected food supplies, such as drinking water contaminated by waste from people with CHOLERA or typhoid fever (see ENTERIC FEVER); milk infected with TUBERCLE bacillus; or ‘fast food’ products contaminated with salmonella. The migrations of certain animals, such as rats, are in some cases responsible for the spread of PLAGUE, from which these animals die in great numbers. Certain epidemics occur at certain seasons: for example, whooping-cough occurs in spring, whereas measles produces two epidemics – as a rule, one in winter and one in March. In?uenza, the common cold, and other infections of the upper respiratory tract, such as sore throat, occur predominantly in the winter.
There is another variation, both as regards the number of persons affected and the number who die in successive epidemics: the severity of successive epidemics rises and falls over periods of ?ve or ten years.... epidemic
The embryo develops upon one side of the ovum, its ?rst appearance consisting of a groove, the edges of which grow up and join to form a tube, which in turn develops into the brain and spinal cord. At the same time, a part of the ovum beneath this is becoming pinched o? to form the body, and within this the endoderm forms a second tube, which in time is changed in shape and lengthened to form the digestive canal. From the gut there grows out very early a process called the allantois, which attaches itself to the wall of the uterus, developing into the PLACENTA (afterbirth), a structure well supplied with blood vessels which draws nourishment from the mother’s circulation via the wall of the womb.
The remainder of the ovum – which within two weeks of conception has increased to about 2 mm (1/12 inch) in size – splits into an outer and inner shell, from the outer of which are developed two covering membranes, the chorion and amnion; while the inner constitutes the yolk sac, attached by a pedicle to the developing gut of the embryo. From two weeks after conception onwards, the various organs and limbs appear and grow. The human embryo at this stage is almost indistinguishable in appearance from the embryo of other animals. After around the middle of the second month, it begins to show a distinctly human form and then is called the fetus. The property of ‘life’ is present from the very beginning, although the movements of the fetus are not usually felt by the mother until the ?fth month.
During the ?rst few days after conception the eye begins to be formed, beginning as a cup-shaped outgrowth from the mid-brain, its lens being formed as a thickening in the skin. It is very soon followed by the beginnings of the nose and ear, both of which arise as pits on the surface, which increase in complexity and are joined by nerves that grow outward from the brain. These three organs of sense have practically their ?nal appearance as early as the beginning of the second month.
The body closes in from behind forwards, the sides growing forwards from the spinal region. In the neck, the growth takes the form of ?ve arches, similar to those which bear gills in ?shes. From the ?rst of these the lower jaw is formed; from the second the hyoid bone, all the arches uniting, and the gaps between them closing up by the end of the second month. At this time the head and neck have assumed quite a human appearance.
The digestive canal begins as a simple tube running from end to end of the embryo, but it grows in length and becomes twisted in various directions to form the stomach and bowels. The lungs and the liver arise from this tube as two little buds, which quickly increase in size and complexity. The kidneys also appear very early, but go through several changes before their ?nal form is reached.
The genital organs appear late. The swellings, which form the ovary in the female and the testicle (or testis) in the male, are produced in the region of the loins, and gradually descend to their ?nal positions. The external genitals are similar in the two sexes till the end of the third month, and the sex is not clearly distinguishable till late in the fourth month.
The blood vessels appear in the ovum even before the embryo. The heart, originally double, forms as a dilatation upon the arteries which later produce the aorta. These two hearts later fuse into one.
The limbs appear at about the end of the third week, as buds which increase quickly in length and split at their ends into ?ve parts, for ?ngers or toes. The bones at ?rst are formed of cartilage, in which true bone begins to appear during the third month. The average period of human gestation is 266 days – or 280 days from the ?rst day of the last menstrual period. The average birth weight of an infant born of a healthy mother (in the UK) is 3,200 g (see table).

The principle of ?breoptics in medicine is that a light from a cold light source passes down a bundle of quartz ?bres in the endoscope to illuminate the lumen of the gastrointestinal tract or the bronchi. The re?ected light is returned to the observer’s eye via the image bundle which may contain up to 20,000 ?bres. The tip of the instrument can be angulated in both directions, and ?ngertip controls are provided for suction, air insu?ation and for water injection to clear the lens or the mucosa. The oesophagus, stomach and duodenum can be visualised; furthermore, visualisation of the pancreatic duct and direct endoscopic cannulation is now possible, as is visualisation of the bile duct. Fibreoptic colonoscopy can visualise the entire length of the colon and it is now possible to biopsy polyps or suspected carcinomas and to perform polypectomy.
The ?exible smaller ?breoptic bronchoscope has many advantages over the rigid tube, extending the range of view to all segmental bronchi and enabling biopsy of pulmonary parenchyma. Biopsy forceps can be directed well beyond the tip of the bronchoscope itself, and the more ?exible ?breoptic instrument causes less discomfort to the patient.
Fibreoptic laparoscopy is a valuable technique that allows the direct vizualisation of the abdominal contents: for example, the female pelvic organs, in order to detect the presence of suspected lesions (and, in certain cases, e?ect their subsequent removal); check on the development and position of the fetus; and test the patency of the Fallopian tubes.

Habitat: Hedges and woods.
Features ? The upright, woody but slender stem, branching from the upper part only, attains a height of between one and two feet. The leaves are stalkless and elliptical in shape, about half an inch long, grow in pairs on opposite sides of the stem and branches and, in addition to the transparent dots noticed above, are sometimes marked with black spots on the under side. Numerous bright yellow flowers, dotted and streaked with dark purple, cluster, in June and July, at the ends of side branches and stem. A bitter, astringent taste is remarked.Action: Expectorant, diuretic and astringent.
Indicated in coughs, colds, and disorders of the urinary system. It was prescribed more often by the English herbal school of a hundred years ago than it is to-day, and was noticed as far back as Culpeper for "wounds, hurts and bruises." Indeed, an infusion of the fresh flowers in Olive oil, to make the "Oil of St. John's Wort," is still used as an application to wounds, swellings, and ulcers. Internally, the infusion of 1 ounce of the herb to 1 pint of boiling water is taken in wineglass doses.In America St. John's Wort grows freely in the cornfields, which proximity was held by Tilke to operate beneficially upon both herb and grain. Discussing American wheat which has grown among quantities of St. John's Wort he tells us ? "It is well known, by almost every baker who works in his business, that this flour improves the quality of the bread, byhaving a small quantity of it in every batch, particularly in seasons when the English flour is of inferior quality. A clever author informs us that it contains one-fourth more gluten than our famous wheats grown in Gloucestershire, known by the name of 'rivets.' " Tilke was himself a baker in his early days.... st. john's wortStructure The substance of the tongue consists almost entirely of muscles running in various directions. The tongue also has numerous outside attachments: one muscle on each side unites it to the lower jaw-bone just behind the chin, and this muscle serves to protrude the tongue from the mouth; other muscles, which retract the tongue, attach it to the hyoid bone, the larynx, the PALATE, and the styloid process on the base of the SKULL.
The mucous membrane on the undersurface of the tongue is very thin. In the middle line, a fold of mucous membrane, the frenum, passes from the under-surface to the ?oor of the mouth; when this frenum is attached too far forwards towards the tip of the tongue, the movements of the organ are impeded – the condition being known as tongue-tie. On the upper surface or dorsum of the tongue, the mucous membrane is thicker, and in its front two-thirds is studded with projections or papillae, most of which are conical. Some of them end in long ?laments, and are then known as ?liform papillae. On the tip, and towards the edges of the tongue, small, red, rounded fungiform papillae are seen, which act as end-organs for the sense of taste – as do circumvallate papillae, each of which is surrounded by a trench along which open numerous taste-buds. These taste-buds are also found in the fungiform papillae, scattered over
the throat, FAUCES, and palate. Five nerves, originating from the ?fth, seventh, ninth, tenth and 12th cervical nerves supply the tongue.... tongue
Ankyloglossia or tongue-tie is a rare disorder in which the frenum or band connecting the lower surface of the tongue to the ?oor of the mouth is so short or tight that the tongue cannot be protruded. Surgery can remedy the defect. It is easy to overdiagnose and is not a common cause of di?culty in feeding at birth or speech defects in infancy.
Gross enlargement of the tongue can make speech indistinct or make swallowing and even breathing di?cult. This is known as macroglossia and may be such that the tongue is constantly protruded from the mouth. The cause may be CONGENITAL, as in severe cases of DOWN’S (DOWN) SYNDROME, or it may occur as a result of ACROMEGALY or be due to abnormal deposits as in AMYLOIDOSIS.
A marked tremor of the tongue when protruded may be seen in various neurological diseases, but may be caused by alcoholism.
After a STROKE involving the motor nerve centre, the control of one side of the tongue musculature will be lost. This will result in the protruded tongue pointing to the side of the body which is paralysed. The sense of taste on one side of the tongue may also be lost in some diseases of the brain and facial nerve.
The presence of fur on the tongue may be obvious and distressing. This is due to thickening of the super?cial layers of the tongue which may appear like hairs which trap food debris and become discoloured. Furring is common during fever and as a result of mouth-breathing and smoking.
In some conditions the tongue may appear dry, red and raw (GLOSSITIS). An in?amed beefy tongue is characteristic of pellagra, a disease caused by de?ciency of NICOTINIC ACID in the diet. A magenta-coloured tongue may be seen when there is a lack of RIBOFLAVIN.
Ulcers of the tongue are similar to those elsewhere in the mouth. The most common are aphthous ulcers which are small, red and painful and last for about ten days. They are associated with stress, mild trauma (such as from jagged teeth), and occasionally with folic acid and vitamin B12 de?ciency. Ulcers of the tongue are sometimes found in patients with chronic bowel disease.... tongue, disorders of
Those in the row nearest the hand are the trapezium, trapezoid, capitate and hamate. These latter articulate with the metacarpal bones in the hand and are closely bound to one another by short, strong ligaments; and the wrist-joint is the union of the composite mass thus formed with the RADIUS and ULNA in the forearm. The wrist and the radius and ulna are united by strong outer and inner lateral ligaments, and by weaker ligaments before and behind, whilst the powerful tendons passing to the hand and ?ngers strengthen the wrist.
The joint can move in all directions, and its shape and many ligaments mean that it rarely dislocates – although stretching or tearing of some of these ligaments is a common accident, constituting a sprain. (See JOINTS, DISEASES OF.) In?ammation of the tendon-sheaths may occur as a result of injury or repetitive movement (see UPPER LIMB DISORDERS). A fairly common condition is the presence of a GANGLION, in which an elastic swelling full of ?uid develops on the back or front of the wrist in connection with the sheaths of the tendons. Uses: To strengthen adrenal glands weakened by intake of steroids (cortisone, etc). Stress, mental exhaustion, depression. Helps to prevent inflammation of stomach and intestines in cases of toxicity, allergy and infection. Colitis, Gastritis, Gastric ulcer. “Borage cheers the heart and raises drooping spirits” (Dioscorides). Old Italian remedy to increase breast milk in nursing mothers. Chronic catarrh. Borage oil is combined with Evening Primrose oil to reduce cholesterol deposits. Leaky-gut syndrome (Borage seed oil). External use said to defer wrinkling and skin dryness of old age. Used internally under directions of a qualified practitioner.
Preparations: Thrice daily.
Tea: 1 teaspoon to each cup boiling water; infuse 15 minutes. Half-1 cup.
Home tincture: 1oz cut herb to 20oz 45 per cent alcohol (Vodka, etc) in wide-neck bottle. Macerate and shake daily for 4 days. Filter. Bottle. Dose: 1-2 teaspoons in water.
Liquid Extract: half-1 teaspoon.

1. It is illegal for anyone not a registered medical practitioner to attempt to procure an abortion: a member must not knowingly administer an abortifacient or known uterine muscle stimulant remedies to a pregnant patient, nor instruments for the purpose of procuring an abortion, nor assist in any illegal operation.
2. It is required that any intimate examinations on a patient of the opposite sex be conducted in the presence of a relative of the patient or a suitable assistant.
3. A member must not treat or prescribe any remedy for gonorrhoea, syphilis, or urinary affections of a venereal nature.
4. It is the duty of the practitioner to notify the District Medical Officer regarding any disease on the current list of notifiable diseases. In cases of industrial poisoning or accident the local district branch of the Health and Safety Executive should be notified.
5. A member must consider very carefully the implications of recommending a course of treatment contrary to the advice of the patient’s registered medical practitioner or of not recommending referral to a registered medical practitioner in the case of serious disease or uncertain diagnosis. Members must be aware of their vulnerability in law on this issue and must ensure in such a case that all available information is given to the patient and that the patient makes the final decision without coercion.
6. A parent or supervising adult must be present at any treatment or examination of a child under the age of 16, or of a mentally-retarded patient.
7. The Data Protection Act means that any practitioner keeping patient’s data on computer file must register under the terms of the Act.
8. A member must become familiar with the terms of the Medicine’s Act 1968 and subsequent statutory instruments, notably the Medicines (Retail Sale or Supply of Herbal Remedies) Order 1977. Particular care should be taken to become familiar with the statutory maximum doses of those remedies listed in Schedule III of the latter order. Detailed records of prescriptions and dispensing must also be kept.
9. The Medicines Act further states that to claim exemptions from the restrictions on the supply of certain herbal remedies, the practitioner should supply said remedies from premises occupied by the practitioner and able to be closed so as to exclude the public.
10. The Medicines Act adds that to claim the said exemptions, the person supplying the remedy “sells or supplies it for administration to a particular person after being requested by or on behalf of that person and in that person’s presence to use his own judgement as to the treatment required”. The member should avoid treatment through telephone or postal contact, although repeat prescriptions may be supplied on this basis for a limited period.
11. Dispensing and labelling of medicines should at least comply with the terms of the Medicines Act. All medicines should be labelled to clearly indicate the correct dosage or other directions for use (especially for those remedies subject to a statutory maximum dose), and with the name and address of the practitioner and the date of dispensing.
12. A member should never claim verbally or in print to be able to cure any life-threatening or serious disease.
13. The distribution or display of letter headings, business cards or practice information should be compatible with the highest professional medical standards. ... code of practice
Requirements. Basin, towel, kettle of water, piece of cotton wool, oilcloth, binder and safety pins. Method: Place towel across basin; lay flannel on towel and press down. Pour on hot herbal infusion, decoction or tincture and thoroughly soak. Bring together ends of the towel and twist hands in opposite directions to squeeze out surplus fluid. Untwist towel, free the flannel, shake it out and apply direct to the skin. Smear affected skin with olive oil before application. Add a layer of cotton wool; cover with plastic or oilskin; bind in position and pin securely. Moisten compress when dry, every half hour or less.

Misleading claims and the use of such words as “organic”, “wholesome”, “natural” or “biological” cannot be accepted on product labels. The Licensing Authority treats herbal manufacturers no differently than manufacturers of allopathic products for serious conditions.
The Advertising Standards Authority does not allow quotation of any medicinal claims, except where a Product Licence (PL) has been authorised by the Licensing Authority.
All labels must include: Name of product (as on Product Licence), description of pharmaceutical form (tablet, mixture etc), Product Licence No., Batch No., quantity of each active ingredient in each unit dose in metric terms; dose and directions for use; quantity in container (in metric terms); “Keep out of reach of children” or similar warning; Name and address of Product Licence Holder; expiry date (if applicable); and any other special warnings. Also to appear: excipients, method/route of administration, special storage instructions, and precautions for disposal, if any.
Where licences are granted, the following words should appear on the label of a product: “A herbal product traditionally used for the symptomatic relief of . . .”. “If symptoms persist see your doctor.” “Not to be used in pregnancy” (where applicable). “If you think you have . . . consult a registered medical practitioner before taking this product.” “If you are already receiving medical treatment, tell your doctor that you are taking this product.” These warnings are especially necessary should symptoms persist and be the start of something more serious than a self-limiting condition.
Herbal preparations should be labelled with the additives and colourings they contain, if any. This helps practitioners avoid prescribing medicines containing them to certain patients on whom they may have an adverse reaction.
Labels of medicinal products shall comply with the Medicines (Labelling) Regulations 1976 (SI 1976 No. 1726) as amended by the Medicines (Labelling) Amendment Regulations 1977 (SI 1977 No. 996), the Medicines (Labelling) Amendment Regulations 1981 (SI 1981 No. 1791) and the Medicines (Labelling) Amendment Regulations 1985 (SI 1985 No. 1558).
Leaflets issued with proprietory medicinal products shall comply with the requirements of the Medicines (Leaflets) Regulations 1977 (SI 1977 No. 1055).

If benefits outweigh risks, radiotherapy may be used to treat noncancerous diseases; for example, part of an overactive thyroid gland (see thyrotoxicosis) may be destroyed using radioactive iodine.
Radiotherapy is usually performed on an outpatient basis. X-rays (or sometimes electrons) produced by a machine called a linear accelerator are aimed at the tumour from many directions. This produces a large enough dose of radiation to destroy the tumour. Alternatively, a source of radiation, in the form of tiny pellets, is inserted into the tumour through a hollow needle (see interstitial radiotherapy) or into a body cavity (see intracavitary therapy). Radioactive iodine used to treat thyrotoxicosis is given in liquid form and drunk through a straw.
There may be unpleasant side effects, including fatigue, nausea and vomiting, and loss of hair from irradiated areas. Rarely, there may be reddening and blistering of the skin.... radiotherapy
FAMILY: Rutaceae
SYNONYMS: C. vulgaris, C. bigaradia, orange blossom, orange flower, neroli bigarade.
GENERAL DESCRIPTION: An evergreen tree up to 10 metres high with glossy dark green leaves and fragrant white flowers. There are two flowering seasons when the blossom is picked, one in May and another in October (in mild weather). See also bitter orange.
DISTRIBUTION: Native to the Far East, but well adapted to the Mediterranean climate. Major producers include Italy, Tunisia, Morocco, Egypt, America and especially France.
OTHER SPECIES: The sweet orange (C. aurantium var. dulcis) is also used to make an absolute oil called neroli Portugal or neroli petalae – however, it is less fragrant and considered of inferior quality.
HERBAL/FOLK TRADITION: This oil was named after a princess of Nerola in Italy, who loved to wear it as a perfume. Orange flowers have many folk associations. They were used in bridal bouquets and wreaths, to calm any nervous apprehension before the couple retired to the marriage bed.
On the Continent an infusion of dried flowers is used as a mild stimulant of the nervous system, and as a blood cleanser. The distillation water, known as orange flower water, is a popular cosmetic and household article.
ACTIONS: Antidepressant, antiseptic, antispasmodic, aphrodisiac, bactericidal, carminative, cicatrisant, cordial, deodorant, digestive, fungicidal, hypnotic (mild), stimulant (nervous), tonic (cardiac, circulatory).
EXTRACTION: 1. A concrete and absolute are produced by solvent extraction from the freshly picked flowers. 2. An essential oil is, produced by steam distillation from the freshly picked flowers. An orange flower water and an absolute are produced as a byproduct of the distillation process.
CHARACTERISTICS: 1. The absolute is a dark brown or orange viscous liquid with a fresh, delicate yet rich, warm sweet-floral fragrance; very true to nature. It blends well with jasmine, benzoin, myrrh and all citrus oils. 2. The oil is a pale yellow mobile liquid (darkening with age) with a light, sweet-floral fragrance and terpeney topnote. Blends well with virtually all oils: chamomile, coriander, geranium, benzoin, clary sage, jasmine, lavender, rose, ylang ylang, lemon and other citrus oils.
PRINCIPAL CONSTITUENTS: Linalol (34 per cent approx.), linalyl acetate (6–17 per cent), limonene (15 per cent approx.), pinene, nerolidol, geraniol, nerol, methyl anthranilate, indole, citral, jasmone, among others.
SAFETY DATA: Non-toxic, non-irritant, nonsensitizing, non-phototoxic.
AROMATHERAPY/HOME: USE
Skin care: Scars, stretch marks, thread veins, mature and sensitive skin, tones the complexion, wrinkles.

OTHER USES: Neroli oil and orange flower water are used to flavour pharmaceuticals. The absolute is used extensively in high-class perfumery work, especially oriental, floral and citrus blends; also as a fixative. The oil is used in eau-de-cologne and toilet waters (traditionally with lavender, lemon, rosemary and bergamot). Limited use as a flavour ingredient in foods, alcoholic and soft drinks.... neroli
